ВСТУП ВІЛ-інфекція-хронічне інфекційне захворювання, в основі якого лежить ураження імунної системи з відповідними патологічними змінами, зокрема в системі гемостазу [1, 20, 21], що призводить до акушерських та перинатальних ускладнень. Ризик розвитку венозних тромбоемболій у ВІЛ-інфікованих пацієнтів до 10 разів вищий [2, 3], ніж у загальній популяції. При прогресуванні фізіологічної вагітності відбуваються зміни в системі гемостазу, спричинені уповільненням кровотоку в глибоких венах, підвищенням гідростатичного тиску у венах нижніх кінцівок, гіперестрогенемією і конверсією показників у всіх ланках системи згортання крові [4-8, 22-24]. АНАЛІЗ ЛІТЕРАТУРНИХ ДАНИХ На даний час відомі результати досліджень, що висвітлюють вплив ВІЛ на стан системи гемостазу. Автори доводять, що ризик розвитку серцево-судинно захворювань на 61,0% вище саме у ВІЛ-інфікованих пацієнтів [9]. За наявності ВІЛ-інфекції темпи змін у системі гемостазу в бік гіперкоагуляції і гіперагрегації тромбоцитів випереджають такі при фізіологічній вагітності [10]. З іншого боку, при ВІЛ-інфекції внаслідок порушень реологічної властивості крові існує ризик розвитку геморагічних ускладнень [11-13]. ВІЛ-інфекція та зміни в системі гемостазу у вагітних обумовлюють розвиток тромбоутворень або, навпаки, геморагічних ускладнень, що призводить до перинатальних та акушерських проблем [14-16]. Синдром дисемінованого внутрішньосудинного згортання крові (ДВЗ-синдром)-набуте захворювання, яке характеризується внутрішньосудинним згортанням крові в дрібних судинах без певної локалізації, що виникає під впливом різних причин, призводить до блокування мікроциркуляторного русла з розвитком поліорганної недостатності, а при ураженні факторів згортання і тромбоцитів-до коагулопатичних кровотеч [17, 25-27].
