© Сахарный диабет, 2015ольшая распространенность сахарного диа-бета (СД), а также ежегодный рост заболевае-мости во всем мире носит характер эпидемии, несмотря на значительные успехи фармакологии в об-ласти создания гипогликемизирующих средств. В РФ на январь 2013 г. зафиксировано 3,779 млн больных СД. Из них больше половины -это люди трудоспособного возраста. При этом реальное число больных СД в России может быть в 3-4 раза больше [1].Пропорционально росту заболеваемости диабетом со-ответственно растет число его осложнений, которые могут иметь тяжелые последствия. Почти пятая часть пациентов с СД (17,63%) имеет проблемы со зрением. Слепота на-ступает в 25 раз чаще у лиц с диабетом [2]. Диабетическая нефропатия является ведущей причиной терминальной почечной недостаточности и занимает 20-44% всех диа-лизных мест. Наличие СД 2 типа (СД2) повышает вероят-ность развития инсульта в 2-6 раз. Сосудистая патология головного мозга на фоне инсулинорезистентности и ги-пергликемии при СД2 являются одной из основных при-чин развития диабетической нейропатии [3,4].По приблизительным подсчетам, мировые затраты на борьбу с СД достигли 471 млрд долларов США [5].Имеющиеся гипогликемизирующие средства и фарма-котерапевтические стратегии не в полной мере решают проблемы лечения СД2 и его осложнений. Поэтому ак-туальность поиска новых мишеней и терапевтических стратегий лечения и профилактики СД не вызывает со-мнений. The search for new drugs for the treatment of type 2 diabetes mellitus (T2DM) and obesity remains an urgent problem. Drugs with influence on incretin system are widely used in the treatment of T2DM and obesity, since in addition to the hypoglycemic action of their inherent hypophagic effects. With the discovery of GPR119 receptor, there is the opportunity to pharmacological stimulation of endogenous secretion of incretins. Compound ZB-16 is active GPR119 agonist with IC50=7 nM. Its activation leads to increased secretion of the major incretins (GLP-1 and GIP), which are able to influence glucose metabolism and feeding behavior.Aims — to study the effect of GPR 119 receptor agonist compounds ZB-16 on blood glucose, body weight and food intake in rats with obesity.Material and methods.Male rats with initial weight 390—400 g were fed with high-carbohydrate and high-fat diet. During the next four weeks the animals orally received ZB-16 (1 mg/kg) and metformin (400 mg/kg) and then we assessed the level of water and food consumption, blood glucose levels, and performed oral glucose tolerance test (OGTT).Results.Compound ZB-16 and metformin reduced fasting blood glucose levels and weight of experimental animals, while the control rats gained weight. GPR119 agonist is more pronounced than metformin reduced the area under the curve «glucose of concentration—time» during the OGTT.Conclusions.Novel GPR119 agonist — ZB-16 is comparable to metformin in hypoglycemic and anorexigenic effect in animals with obesity caused high-carbohydrate and high-fat diet.

При его активации повышается секреция инкретинов и возрастает глюкозозависимая секреция инсулина, что важно для эффективного контроля постпрандиальной гликемии и снижения риска гипогликемий. Цель исследования -сравнение гипогликемической активности ингибитора ДПП-4 (ситаглиптина) и нового агониста GPR119 (соединения ZB-16) у животных с экспериментальной формой сахарного диабета 2-го типа (СД2).

