Here we discuss the recent trends in developing surgical technologies, their impact on the incidence of healthcare associated infections (HAIs), and approaches for the efficient infection control. Features of the contemporary surgery include permanent increase in the number of minimally invasive interventions, implants, and transplants, ongoing development of surgical equipment, and constant improvement in anesthesiology techniques, along with the decrease in use of natural suture materials and length of hospital stay all resulting in a significant decline in the incidence of HAIs. Perioperative use of antimicrobial agents, refusal of preoperative shaving, implementation of barrier methods, and improvement in sterilization also contributed to this trend. However, the risk of severe complications significantly increased, and the multidrug-resistant microorganisms became widespread. Currently, permanent monitoring and risk assessment are the main approaches to overcome these problems.

Основные положения Резюме Доктрина профилактики инфекций, связанных с оказанием медицинской помощи (ИСМП), представляет собой декларацию о политике государства в области эпидемиологической безопасности медицинской помощи. Это система взглядов и положений, устанавливающая направления профилактики ИСМП, способы и формы их реализации. Наличие глубоко проработанной доктрины позволяет иметь базу для принятия решений. По заключению экспертов Всемирной организации здравоохранения, ни один тип медицинских учреждений ни в одной стране не может претендовать на то, чтобы быть свободным от риска возникновения ИСМП. Четыре ключевых положения лежат в основе риск-ориентированного подхода к ИСМП: 1 -риск ИСМП в медицинской организации существует всегда; 2 -риск ИСМП определяется степенью агрессии и инвазии, эпидемиологической безопасности применяемых медицинских технологий, свойствами возбудителей и условиями больничной среды; 3 -необходимость перехода от оценки и управления эпидемиологической ситуацией по заболеваемости к оценке потенциального риска, риск-менеджменту и риск-ориентированным технологиям профилактики; 4 -эпидемиологическая безопасность -неотъемлемая составляющая обеспечения качества и безопасности медицинской помощи. Резюме. На территории РФ в общей структуре острых кишечных инфекций удельный вес вирусных диарей среди детей колеблется в зависимости от сезона от 24 до 78% случаев. Этиологическая расшифровка вирусных острых кишечных инфекций проводится в основном среди пациентов инфекционных стационаров. Вопрос о распространенности вирусных острых кишечных инфекций в неинфекционных стационарах, в том числе как инфекций, связанных с оказанием медицинской помощи, остается недостаточно исследованным. Опреде-ление доли вирусного компонента в общей структуре острых кишечных инфекций сводится преимущест венно к определению процентного соотношения ротавирусной инфекции без учета других возбудителей. В рамках эпидемиологического надзора за внутрибольничными инфекциями вирусной этиологии в детском много-профильном неинфекционном стационаре проведено изучение этиологической структуры вирусных острых кишечных инфекций и молекулярно-генетическая характеристика выявленных кишечных вирусов. Была внедрена синдромальная диагностика случаев острых кишечных инфекций -выявление и обследование па-циентов с признаками дисфункции желудочно-кишечного тракта, не связанной с основным заболеванием. Комплекс лабораторных методов включал: выявление и дифференциация ДНК (РНК) различных кишечных возбудителей методом ОТ-ПЦР; генотипирование кишечных вирусов методом секвенирования; анализ ну-клеотидных последовательностей фрагментов кДНК с применением пакета программ BLAST для идентифи-кации близкородственных штаммов и онлайн-сервиса для автоматического генотипирования норовирусов Norovirus Genotyping Tool Version 1.0. Выравнивание нуклеотидных последовательностей и филогенетический анализ осуществляли с помощью программного обеспечения MEGA. Полученные последовательности фраг-ментов генома представлены в международной базе данных GenBank. Использование молекулярно-генетиче-ских методов исследования позволило дифференцировать вирусных возбудителей острых кишечных инфек-ций и установить факт внутрибольничной передачи. Доля острых кишечных инфекций вирусной этиологии среди пациентов, имеющих клинические признаки кишечной инфекции, и контактных лиц составила 43,8%. Infections caused by opportunistic microorganisms occupy a special position. In microbial associations of opportunistic microorganisms is easier to implement its low pathogenic potential, activate properties of each other. Group polyetiologic infections caused by conditionally pathogenic microorganisms are quite diverse in terms of a nosological forms and groups. These include the so-called septic infection, which are also very diverse and clinical, epidemiological and etiological characteristics. Scientific evidence shows the urgency of the problem mixed infections, especially in surgery and indicate a lack of systematic data about the whole spectrum and the composition of microbial associations in different infections, quantification, insufficient study of the properties compared to monoagents. The problem mixed infections practically not been studied with epidemiologic standpoint, only a few studies devoted to the study of nosocomial infection of microbial associations are not developed microbiological monitoring. In this connection, you need comprehensive research mixed infections compared with monoagents to identify the characteristics and development of epidemiologic surveillance and control.
