Введение. Среди всех первичных опухолей сердца злокачественные новообразования составляют 7–10%.Цель. Представить опыт хирургического лечения пациентов со злокачественными опухолями сердца с обращением особого внимания на обеспечение максимальной радикальности операций и улучшения их результатов.Материалы и методы. В Институте сердечно-сосудистой хирургии им. Н.М. Амосова НАМН Украины в период с 1970 по 01.01.2020 г. наблюдалось 67 пациентов со злокачественными новообразованиями сердца различного вида и локализации, что составило 7,1% от общего числа пациентов с опухолями сердца (949).Результаты. Госпитальная летальность при хирургическом лечении составила 19,4% (13 пациентов). Из числа 67 выполненных операций 57 (83,3%) проведено с использованием искусственного кровообращения, из которых выполнено 21 (31,9%) условно радикальное кардиохирургическое вмешательство с хорошим гемодинамическим эффектом.Обращается особое внимание на необходимость выполнения максимально радикального удаления новообразования, что в 29 случаях требовало дополнительных хирургических манипуляций, включающих пластику межпредсердной перегородки, стенок предсердий, пластические операции на клапанах, а также их протезирование. Показано, что расширенный объем операции оказывает существенное влияние на непосредственный результат хирургического вмешательства.Заключение. Для достижения условной радикальности операции и максимального гемодинамического эффекта в целом ряде случаев необходимо применение расширенных методик реконструкций поврежденных структур сердца в сочетании с активной дезинтоксикационной терапией и безотлагательной химиотерапией. Introduction. In the structure of all primary tumors of the heart, malignant neoplasms make up 7–10%.Purpose. To present the experience of surgical treatment of patients with malignant heart tumors with special attention on ensuring the maximum radicalism of the operation and improving their results.Materials and methods. At the N.M. Amosov National Institute of Cardiovascular Surgery of the Academy of Medical Sciences of Ukraine, in the period from 1970 to 01.01.2020, 67 patients with malignant neoplasms of the heart of various types and localization were observed, which was 7.1% of the total number of patients with heart tumors (949).Particular attention is paid to the need to perform the most radical removal of the tumor, which in 29 cases required additional surgical manipulations, including atrial septal plastics, atrial walls, repair surgery of the valves, as well as their replacement. It is shown that the expanded volume of operation has a significant impact on the direct result of surgery.Results. Hospital mortality during surgical treatment was 19.4% (13 patients). From 67 operations, 57 (83.3%) ones were performed using artificial blood circulation; 21 (3.8%) conditionally radical cardiac surgeries were performed with good hemodynamic effect.Conclusion. To achieve the conditional radicalism of the operation and the maximum hemodynamic effectinanumberofcases, it isnecessarytouseadvancedtechniquesforreconstructingthedamaged cardiac structures in combination with active detoxification therapy and urgent chemotherapy.
Введение. Данные инструментальных исследований (ЭхоКГ, КТ, МРТ) не могут дать окончательное заключение о характере новообразований сердца, несмотря на имеющиеся признаки злокачественного или доброкачественного роста. Гистологическое исследование выносит окончательный вердикт о характере патологического процесса, что определяет оптимальную тактику последующего лечения опухолей сердца. Цель. Представить опыт диагностики и хирургического лечения злокачественных опухолей сердца с акцентированием внимания на методике биопсии, позволяющей верифицировать диагноз и провести адекватное лечение новообразования. Материалы и методы. Методы биопсии, которые применялись в диагностике новообразований сердца, условно представлены 3 группами: 1) в условиях выполнения эксплоративной стернотомии (10 случаев); 2) при проведении диагностической боковой торакотомии (2 случая); 3) проведение пункционной биопсии под контролем КТ-навигации (3 случая). Результаты. Проведенное гистологическое исследование материала, полученного при биопсии опухолей сердца, позволило уточнить диагноз и назначить адекватную специфическую терапию с положительной динамикой заболевания. Выводы. Биопсия в кардиоонкологии во многом определяет тактику лечения опухолевых поражений сердца и перикарда. Безопасность выполнения биопсии должна быть приоритетом при ее проведении. Накопление опыта применения пункционной биопсии при опухолевом поражении сердца позволит более эффективно использовать ее в кардиохирургической практике. Introduction. The data of instrumental studies (echocardiography, CT, MRI) cannot give a final conclusion about the nature of the heart neoplasms, despite the presence of signs of malignant or benign growth. The histological examination carries out the final verdict on the nature of the pathological process, which determines the optimal tactics for the subsequent treatment of cardiac tumors. Purpose. To present the experience of diagnostics and surgical treatment of malignant cardiac tumors with an emphasis on biopsy techniques that allow verifying the diagnosis and conducting adequate treatment of the neoplasm. Materials and methods. Biopsy methods, which were used in the diagnosis of heart neoplasms are conventionally represented by three groups: 1) in conditions of performing an explorative sternotomy (10 cases); 2) when carrying out diagnostic lateral thoracotomy (2 cases); 3) by the method of puncture biopsy under the control of CT navigation (3 cases). Results. A histological study of the material obtained from a biopsy of heart tumors made it possible to clarify the diagnosis and prescribe an adequate specific therapy with a positive dynamics of the disease. Conclusions. Biopsy in cardio-oncology largely determines the tactics of treating tumor lesions of the heart and pericardium. Biopsy safety should be a priority when performing biopsy. The accumulation of experience in the use of puncture biopsy in tumor lesions of the heart will make it possible to use it more effectively in cardiac surgery practice.
