Obesity is an established risk factor for cardiovascular diseases (CVD) such as hypertension (HD), coronary artery disease (CAD), heart failure (HF), arrhythmias and venous thromboembolism. Lifestyle modifications to reduce weight have been shown to be successful in the short term, however their long-term results are still equivocal likely due to modest weight reduction and high recurrence rates. Bariatric surgery has been recognized as the most effective strategy in achieving substantial sustained weight loss, and can prevent the development or reverse CVD, metabolic syndrome, diabetes mellitus, obstructive sleep apnea syndrome, cancer, and COVID-associated complications. Bariatric surgery results in rapid weight loss over several months that lasts for at least 12-18 months. The surgery lowers all-cause mortality risks, risks of myocardial infarction and stroke. Furthermore, bariatric surgery contributes to the reduction of urgent hospitalizations for heart failure, coronary artery disease, and hypertension. The article shows modern view on the impact of bariatric surgery on the pathogenesis of the CVD in patients with obesity, indications and contraindications of the surgery, tactics of management of patients with existing cardiovascular pathology before the bariatric surgery.

Київ У статті розглянуто вплив підвищеного індексу маси тіла і ожиріння на розвиток серцево-судинної патології. доведено, що у пацієнтів з надмірною масою тіла та ожирінням відбуваються гемодинамічні, структурні та функціональні порушення серця і судин, які призводять до ризику розвитку гіпертонічної хвороби, фібриляції передсердь, ішемічної хвороби серця, хронічної серцевої недостатності, інсульту та підвищеного ризику загальної смертності від серцево-судинних захворювань. Сьогодні у світовій лікарській спільноті триває дискусія щодо існування таких визначень, як «метаболічно здоровий фенотип ожиріння», «парадокс ожиріння», «кардіоміопатія ожиріння». За сучасними уявленнями, «метаболічно здорове ожиріння» розглядається лише як перехідний стан між нормальним метаболічно здоровим станом та метаболічно нездоровим ожирінням. «Парадокс ожиріння» полягає в тому, що пацієнти з хронічною серцевою недостатністю мають кращий прогноз щодо виживаності за наявності надмірної маси тіла або ожиріння порівняно з пацієнтами з нормальним або низьким індексом маси тіла. Тому таким пацієнтам не рекомендовано зниження маси тіла. Однак існують контраверсійні питання щодо причини різниці у виживаності «парадоксу ожиріння». Також невизначеним залишається питання, чи спроможне ізольоване ожиріння через накопичення ліпідів у кардіоміоцитах, запальних, нейрогуморальних та клітинних порушеннях призводити до «кардіоміопатії ожиріння». Наведені вище стани важливо розуміти, щоб мати чітке уявлення про необхідність терапевтичного впливу на них. Безперечною тактикою є запобігання збільшенню маси тіла та ожиріння з метою профілактики серцево-судинних та метаболічних захворювань. При вже наявному ожирінні пропонуються різні шляхи лікування. Одним з ефективних шляхів є лікування за допомогою баріатричної хірургії. It is proven, that patients with excess body weight and obesity develop hemodynamic, structural, functional heart and blood vessels disorders, which increase the risk of hypertension, atrial fibrillation, coronary heart disease, chronic heart failure, stroke and increase the risk of death from cardiovascular diseases.
