ВВЕДЕНИЕВзгляд на магний как специфическое средство, используемое преимущественно для лечения судорог у недоношенных детей [1], устарел по меньшей мере на 50 лет. Воздействие иона магния на организм ребенка вовсе не ограничивается профилактикой судорог, и приводи-мые в настоящей работе результаты анализа белков про-теома человека указывают на широкий спектр возмож-ных применений препаратов магния в педиатрии. Данная работа продолжает серию работ по системно-биологиче-ским анализам белков и генов, участвующих в реализа-ции эффектов микронутриентов; в одной из первых работ были представлены результаты системно-биологи-ческого анализа эффектов витамина D [2].Магний является одним из четырех важнейших элек-тролитов организма. Обязательное комплексообразова-ние ионов Mg 2+ с молекулой АТФ, необходимое для син-теза и осуществления функций АТФ, указывает на фунда-ментальное значение иона магния для поддержки мета- ПРОТЕОМНЫЙ АНАЛИЗ МАГНИЙ-ЗАВИСИМЫХ БЕЛКОВ В СИСТЕМЕ «МАТЬ -ПЛОД -РЕБЕНОК»Системно-биологический анализ более 700 магний-зависимых белков протеома человека указал на 7 важнейших направле-ний действия магния: (1) эмбриональное развитие, (2) энергетический метаболизм, (3) процессы передачи сигналов от рецепторов, (4) неврологические роли, (5) поддержка структуры соединительной ткани, (6) сердечно-сосудистые и (7) имму-нологические роли. Дефицит магния во время беременности будет способствовать развитию пороков развития (скелетных дефектов, рахита, грыж диафрагмы, лицевых пороков, краниосиностоза, нарушений строения ретины и зрения, брахидакти-лий). В раннем возрасте дефицит магния ассоциирован с синдромом внезапной смерти, в дошкольном и подростковом воз-расте приводит к нарушениям функции мышц, что в случае миокарда ассоциировано с нарушениями сократимости желудоч-ков сердца и аритмией. Для дефицита магния также характерны митохондриальные нарушения, гиперинсулинемия, наруше-ния структуры кожи и ее придатков, опухолевые заболевания и заболевания, ассоциированные с нарушениями энергетиче-ского метаболизма (в т. ч. гипопаратиреоз и анемия). Эффекты дефицита магния у плода и у детей существенно утяжеляются на фоне недостаточности витамина В6 (пиридоксин). Результаты протеомного анализа позволили указать на соответствующие молекулярно-физиологические механизмы синергизма магния и пиридоксина. В целом результаты настоящего анализа ука-зывают на весьма обширные области для коррекции недостаточности магния и пиридоксина с целью профилактики и терапии широкого круга заболеваний, начиная с периода внутриутробного развития и раннего детства до подросткового периода. The review presents modern aspects of the etiology of placental insufficiency (uteroplacental vascular insufficiency), fetal growth restriction and preeclampsia, which arises primarily due to deficient remodeling of the uterine spiral arteries supplying the placenta during early pregnancy. The embryonic, maternal and placental factors of the occurrence of placental insufficiency and placental-related pathology considered. The issues of prevention and treatment of placental insufficiency are considered taking into account the common pathogenesis of this pathological condition.
The search for original publications on fundamental and clinical medicine that would produce results of the highest scientific quality represents an urgent need for every medical researcher. Such publications are essential, in particular, for the development of reliable treatment standards. The Englishlanguage resources PUBMED and EMBASE are essential to help in solving this problem. However, there is an obvious problem in assessing the quality of the studies found. The paper formulates a method for analyzing the texts of biomedical publications, which is based on an algorithmic assessment of the emotional modality of medical texts (so-called sentiment analysis). The use of the topological theory of data analysis made it possible to develop a set of high-precision algorithms for identifying 16 types of sentiments (manipulative turns of speech, research without positive results, propaganda, falsification of results, negative personal attitude, aggressiveness of the text, negative emotional background, etc.). On the basis of the developed algorithms, a point scale for assessing the sentiment quality of research was obtained, which we called the "β-score": the higher the β-score, the less the evaluated text contains manipulative language constructions. As a result, the ANTIFAKE system (http://antifake-news.ru) was developed to analyze the sentiment-quality of Englishlanguage scientific texts. An analysis of ~ 20 million abstracts from PUBMED showed that publications with low sentiment quality (β-score <0, that is, that the prevalence of manipulative constructions over meaningful ones) is only 19 %. In the overwhelming majority of thematic headings (27,090 out of 27,840 headings of the MESH system PUBMED), a positive dynamics of sentiment quality of the texts of publications is shown by years). At the same time, as a result of the study, 249 headings were identified with sharply negative dynamics of sentiment quality and with a pronounced increase in manipulative sentiments characteristic of the "yellow" English-language press. These headings include tens of thousands of publications in peer-reviewed journals, which are aimed at (1) legalizing ethically unacceptable practices (euthanasia, perversions, so-called "population control", etc.), (2) discrediting psychiatry as a science, (3) media the war against micronutrients and (4) discrediting evidence-based medicine under the guise of developing the so-called "international standards of evidence-based medicine". In general, the developed system of artificial intelligence allows researchers to filter out pseudoscientific publications, the text of which is overloaded with emotional manipulation and which are published under the guise of "evidence-based standards".
A review of studies on chronic endometritis enables the authors to conclude that bacterially contaminated endometrium is the reason causing reduced fertility in case of confirmed subclinical inflammation. Chronic subclinical inflammation of the endometrium is often associated with genital endometriosis, which apparently leads to abnormal uterine bleedings. The identification of a microbial agent in case of endometritis in the routine outpatient practice is not mandatory because global experience including high-level evidence-based studies confirms the expediency of an empirical therapy based on minimum clinical criteria; otherwise, there may be irreversible consequences for the reproductive system. A preventive antibacterial therapy during the pre-conception period in infertile patients suffering from recurrent miscarriages is an efficient method to prevent reproductive losses.
В рамках II Национального конгресса «Дискуссионные вопросы современного акушерства» и XI Всемирного конгресса по перинатальной медицине, который прошел в Санкт-Петербурге 16-18 июня 2013 г., значительное внимание было уделено проблеме преждевременных родов, особенно очень ранних (22-27 недель), которые являются основной причиной неонатальной заболеваемости и смертности. Обсуждались патогенетические причины и факторы риска, а также возможности медикаментозной профилактики преждевременных родов.
