This review addresses the contribution of some genes to the phenotype of familial hypercholesterolemia. At present, it is known that the pathogenesis of this disease involves not only a pathological variant of low-density lipoprotein receptor and its ligands (apolipoprotein B, proprotein convertase subtilisin/kexin type 9 or low-density lipoprotein receptor adaptor protein 1), but also lipids, including sphingolipids, fatty acids, and sterols. The genetic cause of familial hypercholesterolemia is unknown in 20%–40% of the cases. The genes STAP1 (signal transducing adaptor family member 1), CYP7A1 (cytochrome P450 family 7 subfamily A member 1), LIPA (lipase A, lysosomal acid type), ABCG5 (ATP binding cassette subfamily G member 5), ABCG8 (ATP binding cassette subfamily G member 8), and PNPLA5 (patatin like phospholipase domain containing 5), which can cause aberrations of lipid metabolism, are being evaluated as new targets for the diagnosis and personalized management of familial hypercholesterolemia.
Регистр пациентов с семейной гиперхолестеринемией и пациентов очень высокого сердечно-сосудистого риска с недостаточной эффективностью проводимой гиполипидемической терапии (РЕНЕССАНС)Цель. В 2017г Российский многоцентровый регистр семейной гиперхолестеринемии (РоСГХС) был преобразован в Регистр пациентов с семейной гиперхолестеринемией и пациЕНтов очень высокого сЕрдечно-Сосудистого риска с недоСтАточной эффективНоСтью проводимой гиполипидемической терапии (РЕНЕССАНС), целью которого стал максимальный охват не только больных семейной гиперхолестеринемией (СГХС), но и больных с имеющимися атеросклеротическими сердечно-сосудистыми заболеваниями, у которых современная гиполипидемическая терапия не позволяла добиться целевого уровня холестерина липопротеидов низкой плотности (ХС ЛНП). Материал и методы. Регистр РЕНЕССАНС является открытым, национальным, наблюдательным исследованием и включает больных с определенной и вероятной (согласно голландским или британским критериям) гетерозиготной и гомозиготной СГХС, а также пациентов очень высокого сердечно-сосудистого риска. Были разработаны 2 регистрационных формы: для больных СГХС и больных очень высокого риска. Врачи заполняли формы в бумажном и электронном виде. Учитывали наличие факторов риска атеросклероза и анамнеза сердечно-сосудистых заболеваний, соблюдение диеты и гиполипидемической терапии. В каждом центре выполняли определение концентрации: общего холестерина (ОХС), триглицеридов (ТГ), холестерина липопротеидов высокой плотности (ХС ЛВП) в сыворотке крови. Содержание ХС ЛНП рассчитывали по формуле Фридвальда: ХС ЛНП=ОХС-ХС ЛВП-ТГ/2,2 (ммоль/л). Результаты. Всего включено 1208 больных СГХС и 497 больных очень высокого риска (средний возраст 54±13 и 61±8 лет, соответственно, 38% и 36% мужчин). При включении средние уровни липидов крови: ОХС -9,4±2,3 и 6,9±1,5 ммоль/л, ХС ЛНП -6,6±2,1 и 4,5±1,3 ммоль/л, соответственно. Частота применения гиполипидемической лекарственной терапии достигает в обеих группах 70%, однако достижение целевого уровня ХС ЛНП крайне редко. Заключение. Полученные результаты свидетельствуют о недостаточной приверженности и невысокой эффективности стандартной гиполипидемической терапии как при с СГХС, так и у больных с очень высоким сердечно-сосудистым риском. Для лечения резистентной гиперхолестеринемии показаны ингибиторы PCSK9. The aim of this work was to identify genetic variants potentially involved in familial hypercholesterolemia in 43 genes associated with lipid metabolism disorders. Targeted high-throughput sequencing of lipid metabolism genes was performed (80 subjects with a familial-hypercholesterolemia phenotype). For patients without functionally significant substitutions in the above genes, multiplex ligation-dependent probe amplification was conducted to determine bigger mutations (deletions and/or duplications) in the LDLR promoter and exons. A clinically significant variant in some gene associated with familial hypercholesterolemia was identified in 47.5% of the subjects. Clinically significant variants in the LDLR gene were identified in 19 probands (73.1% of all variants identified in probands); in three probands (11.5%), pathogenic variants were found in the APOB gene; and in four probands (15.4%), rare, clinically significant variants were identified in genes LPL, SREBF1, APOC3, and ABCG5. In 12 (85.7%) of 14 children of the probands, clinically significant variants were detectable in genes associated with familial hypercholesterolemia. The use of clinical criteria, targeted sequencing, and multiplex ligation-dependent probe amplification makes it possible to identify carriers of rare clinically significant variants in a wide range of lipid metabolism genes and to investigate their influence on phenotypic manifestations of familial hypercholesterolemia.

