Москвы Функция щитовидной железы находится в тесном взаимодействии с гипоталамо-гипофизарно-гонадной системой, прежде всего благодаря наличию общих центральных механизмов регуляции. Одним из возможных и важных проявлений нарушения функции щитовидной железы у мужчин является послеоперационный гипогонадизм, или снижение функциональной активности репродуктивной системы. Цель -определение возможности применения эластографии и морфологических исследований при послеоперационном гипотиреозе. Было проведено рандомизированное исследование 48 пациентов в возрасте от 35 до 55 лет. Исследуемые были разделены на три группы. В первую группу вошли доноры -10 человек (здоровые), во вторую группу -18 пациентов после гемитиреоидэктомии, третью группу (n=20) составили мужчины после субтотальной тиреоидэктомии. Эластографию проводили на ультразвуковом сканере Aixplorer с использованием датчика oт 5 до 15 МГц. Исследование гормонов осуществляли на иммунохемилюминесцентном анализаторе LIASON XL, морфологическое исследование -по стандартной методике. Скорость сдвига поперечной волны Vs у пациентов 2-й группы составила 1,1-1,5 м/с и была увеличена на 10-15% по сравнению с пациентами первой группой; у пациентов 3-й группы отмечались уменьшение площади картирования и снижение концентрации и доли активноподвижных сперматозоидов. Коэффициент эластичности деформаций в стенке мошонки тест-зоны ближнего поля к тестикулярной ткани был ниже во 2-й группе, чем в 3-й, и различие было статистически значимо (р=0,008). Показатели степени сперматогенеза по шкале Johnsen были выше во 2й группе, чем в 3-й, и имели статистически значимое различие (р=0,003). Применение подобной тактики метода инструментальной диагностики позволит на этапе обследования заподозрить репродуктивные нарушения у пациентов, оперированных на щитовидной железе.
