ВСТУП ВІЛ-інфекція-хронічне інфекційне захворювання, в основі якого лежить ураження імунної системи з відповідними патологічними змінами, зокрема в системі гемостазу [1, 20, 21], що призводить до акушерських та перинатальних ускладнень. Ризик розвитку венозних тромбоемболій у ВІЛ-інфікованих пацієнтів до 10 разів вищий [2, 3], ніж у загальній популяції. При прогресуванні фізіологічної вагітності відбуваються зміни в системі гемостазу, спричинені уповільненням кровотоку в глибоких венах, підвищенням гідростатичного тиску у венах нижніх кінцівок, гіперестрогенемією і конверсією показників у всіх ланках системи згортання крові [4-8, 22-24]. АНАЛІЗ ЛІТЕРАТУРНИХ ДАНИХ На даний час відомі результати досліджень, що висвітлюють вплив ВІЛ на стан системи гемостазу. Автори доводять, що ризик розвитку серцево-судинно захворювань на 61,0% вище саме у ВІЛ-інфікованих пацієнтів [9]. За наявності ВІЛ-інфекції темпи змін у системі гемостазу в бік гіперкоагуляції і гіперагрегації тромбоцитів випереджають такі при фізіологічній вагітності [10]. З іншого боку, при ВІЛ-інфекції внаслідок порушень реологічної властивості крові існує ризик розвитку геморагічних ускладнень [11-13]. ВІЛ-інфекція та зміни в системі гемостазу у вагітних обумовлюють розвиток тромбоутворень або, навпаки, геморагічних ускладнень, що призводить до перинатальних та акушерських проблем [14-16]. Синдром дисемінованого внутрішньосудинного згортання крові (ДВЗ-синдром)-набуте захворювання, яке характеризується внутрішньосудинним згортанням крові в дрібних судинах без певної локалізації, що виникає під впливом різних причин, призводить до блокування мікроциркуляторного русла з розвитком поліорганної недостатності, а при ураженні факторів згортання і тромбоцитів-до коагулопатичних кровотеч [17, 25-27]. Київ 2 Київський міський центр репродуктивної та перинатальної медицини ОСОБЛИВОСТІ СТАНУ СУДИННО-ТРОМБОЦИТАРНОЇ ЛАНКИ СИСТЕМИ ГЕМОСТАЗУ У ВІЛ-ІНФІКОВАНИХ ВАГІТНИХ ТА РОДІЛЬ Зміни в системі гемостазу, що спостерігаються у вагітних з ВІЛ, призводять до тромбогеморагічних ускладнень для матері і плода, які проявляються акушерськими та перинатальними ускладненнями. Мета дослідження. Вивчити зміни в судинно-тромбоцитарній ланці системи гемостазу у ВІЛ-інфікованих вагітних та роділь в залежності від клінічної стадії та початку прийому ВААРТ. Матеріали та методи. У 150 ВІЛ-інфікованих вагітних та роділь оцінювали стан судинно-тромбоцитарної ланки системи гемостазу, а також об'єм крововтрати в пологах. Результати дослідження та їх обговорення. В ІІ триместрі у вагітних з ІІ та ІІІ клінічними стадіями ВІЛ спостерігається достовірно підвищена активність судинно-тромбоцитарної ланки системи гемостазу при визначенні агрегаційної функції тромбоцитів та фактору Вілебранду. В ІІІ триместрі у цих пацієнток відмічене підвищення активності судинно-тромбоцитарної ланки та зниження кількості тромбоцитів і тромбокриту на тлі підвищення MPV та PDW у жінок з ІІІ клінічною стадією ВІЛ-інфекції. У роділь з ІІІ клінічною стадією ВІЛ-інфекції спостерігається зниження кількості тромбоцитів, ознаки тромбоцитопатії (гальмування агрегаційної функції тромбоцитів при індукції АДФ, колагеном та ристоміцином і підвищення агрегації тромбоцитів адреналіном). У роділь, шо розпочали прийом ВААРТ під час даної вагітності відмічене достовірне зниження рівня тромбоцитів на тлі зниження показника тромбоцитопоезу. Рівень фактору Вілебранду достовірно перевищував у жінок ІІ та ІІІ групи та мав тенденцію до збільшення у роділь ІА групи. Висновки. Під час вагітності у жінок з ІІ та ІІІ клінічними стадіями ВІЛ-інфекції відмічається підвищена активність судинно-тромбоцитарної ланки системи гемостазу. В пологах у ВІЛ-інфікованих з ІІІ клінічною стадією та жінок, що розпочали прийом ВААРТ під час даної вагітності спостерігається схильність до кровотеч.