Целью исследования явилась оценка эффективности и приверженности гиполипидемической терапии, частоты развития сердечно<со< судистых осложнений в течение 3<летнего периода наблюдения в рамках регистра РЕНЕССАНС (Регистр пациентов с СГХС и пациентов очень высокого сЕрдечно<Сосудистого риска с недоСтАточной эффективНоСтью проводимой гиполипидемической терапии). Материал и методы. РЕНЕССАНС является открытым национальным наблюдательным исследованием и включает больных с семейной ги< перхолестеринемией (СГХС), а также пациентов очень высокого сердечно<сосудистого риска (ОВССР). Учитывали наличие факторов риска атеросклероза, анамнез сердечно<сосудистых заболеваний, гиполипидемическую терапию. В каждом центре выполняли определение концен< трации: общего холестерина (ОХС), триглицеридов (ТГ), холестерина липопротеидов высокой плотности (ХС ЛВП) в сыворотке крови. Содер< жание холестерина липопротеидов низкой плотности (ХС ЛНП) рассчитывали по формуле Фридвальда. Уровень липопротеида(а) измеряли методом иммуноферментного анализа в некоторых центрах. При оценке частоты конечной точки, включавшей фатальные и нефатальные сердечно<сосудистых осложнения (ССО), проводили анализ Каплана — Майера. Результаты. В регистр включено 1570 (средний возраст 54,0±14,6 лет) пациентов с СГХС и 121 (63,5±10,9 лет) больной с ОВССР. В группе СГХС динамическое наблюдение проведено у 594 пациентов (38%) в течение 23,6±14,6 месяцев, конечная точка зарегистрирована у 9% больных. Мужской пол (относительный риск 2,1; 95% доверительный интервал 1,13–3,66; p<0,01), гипертония (2,8; 1,4–5,2; p<0,01), ишеми< ческая болезнь сердца (6,8; 3,5–13,2; p<0,0001), отягощенный анамнез по сердечно<сосудистым заболеваниям (ССЗ) (2,1; 1,1–3,9; p<0,05) и концентрация липопротеида(а) ≥ 30 мг/дл (2,8; 1,1–7,7; p<0,05) явились предикторами развития ССО. В группе СГХС отмечено снижение уровня ОХС от исходного на 19%, ХС ЛНП на 25% (р<0,001 для обоих), целевых значений ХС ЛНП достигли 2% больных. В группе ОВССР динамическое наблюдение проведено у 72 (60%) пациентов в течение 19,7±5,8 месяцев. Ни один больной не достиг целевого уровня ХС ЛНП менее 1,4 ммоль/л. Заключение. Трехлетнее наблюдение за участниками регистра РЕНЕССАНС демонстрирует усиление приверженности гиполипидемической терапии. С увеличением риска развития сердечно<сосудистых осложнений при СГХС ассоциированы мужской пол, наличие гипертонии, ише< мической болезни сердца, отягощенного анамнеза по ССЗ и высокий уровень липопротеида(а). The aim of the study was to evaluate the effectiveness and adherence to hypolipidemic therapy, the frequency of cardiovascular events (CVE) during the 3!year follow!up in the RENAISSANCE registry (Registry of patients with familial hypercholesterolemia and very high cardiovascular risk with insufficient effect of hypolipidemic therapy). Methods. The RENAISSANCE registry is an open, national, observational study and includes patients with familial hypercholesterolemia (FH), as well as patients of very high cardiovascular risk (VHR). We took into consideration atherosclerosis risk factors and history of cardiovascular diseases (CVD), adherence to hypolipidemic therapy. Concentrations of total cholesterol (TC), triglycerides (TG), high density lipoprotein cholesterol (HDL!C) were measured in blood serum in all centers. Low density lipoprotein cholesterol (LDL!C) level was defined according to Friedewald formula. The concentration of lipoprotein(a) was measured by enzyme!linked immunosorbent assay in serum in some centers. Kaplan!Mayer analysis was performed to assess the frequency of fatal and nonfatal CVE. Results. The Registry consisted of 1570 (mean age 54.0±14.6 years) FH patients and 121 (63.5±10.9 years) VHR patients. Data of 594 patients (38%) who had follow!up visits were obtained in FH patients, follow!up duration 23.6±14.6 months, 54 (9%) patients experienced CVE. Male sex (hazard ratio 2.1; 95% confidence interval 1.13!3.66, p<0.01), hypertension (2.8;1.4–5.2; p<0.01), ischemic heart disease (6.8;3.5!13.2; p<0.0001), family history of CVD (2.1;1.1–3.9, p<0.05) and lipoprotein(a) level ≥30 mg/dl (2.8;1.1–7.7; p<0.05) were predictors of CVE. In FH patients the level of TC decreased by 19%, LDL!C by 25% (p<0.001 for both). Data on 72 VHR patients (60%) were obtained with follow!up duration of 19.7±5.8 months. No patient achieved the target LDL!C level of less than 1.4 mmol/L. Conclusion. Three!year follow!up of participants in the RENAISSANCE registry shows an enhanced adherence to hypolipidemic therapy. In FH patients the increased risk of new CVE is associated with male sex, hypertension, CHD, family history of CVD and lipoprotein(a) level ≥30 mg/dl
The aim of the study was to investigate Klotho protein levels in men with type 2 diabetes blood and its associations with several cardiometabolic risk factors. Material and methods. The study included 37 men with diabetes and 141 men without diabetes. Fasting blood samples were collected to measure Klotho protein levels and some biochemical parameters. Results and its discussion. The Klotho protein level in men with diabetes was significantly lower than in men without diabetes (374 [117; 500] and 515 [315; 1009] pg/dl, p<0.0001). Among the examined men with diabetes with a glomerular filtration rate of less than 60 ml/min/1.73 cm2, the concentration of Klotho protein was 4 times lower than in the comparison group (104 [93; 118] and 413 [147; 535] pg/dl, p = 0.014) In men with diabetes, the Klotho protein was inversely correlated with the ratio of waist to hip circumference (-0.329; p = 0.047). But with multivariate analysis, only a tendency towards a negative association of the Klotho protein with abdominal obesity was determined (-0.385, p = 0.078). Conclusion. The content of Klotho protein in men with diabetes is significantly lower, especially in middle-aged men and in those with a reduced glomerular filtration rate. In men with diabetes, the Klotho protein has a negative correlation with the presence of abdominal obesity. In a multivariate analysis among men with diabetes, the Klotho protein tends to be inversely associated with the presence of abdominal obesity.
