Стаття колективу провідних вчених педіатрів та організаторів педіатричної охорони здоров’я України присвячена вивченню основних тенденцій у стані здоров’я дитячого населення країни за останні 22 роки. Аналіз захворюваності та поширеності хвороб серед дитячого населення України, рівнів малюкової смертності протягом останніх двох десятиліть свідчить про те, що вони залишаються значно вищими від середньоєвропейських показників на тлі прогресивного зменшення чисельності дитячої популяції на 3,16 млн осіб. Поширеність дитячих хвороб в Україні за останні 22 роки зросла на 41 %, захворюваність на дитячі хвороби — на 36 %. Розглянуті шляхи вирішення низки нових соціальних та медико-екологічних проблем, що негативно впливають на здоров’я дітей.
Імунна відповідь в організмі дитини залежить від злагодженого функціонування як вроджених неспецифічних механізмів захисту (бар'єрні функції слизових оболонок, гуморальні неспецифічні чинники місцевого захисту, функціонування системи комплементу, моноцитарно-макрофагальна активність імунокомпетентних клітин, тощо), так і набутої специфічної (адаптивної) ланки імунітету [3]. Мета роботи. Підвищення ефективності лікування та профілактики гострих рекурентних респіраторних інфекцій (ГРРІ) шляхом застосування сиропу Імунофлазід® у дітей молодшого шкільного віку. Матеріали і методи. Обстежено 50 дітей молодшого шкільного віку з ГРРІ. Ефективність застосування сиропу Імунофлазід® визначалась шляхом порівняння динаміки клінічних симптомів, особливостей мікрокристалізації слини, бактеріального складу фарингеального ексудату до дослідження та через рік проведення лікувально-профілактичних курсів. Висновки. Сироп Імунофлазід® при лікувально-профілактичному застосуванні сприяє покращанню загального стану здоров’я дітей, зменшенню проявів інтоксикаційного, астенічного синдромів, частоти та тяжкості перебігу гострих респіраторних інфекцій, кількості днів пропусків занять у школі, числа та тривалості госпіталізацій, нормалізації кристалоутворюючого потенціалу слини на тлі зменшення мікробіологічного навантаження ротової порожнини.
The aim: The work is aimed at determining the relationship between TLR4 expression on CD14+monocytes in whole heparinized blood and B1a lymphocyte synthesis in various subtypes of JIA. Materials and methods: 64children aged3to17years were examined, including42children with different subtypes of JIA and22healthy children. The intensity of TLR4 expression onCD14+monocytes was determined in whole heparinized blood incubated with a CD14-FITC/TLR4-PE monoclonal antibody cocktail(Biolegend, USA)using flow cytometry. Monoclonal antibodies (BD Bioscience) were used to determine the main subpopulations of lymphocytes. Results: A statistically significant increase in TLR4 expression has been determined in JIA compared to the control group. The most prominent TLR4 expression was detected in children with oligoarthritis, while in systemic arthritis, there was no statistical difference compared to healthy children. High TLR4 expression on peripheral CD14+monocytes inversely depends on the activity of the autoimmune process, which may have a protective effect against the aseptic inflammation.Increased TLR4 expression involves a statis¬tically significant increase in the percentage and quantity of В1а lymphocytes(p≤0.05). Conclusions: A statistically significant increase in TLR4 expression on CD14+monocytes in whole heparinized blood was detected in patients with JIA compared to healthy children. Children with oligoarthritis had the highest rates, which indicates possible differences in the development of pathogenetic processes in different subtypes of arthritis. Determining the degree of TLR4 activation on CD14+monocytes is reasonable for predicting JIA activity.