Background. GPR119 receptor is a promising target for novel antidiabetic drugs development because of its agonists in addition to hypoglycemic effects have cytoprotective effect on beta cells.Aim — to assess the effect of the novel GPR119 receptor agonist and sitagliptin on carbohydrate metabolism and morphological structure of the pancreas in animals with experimental type 2 diabetes.Material and methods. The study was performed in Wistar rats with streptozotocin-nicotinamide-induced diabetes. Serum insulin was determined by ELISA (Rat Insulin, (INS) ELISA Kit, 96 CSB). Immunohistochemical studies were performed using mouse monoclonal antibodies to insulin (clone 1G4, GeneTex) with determination of the absolute and relative area of immunopositive material (beta-cells) of the pancreatic islets.Results. 28-day treatment of animals with diabetes by GPR119 receptor agonist — ZB-16 (dipiaron) (1 mg/kg, per os, daily) and an inhibitor of the DPP-4 (sitagliptin) led to a reduction of fasting hyperglycemia and improve its glucose tolerance and enhance basal and stimulated insulin secretion relative to the control without treatment. Morphometric assessment of islets revealed that animals treated with the ZB-16 and sitagliptin have significantly higher absolute and relative islets area, as well as the perimeter of the insulin-positive material compared to islets of rats without treatment.Conclusion. Course administration of GPR119 receptor agonist to animals with streptozotocin-nicotinamide-induced diabetes has a pronounced antidiabetic effect consists in reducing fasting plasma glucose, improve glucose utilization, increase basal and stimulated insulin secretion, as well as increasing the area of the insulin-positive material in islets. Antidiabetic action of novel GPR119 receptor agonist, was comparable to that of sitagliptin.
BACKGROUND: Metabolic syndrome and obesity are often precursors of type 2 diabetes mellitus (DM), and current recommendations indicate the advisability of early initiation of drug therapy at the stage of prediabetes. Drugs with incretin activity are one of the priority groups for monotherapy of type 2 diabetes in the onset of the disease, and certain drugs are used to treat obesity. GPR119 agonists increase the secretion of endogenous incretins, and their effectiveness in the treatment of type 2 diabetes and obesity in mono- and combination therapy is currently being actively studied. AIM. To evaluate of the effect of administration of a GPR119 receptor agonist, its combination with metformin or sitagliptin on body weight, food intake and glycemia in rats under a high-calorie diet. MATERIALS AND METHODS: The study was conducted on 56 outbred female rats aged 78 months and an initial weight of 305320 g. Compound ZB-16 is a highly active GPR119 receptor agonist (EC50 = 7 nM). For 12 weeks, the animals were kept on a high-fat and carbohydrate diet and at the same time received the compound ZB-16, metformin and sitagliptin, or its combination (ZB-16 + metformin and ZB-16 + sitagliptin). During the experiment, the weight of the animals, the mass of feed eaten, as well as the level of glycemia after 6 hours of fasting and with an oral glucose load were assessed. RESULTS: In animals of the control group that were on a high-calorie and fatty diet for 12 weeks, an increase in body weight, glycemia and a decrease in the rate of glucose utilization were observed. The introduction of the GPR119 agonist (ZB-16) for 12 weeks led to a significant reduction in the amount of food consumed, limited weight gain and prevented the development of carbohydrate metabolism disorders. The addition of sitagliptin and especially metformin to therapy with the GPR119 agonist significantly increased the effectiveness of therapy compared to the control group, which was expressed in the normalization of animal body weight and glycemia (p 0.05). CONCLUSIONS: The introduction of a combination of the GPR119 agonist (compound ZB-16) with metformin and sitagliptin is more effective than monotherapy in terms of weight gain, food intake, and also prevents the development of carbohydrate metabolism disorders in animals when kept on a high-fat and carbohydrate diet.

В.И. Петров) Цель . Проанализировать влияние сулодексида на эндотелий-зависимую вазодилатацию (ЭЗВД) мозговых сосудов у животных с экспериментальным сахарным диабетом (ЭСД). Материалы и методы . Исследование было выполнено на 45 крысах-самцах линии Wistar, массой 180-210 г, разделенных на 3 равные группы: группа интактных животных, группа с ЭСД, получавших физиологический раствор, группа животных с ЭСД, которым per os в течение 4 недель вводился сулодексид -30 ЕВЛ. Эндотелиальная дисфункция (ЭД) моделировалась развитием ЭСД. ЭСД вызывали путем однократного в/в введения стрептозотоцина (50 мг/кг). Через месяц осуществлялось тестирование эндотелий-зависимой и эндотелий-независимой вазодилатации путем регистрации кровотока в сосудах мозга в проекции средней мозговой артерии, а также индексов Пурселло и Госслинга до и после внутривенного введения ацетилхолина (АХ) («Acros organics», США) (0,001 мг/кг), L-аргинина (300 мг/кг) («Acros organics», США), нитроглицерина (НГ) (МТХ, Москва) (0,007 мг/кг), нитро-L-аргинина (10 мг/кг) («Acros organics», США). Иммуногистохимическую подготовку образцов и исследование экспрессии антител к е-NOS и эндотелину-1 проводили по рекомендациям производителя используемых в данной работе антител (компания Novocastra™). Результаты всех опытов обрабатывались методами базисного статистического анализа в программе STATISTICA/7.0 фирмы StatSoft, Inc. (США) для Windows. Результаты . У животных с ЭСД по сравнению с животными интактной группы при введении ацетилхолина отмечалось значительное снижение ЭЗВД (в 2,58 раза) и практически неизменная эндотелий-независимая вазодилатация (при введении нитроглицерина); наблюдалось достоверное повышение индексов сопротивления сосудов (индекс Пурселло) и эластичности сосудистой стенки (индекс Госслинга). У животных с ЭСД, получавших в течение 4 недель сулодексид, наблюдалось более низкое сопротивление и более высокая эластичность мозговых сосудов (индексы Пурселло и Госслинга), по сравнению с животными с ЭСД без фармакологической коррекции.
