В статье представлены морфологические особенности пищевода, включающие в себя подслизистые и кардиальные железы, выполняющие специфическую защитную функцию, многослойный плоский неороговевающий эпителий, непроницаемый для внутрипросветного содержимого, подслизистое нервное сплетение, участки гетеротопии слизистой оболочки желудка в проксимальной трети пищевода, гастроэзофагеальное соединение и нижний пищеводный сфинктер. Описаны характерные паттерны ответа на воздействие агрессивных факторов внешней среды, а также особые формы воспалительного ответа слизистой оболочки пищевода, такие как лимфоцитарный эзофагит, эозинофильный эзофагит, IgG4-ассоциированный эзофагит. The article presents the morphological features of the esophagus, including submucosal and cardiac glands that perform a specific protective function, stratified squamous non -keratinized epithelium, impermeable to intraluminal nutrition, submucosal nerve plexus, areas of mucous gland heterotopy penetrating into the proximal alimentary esophagus, gastroesophageal junction and lower digestive tract sphincter. Characteristic patterns of manifestation of aggressive environmental factors are described, as well as inflammation of the esophageal mucosa, such as lymphocytic esophagitis, eosinophilic esophagitis, IgG4-associated esophagitis.
В этой статье подробно обсуждается понятие о дуоденогастральном рефлюксе, эпидемиологические данные и патогенетические аспекты подобного состояния, включая роль изменений нейрогуморальной регуляции, молекул клеточной адгезии, а также влияние Helicobacter pylori. Приведена информация о компонентах дуоденального рефлюксата, а также особенности их влияния на слизистую оболочку желудка. Кроме того, в статье описываются алгоритмы диагностики химического гастрита, особенности гистопатологических изменений при химическом гастрите, применение расчетных индексов при гистологическом исследовании и возможные подходы к терапии. This article discusses in detail the concept of duodenogastric reflux, epidemiological data and pathogenetic aspects of this condition, including the role of changes in neurohumoral regulation, cell adhesion molecules, and the influence of Helicobacter pylori. The data on the components of duodenal refluxate, as well as their features of their effect on the gastric mucosa are presented. In addition, the article describes the algorithms for diagnosing chemical gastritis, the features of histopathological changes in chemical gastritis, the use of calculated indices in histological examination, and possible approaches to therapy.
