В статье представлен проект клинических ре-комендаций по оказанию реанимационной помощи детям, нуждающимся в межгоспиталь-ной транспортировке, разработанный с учетом территориальных особенностей Российской Федерации. Детально рассмотрены правовые аспекты организации службы межгоспитальной транспортировки, возможные побочные эффек-ты транспортировки, дана характеристика авиа-ционного и наземного санитарного транспорта, отражены его основные достоинства и недостат-ки. Предложена оценка риска транспортировки, выраженная в рангах, соответствующих продол-жительности безопасного времени транспорти-ровки. Приведены оригинальные шкалы оценки риска транспортировки, основанные на необхо-димости проведения экстренной регидратации, инотропной и вазопрессорной терапии, а также респираторной поддержки. Особое внимание уделено подготовке детей в критическом со-стоянии к межгоспитальной транспортировке, изложен детальный протокол мероприятий интенсивной терапии, мониторинга и ухода на этапе транспортировки. Описаны целевые пока-затели стабилизации состояния пациента в бли-жайшие шесть часов после транспортировки. Аноректальные аномалии являются одними из наиболее частых врожденных пороков развития у детей, требующих экстренных хирургических вмешательств� Цель: анализ имеющихся отечественных и зарубежных публикаций, посвященных особенностям анестезии при аноректальных аномалиях у детей� Материалы и методы. Поиск публикаций осуществляли в реферативных базах РИНЦ, PubMed и Scopus по ключевым словам: аноректальные аномалии, анестезия, регионарная анестезия, дети� Найдено 105 публикаций, для анализа отобрано 49 исследований� Результаты. С целью анестезиологического обеспечения оперативных вмешательств при коррекции аноректальных аномалий используется как общая, так и регионарная анестезия� Среди регионарных методик чаще всего применяют спинальную и каудальную анестезию� Наиболее эффективным вариантом анестезии при операциях на аноректальной области является седловидная анестезия, поскольку она сопровождается значительным снижением тонуса анального сфинктера� Комбинированное использование общей анестезии и каудальной анальгезии способствует уменьшению объема интраоперационной кровопотери и раннему восстановлению функций кишечника� Заключение. Необходимо проведение дальнейших исследований с целью создания протоколов быстрого восстановления в послеоперационном периоде после операций по поводу аноректальных пороков развития у детей�
The analysis of course of pneumonia in 33 children with various forms of infantile cerebral palsy (ICP) had been conducted. It is determined that more severe course of pneumonia with distinct, long‐lasting symptoms of respiratory failure and fever was observed in double hemiplegia with spastic tetraplegia and distinct muscle hypertension; atonic‐astatic with low muscle tone and impairment of cortical‐subcortical connections; hyperkinetic with hyperkinesia. These patients had stayed long in the intensive care department (an average of 29 ± 3,2) with artificial lung ventilation. Persistence and distinction of respiratory symptoms were supported by the presence of GORD, microaspiration, vomiting, impaired swallowing reflex (85.7%). This entails that all patients with ICP should regularly examine lung function, even in the absence of respiratory symptoms, examine for finding the presence of pathology of aspiration, select special breathing techniques in ineffective cough reflex.
INTRODUCTION: Anorectal anomalies are one of the most common congenital malformations in children that require corrective surgical interventions. OBJECTIVE: To study the influence of anesthesia on the hemodynamic indices in the correction of anorectal anomalies in children. MATERIALS AND METHODS: A prospective single-center randomized study. Sixty children were examined, all underwent posterior sagittal anorectoplasty according to Penya. Children were divided into two groups: group 1 - combined anesthesia (n = 30), group 2 - general with regional anesthesia (n = 30). The study included five stages: Stage I - one day before surgery; Stage II - after induction of anesthesia; Stage III - bowel reduction; Stage IV - awakening; Stage V - one day after surgery. Heart rate (HR), systolic and diastolic blood pressure (BP), ejection fraction (EF) and stroke volume (SV) were assessed during the study. RESULTS: It was revealed that at the stage of bowel movement in children where concomitant anesthesia was used, there was a more pronounced decrease of HR (120 vs 106; p < 0.001). Similar changes were characteristic of diastolic BP (48 vs 42; p < 0.001). Mean BP was also significantly lower in the concomitant anesthesia group, and this was statistically significant in both stages III and IV (59.3 vs 54.7, p < 0.001; 61.5 vs 57.0, p < 0.001). In the group where combined anesthesia was used, a decrease in stroke volume was noted at the awakening stage, which was statistically significant compared to the I-III stages. The maximum ejection fraction indices were observed at the stage of bowel lowering against the background of combined anesthesia, which was statistically significant compared to other stages of the study CONCLUSIONS: At the stage of bowel reduction using concomitant anesthesia there was a significant statistically significant decrease of HR and systemic BP which was most probably due to the powerful sympatholytic effect of local anesthetics.
