This review deals with the modern concepts of age-related changes in protective functions of the skin with an emphasis on the impairment of the interaction between the immune cells of innate and acquired immunity, which leads to decrease in antigenspecific T-cell immune surveillance in the skin. We discuss various defects of T-cells and their environment and address the issue of possible correction of activity of T-reg and other cells in the skin, which would improve the immunological surveillance in elderly people and reduce the risk of developing malignant tumors or skin infections. В данном обзоре мы сфокусировались на современ-ных концепциях понимания возрастных изменений защитных функций кожи с акцентом на то, каким образом нарушение взаимодействия между иммун-ными клетками врожденного и приобретенного им-мунитета приводит к снижению антиген-специфич-ного Т-клеточного иммунологического надзора в коже. Кожа и иммунитетКожные покровы формируют границу раздела тела с окружающей средой и их основная функция -барьерная. Кожа защищает организм, делая его не-проницаемым для множества вредных экзогенных субстанций, и поддерживает гомеостаз, предотвра-щая потерю влаги и тепла. Вдобавок это высокоспе-циализированная иммунная система, состоящая из резидентных, активированных или мигрировавших в ткань лейкоцитов (см. таблицу). Эти клетки распреде-лены в эпидермальных и дермальных слоях кожи и участвуют в механизмах врожденного и приобретен-ного иммунитета. Они отвечают за распознавание «своего» и «чужого», что является фундаментально важным, так как кожа ежедневно контактирует с эк-зогенными веществами. В статье представлен цитокиновый профиль сыворотки венозной крови и отделяемого из уретры мужчин с хроническим уретритом герпетической и хламидийной этиологии. Выявлены наиболее специфические иммунологические показатели, раскрывающие напряженность иммунной системы при хронической вирусной и бактериальной инфекции на системном -ФНОα (AUC=1), ИЛ-1β (AUC=0,97), ИЛ-10 (AUC=0,87), ИЛ-13 (AUC=0,97), и локальном уровнях -ИЛ-13 (AUC=0,99) и ИЛ-22 (AUC=0,99). Произведена оценка собственного эффекта иммуномодуляторов в лечении герпетической и хламидийной урогенитальной инфекций. Собственный эффект применения рекомбинантного интерлейкина-2 составил 7 % (при этом у 76 % пациентов значения исследуемых медиаторов воспаления вернулись к контрольным уровням), а рекомбинантного интерферона-γ -13 % (при этом у 89 % мужчин показатели соответствовали контролю). The article presents a cytokine profile of the serum of venous blood and urethral discharge of men with chronic urethritis of herpetic and chlamydial etiology. The most specific immunological indices revealed the intensity of the immune system in chronic viral and bacterial infections at the systemic level -TNF-α (AUC = 1), IL-1β (AUC = 0.97), IL-10 (AUC = 0.87), IL -13 (AUC = 0.97), and local levels -IL-13 (AUC = 0.99) and IL-22 (AUC = 0.99). The effect of immunomodulators in the treatment of herpetic and chlamydial urogenital infection was estimated.The intrinsic effect of the use of recombinant interleukin-2 was 7% (in 76% of patients the values of the examined inflammatory mediators returned to the control levels), and recombinant interferon-γ was 13 % (in 89 % of the men the values corresponded to control).
Investigation of the immune status and TNF-α, IFNγ and IL-10 concentration in the ejaculate and venous blood serum of 100 men with STI (marked as main group (MG)) has been conducted. MG men were divided into 2 subgroups: 50 men with chronic viral urethritis (genital herpes-herpetic urethritis (HU)) and 50 patients with с viral-bacterial urethritis (genital herpes, associated with chlamydiosis-herpetic-chlamydial urethritis (HCU)). Comparative group (control group) were included 20 apparently healthy male-volunteers. More perceptible disfunctions of the local and systemic immunity have been revealed in patients with viral-bacterial urethritis: elevating of CD3+ CD25+ expression as well as indicators of late negative activation markers have been recorded on the background of phagocytosis indicators reducing, reliable rising of TNFα level in the venous blood serum have been marked, similar cytokine level in the ejaculate has been considerably higher as compared with the herpetic urethritis group and controls; also IFNγ/IL-10 ratio lowering has been noted either in the venous blood serum, or in the seminal fluid compared with the control group.
