10омплексные проблемы сердечно-сосудистых заболеваний К Основные положения • Предложена принципиально новая классификация дисфункций биологических протезов кла-панов сердца, которая выделяет процесс естественного старения (износа) биологического кла-пана в самостоятельную этиопатогенетическую единицу, что позволило разделить дисфункции, развивающиеся в «идеальных» и «агрессивных» условиях среды реципиента.• Изучение неблагоприятных реципиент-ассоциированных факторов позволит идентифициро-вать их вклад в развитие структурных изменений ксеноткани и соответственно стратифицировать риски развития дисфункции БП. РезюмеВ настоящей работе проведен анализ современных публикаций по механиз-мам развития дисфункций биологических протезов (БП) клапанов сердца. На основании литературных данных о хорошо известных, а также в насто-ящее время активно изучаемых причин нарушения работы БП предпринята попытка выделить основные патогенетические направления формирования дисфункции БП. Помимо процесса естественного старения (износа) ткани клапана, развивающегося в ходе непрерывных циклических механических воздействий, и сопровождающегося формированием очагов кальцифика-ции путем пассивного и активного процесса минералообразования, прове-ден анализ возможного неблагоприятного влияния протез-обусловленных и реципиент-ассоциированных факторов. К протез-обусловленным причинам развития дисфункций были отнесены технологические и технические фак-торы, неблагоприятное воздействие которых на клапан, возможно на этапе предимплантационной подготовки и непосредственно в ходе имплантации. С позиций реципиент-ассоциированных неблагоприятных условий для дол-госрочного функционирования биологического протеза рассмотрены основ-ные дисметаболические, иммунные, гемостазиологические и гиперпроли-феративные (гиперпластические) состояния. На основании этого впервые предложена классификация дисфункций БП, основанная на патогенетиче-ских механизмах формирования нарушения работы клапана с учетом их мор-фологических проявлений. Every year, the frequency of use of biological prostheses for treatment of valvular heart disease increases.The implantation of a biological prosthesis allows avoiding lifelong anticoagulant therapy, but at the same time it may require further replacement due to the dysfunction development.Traditional reoperation is associated with a certain risk of trauma to various structures and cardiac chambers during explantation of non-functioning valve.Transcatheter replacement of the prosthesis (“valve-in-valve”) is not always realizable due to lack of routes for the delivery system and small diameter of the initial valve.“Valve-in-valve” is not always demanded because it does not provide an option to choose mechanical or biological prosthesis.Valve-on-valve implantation technique implies that the new prosthesis is implanted in the carcass of the previous valve. It allows avoiding severe intraoperative complications, reducing the duration of the main stage of the surgery and adverse events in the early postoperative period, and provide an option to choose implantable device.In this review, the authors attempted to comprehensively evaluate the world experience in applying the “valve-on-valve” technique.
Aim.To conduct comparative analysis of life quality (LQ) of the recipients of mechanical and biological prostheses in mitral valve defect after primary and repeat intervention. Material and methods. By the continuous sampling method, with sF-36 questionnaire, we evaluated LQ in 245 patients, operated for mitral valve defect from 1996 to 2013 y. in FsBI sRICCDP. The groups of the recipients of mechanical, biological and "reoperated" patients were 82, 104 and 59, respectively. Results. The parameters of the health physical component had maximum importance for bioprosteses recipients (рgH≤0,05; рRP≤0,05; рPF>0,05; рBP>0,05) and did not almost differ (excluding RF) in groups of mechanical valves and "reoperated" (р>0,050). Psychological health components (Re, VT, MH) had higher values in bioprostheses recipients (р≤0,050) and did not significantly differ in mechanical valves groups and in "reoperated". There were no differences between the groups in social functioning assessment. Conclusion.The usage of bioprosthesis in mitral valve defect increases LQ parameters. LQ of the recipients of mechanic valves and of the patients after reprosthesing of mitral valve does not differ significantly. Teaching of the patients in "school of the patients with prostheses cardiac valves" allows for the significant improvement of LQ, mostly by psychological component of the health.
