In the article argued the necessity of using tactics "hepatoprotektor's escort" in clinical practice. In it demonstrates data about new effects of Ursodeoxycholic Acid and the capabilities of its application as a universal hepatoprotector have been elucidated. РЕЗюМЕ В статье аргументирована необходимость использования клинической практике тактики «гепатопротекции сопровождения». Приве-дены данные о новых эффектах урсодезоксихолевой кислоты. Освещены возможности ее использования как универсального гепатопро-тектора. Включение урсодезоксихолевой кислоты в комплексную терапию способствует повышению эффективности лечения пациентов во многих отраслях медицины.Ключевые слова: урсодзоксихолевая кислота; лекарственно-индуцированные поражения печени.
Цереброваскулярні захворювання є другою причиною у структурі смертності населення у нашій країні та третьоюу світі, левову частку (96%) у структурі яких у нашій країні займають хронічні порушення мозкового кровообігу-дис циркуляторні енцефалопатії. Ризик цереброваскулярної патології суттєво збільшується у разі тривало існуючої і по гано контрольованої артеріальної гіпертензії (АГ). Основними факторами ризику розвитку гіпертензивної енце фалопатії (ГЕ), безпосередньо пов'язаними з тяжкістю та перебігом АГ, є недостатньо контрольована АГ, у тому числі так звана м'яка АГ, гіпертонічні кризи, висока варіабельність артеріального тиску (АТ) з високими нічними показниками, епізоди ортостатичної гіпотензії, надмірне зниження АТ. Ризик виникнення інсульту корелює як з величиною сис толічного, так і діастолічного артеріального тиску. Принцип лікування ГЕ полягає у наступному: терапія АГ з призначенням індивідуально підібраною антигіпертензивною терапією з досягненням цільових значень АТ; вплив на фак тори ризику серцево судинних захворювань (статини, анти агреганти, відмова від куріння тощо); покращення перфузії головного мозку і корекція метаболічних порушень у нерво вих клітинах, що функціонують в умовах ішемії і гіпоксії. Основними принципами сучасної антигіпертензивної терапії при судинній патології мозку є те, що вона базова, постійна, практично пожиттєва з використанням сучасних класів препа ратів. При неускладненій АГ приблизно однакову ефективність демонструють усі п'ять базисних класів антигіпертензивних препаратів: діуретики, бета блокатори, інгібітори АПФ, блока тори кальцієвих каналів, антагоністи рецепторів ангіотензину ІІ (перевага-комбінована терапія, навіть на ранніх етапах!). Ре комендовано досягнення цільових рівнів АТ <140/90 мм рт.ст. (що знижує ризик цереброваскулярних катастроф на 1/3). Переконливі дані доказової медицини стосовно вторинної профілактики поки що є для інгібіторів АПФ, сартанів, діуретиків. Ураховуючи те, що ГЕ є міждисциплінарною проблемою, з якою найчастіше зустрічається сімейний лікар, а також особливості діагностики та тактики ведення таких хворих, ця патологія потребує особливої уваги. Полтава За данням Фремінгемського дослідження, серед хворих на хронічну серцеву недостатність (ХСН) тільки у 25% причиною розвитку був перенесений інфаркт міокарда, а у 75%-артеріальна гіпертензія (АГ). Найбільш значним предиктором розвитку ХСН було збільшення систолічного артеріального тиску (САТ) і пульсового тиску відповідно, кожне збільшення САТ на 20 мм рт.ст. і пульсового АТ на 16 мм рт.ст. призводило до збільшення частоти розвитку ХСН на 52% і 55% відповідно. На прикладі наведеного клінічного випадку пацієнта із ХСН, яка розвинулася на тлі довготривалої АГ, розглянуто механізми розвитку ХСН, також висвітлено питання фармакотерапії АГ у поєднанні з ХСН із систолічною дисфункцією. Основними механізмами, що безпосередньо призводять до розвитку ХСН при АГ, є гемодинамічне перевантаження, зниження міокардіальної скоротливості, гіпертрофія лівого шлуночка (ГЛШ). Так, у пацієнтів з АГ ймовірність у подальшому розвитку ХСН підвищується у 4 рази, а з ГЛШ-у 15 разів. Поряд із ГЛШ, одним із ранніх проявів ремоделювання ЛШ в умовах АГ є розвиток діастолічної дисфункції, яка передує розвитку систолічних порушень при АГ і ГЛШ. Антигіпертензивна терапія призводила до зменшення у середньому на 52% частоти розвитку ХСН порівняно з хворими, які не отримували адекватну терапію. Зниження частоти розвитку ХСН лінійно залежало від зниження САТ: кожне зниження на 10 мм рт.ст. САТ призводило до зменшення на 26% відносного ризику розвитку ХСН. Встановлено, що АГ є не тільки одним із провідних етіологічних чинників розвитку ХСН, а й має з останньою спільність в ключових ланках патогенезу. Стратегія вибору патогенетичної фармакотерапії визначається з урахуванням цієї обставини. На сьогодні Європейське товариство кардіологів рекомендує призначати β-АВ усім пацієнтам зі стабільною ХСН ІІ-ІV ФК в якості стандартного лікування разом із іАПФ і діуретиками за відсутності протипоказань. У хворих із поєднанням АГ та систолічною ХСН можливе застосування додатково до блокаторів РААС тіазидних або петльових діуретиків та антагоністів мінералокортикоїдних рецепторів (АмР). При цьому ефективність антигіпертензивної терапії (АГТ) у жінок після 50 років знижується. У статті наведені основні особливості перебігу АГ у жінок у постменопаузальний період, а також особливості АГТ.
