Кишечная микробиота - это отдельный уникальный экстракорпоральный орган, выполняющий множество функций в организме человека. Наш метаболизм неразрывно связан с деятельностью представителей кишечной микробиоты. Последнее время большое внимание уделяется изучению роли нормальной микробиоты в липидном обмене, а также роли нарушений микробиоты в формировании и прогрессировании нарушений липидного обмена. В статье приведены современные данные литературы, освещающие указанную тематику, а также результаты исследований, демонстрирующие связи между показателями липидного спектра крови, особенностями микробного пейзажа кишки, и влияние пробиотических штаммов на липидный обмен. The intestinal microbiota is a separate unique extracorporeal organ that performs many functions in the human organism. Our metabolism is inseparably linked to the activity of representatives of the gut microbiota. Recently, much attention has been paid to the study of the role of normal microbiota in lipid metabolism, as well as to the study of the influence of microbiota disorders on the formation and progression of disorders of lipid metabolism. The article presents current literature data covering this topic. Researches data are also presented that demonstrate the relationship between the blood lipid profile, the characteristics of the microbial landscape of the intestine, and the effect of probiotic strains on lipid metabolism.
Воспалительные заболевания кишечника (ВЗК), в частности болезнь Крона, являются одной из наиболее непростых и неизученных проблем в современной гастроэнтерологии. Актуальность данной проблемы обусловлена неспецифичностью клинической картины в дебюте заболевания, поздней диагностикой и несвоевременным началом терапии, что зачастую приводит к развитию тяжелых инвалидизирующих осложнений. В данной публикации представлен клинический случай осложненного течения болезни Крона. В анамнезе у пациентки за период с 2013 по 2022 год было проведено около двадцати оперативных вмешательств. Inflammatory bowel disease, particularly, Crohn’s disease, is one of the most difficult and unexplored problems in modern gastroenterology. The relevance of this problem is due to the non -specificity of the clinical picture in the debut of the disease, late diagnosis and untimely initiation of therapy, which often leads to the development of severe disabling complications. This publication presents a clinical case of a complicated course of Crohn’s disease, in the anamnesis morbi for the period from 2013 to 2022, about twenty surgical interventions were performed.
В данной статье обсуждается, каким составом и свойствами обладает желчь в нормальных физиологических условиях, как протекают сложные процессы синтеза и рециркуляции желчных кислот - важнейшего компонента желчи, и как меняются состав и свойства желчи при желчнокаменной болезни. В статье также приведены данные оригинального исследования, в котором впервые определены абсолютные значения содержания желчных кислот в пузырной желчи пациентов с желчнокаменной болезнью методом хромато -масс -спектрометрии. This article discusses the composition and properties of bile in normal physiological conditions, how the complex processes of synthesis and recycling of bile acids - the most important component of bile - proceed, and how the composition and properties of bile change in cholelithiasis. The article also presents data from an original study in which the absolute values of the content of bile acids in the cystic bile of patients with cholelithiasis by chromatography -mass spectrometry were determined for the first time.
Астения - одна из наиболее распространенных неспецифических форм реагирования центральной нервной системы на самые разнообразные внешние и внутренние трудности. Согласно уровню обращаемости за медицинской помощью, более 60 % жалоб пациентов приходится на долю связанных с астеническим синдромом. Подобные расстройства наблюдаются у всех категорий населения. Данная статья представлена вниманию читателей как заключительная, четвертая, из цикла клинических лекций, посвященных проблеме астении. Рассматриваются основные принципы лечения астенических расстройств, возможности психотерапии, физиотерапии и медикаментозной коррекции астенических состояний. Asthenia is one of the most common nonspecific forms of the central nervous system’s response to a wide variety of external and internal difficulties. According to the level of medical care, more than 60 % of patient complaints are related to asthenic syndrome, similar disorders are observed in all categories of the population. This lecture is presented to the readers as the fourth and final of a series of clinical lectures on the problem of asthenia. The lecture discusses the basic principles of therapy of asthenic disorders, the possibilities of psychotherapy, physiotherapy and drug correction of asthenic conditions
В статье описана роль пероральных препаратов 5-аминосалициловой кислоты (5-АСК) в структуре базисной терапии язвенного колита согласно действующим клиническим рекомендациям. Показаны результаты актуальных эпидемиологических исследований, отражающих особенности применения пероральных препаратов 5-АСК в клинической практике в разрезе протяженности поражения толстой кишки, отмечена высокая частота использования монотерапии пероральными препаратами 5-АСК. Представлен спектр применяемых в настоящее время пероральных препаратов 5-АСК и их особенности, определяющие поступление действующего вещества в слизистую оболочку пораженных участков толстой кишки. The article represents the role of oral 5-aminosalicylic acid(5-ASA) drugs in the structure of the basic therapy of ulcerative colitis according to current clinical guidelines. The results of current epidemiological studies reflecting the features of the use of oral preparations 5-ASA in clinical practice in the context of the length of colon lesions are presented, a high frequency of use of monotherapy with oral 5-ASA preparations were noted. The spectrum of currently used oral preparations 5-ASA and their features that determine the flow of the active substance into the mucous membrane of the affected areas of the colon are presented.
