В клинике Иркутского научного центра хирургии и травматологии проведено одноцентровое, рандомизированное, контролируемое, частично ослеплённое исследование, направленное на определение функциональных результатов хирургического лечения пациентов с повреждением пяточного сухожилия с различной степенью диастаза по классификации Myerson. На основе анализа оперативного лечения 20 пациентов с повреждением пяточного сухожилия, разделённых на две группы (с диастазом II и III степени по Myerson), показана эффективность хирургического лечения с использованием сухожилия подошвенной мышцы. Оценивались следующие критерии: 1) давность с момента травмы, длина диастаза, длительность операции; 2) болевой синдром после операции, отказ от средств дополнительной опоры; 3) оценка функциональных результатов по шкале AOFAS. При анализе отмечаются статистически значимые различия в исследуемых группах пациентов по критерию «давность с момента травмы» и «длина диастаза». Также в результате исследования выявлены статистически значимые различия по критерию «длительность операции». Благодаря использованию сухожилия подошвенной мышцы в качестве стабилизирующего фактора при восстановлении пяточного сухожилия не выявлено разницы между группами по уровню и длительности болевого синдрома после операции, по срокам отказа от средств дополнительной опоры после оперативного лечения, а также при комплексной оценке функциональных результатов лечения по шкале AOFAS.

Среди всех повреждений коленного сустава доля повреждений передней крестообразной связки (ПКС) достигает 65 % [4,9,19].Основной контингент пациентов с повреждения-ми передней крестообразной связки составляют лица молодого трудоспособного возраста, ведущие физиче-ски активный образ жизни, занимающиеся спортом. В статье приводится клиническое наблюдение оперативного лечения пациента с застарелым разрывом пяточного сухожилия с применением пластики ахиллова сухожилия по Чернавскому при дефектах III степени по Myerson в сочетании с короткой гипсовой иммобилизацией и ранней активизации пациентов, что позволяет сократить период реабилитации пациентов после оперативного лечения. Ключевые слова: пяточное сухожилие, ахиллово сухожилие, пластика по Чернавскому Heel tendon injuries is the most common trauma of tendomuscular apparatus. Old ruptures of heel tendon occur in 58 % of cases in humans. The aim of the research was to estimate the effectiveness of treatment of the patients with old ruptures of heel tendon with Myerson type III defects who had tendon reconstruction on Chernavsky and short plaster splint immobilization of lower extremity for 4 weeks. We operated 10 patients using reconstruction on Chernavsky in our clinic from 2012 to 2014 (average age -47,6 ± 12,0 years, 8 males and 2 females). All patients had old ruptures of heel tendon. Average time from the moment of trauma till the operation was 112,6 ± 80,4 days. Diastasis between the ends of tendon was 5,8 ± 0,7 cm that corresponds to Myerson type III.Average term of staying at hospital was 10 ± 2 days. We didn't register any complications in postoperative period. Term of plaster immobilization of the operated extremity was 4 weeks. AOFAS score was 34,2 ± 6,8 points at the control examination in 1,5 months that corresponds to bad functional result. We registered good functional results (88,6 ± 3,5 points) in 3 months. In 6 months, functional results were 95,9 ± 1,6 points. In 12 months after the operation average score was 97,9 ± 2,1 points that corresponds to excellent functional result. Using heel tendon reconstruction on Chernavsky at the Myerson type III defects in combination with short plaster immobilization and early activization of patients allows to decrease term of rehabilitation of patients after the operation.
Fractures associated with osteoporosis, due to the high prevalence and high percentage of related complications, are a serious problem for modern traumatology and orthopedics. Among all injuries of the upper extremities, fractures in the proximal humerus occur in 32-65 % of cases. Fractures of the proximal humerus account for 4-5 % of all fractures and 50 % of fractures of the humerus. The aim of the research was to develop a new method for the surgical treatment of fractures of the proximal humerus against the background of osteoporosis and to evaluate the clinical effectiveness of the new method. A pilot study was conducted to evaluate the clinical efficacy and safety of surgical treatment of patients with a fracture of the proximal humerus. The results showed that the new "Method for the surgical treatment of patients with a fracture of the proximal humerus" is clinically effective and safe. Additional intramedullary bone stabilization of the proximal humerus with a fibular autograft allows for more rigid and stable fixation of fragments, especially in the presence of critical osteoporosis. The early restoration of passive and active movements in the shoulder joint made it possible to fully restore the function of the limb in a severe fracture of the proximal section.
Rotator cuff ruptures are the most common injuries of shoulder joint with an incidence of about 20 %. This pathology is more common in adults over 60 years of age because it is caused by degenerative changes in the tendon. Massive injuries account for 10–40 % of all rotator cuff injuries. Currently, there is no unified surgical tactics for the treatment of patients with massive ruptures of rotator cuff tendons.The aim. To assess the efficiency of transposition of the latissimus dorsi tendon in patients with massive ruptures of the rotator cuff tendon.Materials and methods. The study included 15 patients with Patte stage III massive ruptures of the rotator cuff who had transposition of the latissimus dorsi tendon.Results. The article presents clinical cases of surgical treatment of patients. The following criteria were assessed: mean age; time since injury; duration of the surgery; blood loss volume; functional results by the ASES (American Shoulder and Elbow Surgeons) Shoulder Score. Taking into account the ASES Shoulder Score indicators 1 year after the surgical treatment, the following results were obtained: excellent results – in 9 (53.3 %) cases; good results – in 1 (13.4 %) case; satisfactory results – in 5 (33.3 %) cases.Conclusion. When preserving the articular cartilage, the method of choice in the treatment of patients with massive ruptures of rotator cuff tendons is transposition of the latissimus dorsi tendon. At the same time, an incomplete restoration of the function of the injured limb was registered in 33.3 % of patients, which requires further study and modification of the known method of transposition of the latissimus dorsi tendon.
