Воронеж, Россия Реферат В представленном литературном обзоре освещены современные взгляды на патофизиологические аспекты фор-мирования и прогрессирования когнитивных нарушений на фоне хронической сердечной недостаточности у пациентов старших возрастных групп. Пожилой возраст сам по себе является важным предиктором развития сосудистых, нейродегенеративных и других заболеваний. Известно, что инволютивные изменения в сердеч-но-сосудистой системе потенцируют развитие хронической сердечной недостаточности. У пожилых и старых людей сердечная недостаточность развивается, как правило, постепенно. Формирование когнитивного дефици-та на фоне сердечной патологии обусловлено хронической ишемией головного мозга, а также целым каскадом нейрохимических процессов, протекающих в головном мозге, в конечном итоге формируя порочный круг. Не-редко признаки ишемии мозга вследствие уменьшения ударного объёма крови возникают значительно раньше, чем явления застоя в других органах и системах. Хроническая ишемия головного мозга, возникающая вслед-ствие нарушения церебральной гемодинамики, связана как с экстрацеребральными, так и с интрацеребраль-ными причинами, что в свою очередь способствует развитию хронической гипоксии головного мозга и усугу-блению когнитивной дисфункции. Таким образом, особенности развития и течения болезней у людей старших возрастных групп свидетельствуют о том, что в гериатрической практике не всегда применимы сложившиеся схемы диагностики. При наблюдении за пациентами пожилого и старческого возраста с хронической сердеч-ной недостаточностью при оценке их состояния и проведении диагностики необходимо уделять особое внима-ние наиболее раннему выявлению признаков когнитивной дисфункции с целью коррекции лечения пациента и улучшения качества его жизни. Advanced age itself is an important predictor of the development of cardiovascular, neurodegenerative and other diseases. Involutive changes of cardiovascular system are known to potentiate the development of chronic heart failure. Heart failure in older people usually develops gradually. Formation of the cognitive deficit in heart disease is associated with chronic cerebral ischemia as well as a cascade of neurochemical processes occurring in the brain, eventually forming a vicious circle.