Chronic gastroduodenitis (CGD) hold one of main places among leading diseases of gastro-intestinal tract (GIT) in childhood. A special category is children who have CGD against the background of connective tissue (CT) dysplasia (DCT). CGD in such children is characterized by early appearance, aggressive, gradient course with frequent acute conditions and recurrences with evident symptoms of nonspecific intoxication manifestations. Defect of collagen synthesis leads to pronounced morphological changes of proximal GIT mucous membrane (MM) and decline in its cytoptotecive abilities. Challenges related to the qualitative and quantitative local ecosystem changes can aggravate immune and metabolic local processes and enhance the destruction of proximal GIT mucous membrane. Aforesaid defines the need to include multiprobiotics into the treatment schemes of patient with CGD, associated with congenital fibrogenesis disorders, as adjuvant therapy with complex therapeutic action; they positively influence the CT metabolism, improve reparative processes in MM, normalize immune processes and maintain composition and functions of physiological microbiota.Purposes -improvement of CGD treatment efficiency in children with DCT through inclusion of the multiprobiotic Symbiter ® forte-M into the treatment schemes.Materials and methods. There were examined 33 children with CGD on the background of DCT and 32 healthy children (control) from 11 to 17 years old. The state of the mucous barrier was estimated by certain mucin constituents (fucose, glycosaminoglycans, sialic acids) assessing, antimicrobial peptides (ß2-defencins), non-specific humoral factors of the local immunity (immunoglobulins, lysozyme) in mucous secretions (saliva, coprofiltrate (CF)) before and after probiotic therapy.Results. Increased concentrations of ß 2 -defencins (p saliva = 0.03, p CF = 0.03), sialic acids (P = 0.04) and lysozyme (P = 0.01) and decreased of fucose level (P = 0.03) in mucous secretions under application of multicomponent probiotic preparation were observed. Conclusions.In children with DCT and CGD development, mucins, forming protective homeostatic barrier between residential microbiota and basic immune intestinal cells, contain in their composition increased quantity of fucose-containing glycoproteins and low amount of sialic acids; this influences microbiota and activity of cytoprotective and reparative processes in MM. Inclusion of the multiprobiotic Symbiter ® forte-M into the standard treatment scheme leads to the normalization of some mucus glycoproteins and non-specific humoral local immunity factors. Due to its complex composition with high probiotic and immunomodulatory properties and natural mineral gel (smectite) inclusion, the multiprobiotic improves cytoprotective and reparative properties of GIT mucous membrane and reduces the likelihood of severe destructive processes in children with CGD and DCT development.Ад'ювантна терапія хронічних гастродуоденітів у дітей із дисплазією сполучної тканини О. Purpose - to increase the effectiveness of prevention and correction of D hypovitaminosis in juvenile idiopathic arthritis (JIA) by optimizing the algorithms of dosage regimens and duration of vitamin D supplementation. Materials and methods. A clinical and laboratory examination of 118 children aged 3 to 18 years with a diagnosis of JIA was carried out. Vitamin D status was assessed by the concentration of 25(OH)D in the blood serum using a Reader ER 500 enzyme immunoassay photoelectric analyzer. All patients in the complex of therapeutic measures were prescribed vitamin D3 preparations according to the developed algorithms, taking into account the presence of trigger factors for reducing the supply of this vitamin to the body, existing world recommendations and the initial level of 25(OH)D in blood serum. Results. The use of vitamin D3 preparations according to the scheme of 4000 IU/d for two months, followed by continuation at a dose of 2000 IU/d for one month, contributed to the achievement of sufficient concentrations of 25(ОH)D (36.7-47.5 ng/ml) in the blood serum in all patients. When taking vitamin D at a dose of 4000 IU/d for one month, and then for another two months at 2000 IU/d, the optimal concentrations of 25(OH)D in the blood serum (31.55-45.14 ng/ml) were achieved only in 52.9% children. Maintenance therapy in the future for 6 months at a dose of 1000 IU/d provided sufficient concentrations of vitamin D in 61.86% of children, and in 38.14% there was a tendency to reduce the content of 25(OH)D (26.83±2.99) ng/ml, which indicated the insufficiency of the specified maintenance dose of vitamin D for children with chronic progressive inflammation and the presence of comorbid pathology and the impact on adverse risk factors that affect the body’s supply of this vitamin. Conclusions. In JIA, the dose of saturation (4000-6000 IU/d) with vitamin D preparations should be determined depending on the initial degree of its insufficiency in the body, the characteristics of the clinical course and treatment of the underlying disease, and the presence of factors that contribute to a decrease in the supply of vitamin D to the body. Maintenance therapy at a dose of 2000 IU/d for 6 months makes it possible to maintain the optimal concentration of 25(OH)D in the body with a high probability. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the authors.
