Введение. Среди всех первичных опухолей сердца злокачественные новообразования составляют 7–10%.Цель. Представить опыт хирургического лечения пациентов со злокачественными опухолями сердца с обращением особого внимания на обеспечение максимальной радикальности операций и улучшения их результатов.Материалы и методы. В Институте сердечно-сосудистой хирургии им. Н.М. Амосова НАМН Украины в период с 1970 по 01.01.2020 г. наблюдалось 67 пациентов со злокачественными новообразованиями сердца различного вида и локализации, что составило 7,1% от общего числа пациентов с опухолями сердца (949).Результаты. Госпитальная летальность при хирургическом лечении составила 19,4% (13 пациентов). Из числа 67 выполненных операций 57 (83,3%) проведено с использованием искусственного кровообращения, из которых выполнено 21 (31,9%) условно радикальное кардиохирургическое вмешательство с хорошим гемодинамическим эффектом.Обращается особое внимание на необходимость выполнения максимально радикального удаления новообразования, что в 29 случаях требовало дополнительных хирургических манипуляций, включающих пластику межпредсердной перегородки, стенок предсердий, пластические операции на клапанах, а также их протезирование. Показано, что расширенный объем операции оказывает существенное влияние на непосредственный результат хирургического вмешательства.Заключение. Для достижения условной радикальности операции и максимального гемодинамического эффекта в целом ряде случаев необходимо применение расширенных методик реконструкций поврежденных структур сердца в сочетании с активной дезинтоксикационной терапией и безотлагательной химиотерапией. Introduction. In the structure of all primary tumors of the heart, malignant neoplasms make up 7–10%.Purpose. To present the experience of surgical treatment of patients with malignant heart tumors with special attention on ensuring the maximum radicalism of the operation and improving their results.Materials and methods. At the N.M. Amosov National Institute of Cardiovascular Surgery of the Academy of Medical Sciences of Ukraine, in the period from 1970 to 01.01.2020, 67 patients with malignant neoplasms of the heart of various types and localization were observed, which was 7.1% of the total number of patients with heart tumors (949).Particular attention is paid to the need to perform the most radical removal of the tumor, which in 29 cases required additional surgical manipulations, including atrial septal plastics, atrial walls, repair surgery of the valves, as well as their replacement. It is shown that the expanded volume of operation has a significant impact on the direct result of surgery.Results. Hospital mortality during surgical treatment was 19.4% (13 patients). From 67 operations, 57 (83.3%) ones were performed using artificial blood circulation; 21 (3.8%) conditionally radical cardiac surgeries were performed with good hemodynamic effect.Conclusion. To achieve the conditional radicalism of the operation and the maximum hemodynamic effectinanumberofcases, it isnecessarytouseadvancedtechniquesforreconstructingthedamaged cardiac structures in combination with active detoxification therapy and urgent chemotherapy.
