The novel coronavirus 2019 (COVID-19) pandemic has affected not only the somatic health of people but also their mental health and the organization of mental health care. Self-isolation, quarantine, extensive media coverage of the situation, a large amount of inaccurate information, and conflicting recommendations – all this has led to an increase in admission of patients who have not previously had mental illness to psychiatrists. As a confirmation of the influence of the above factors on the population's mental health, we present a case report of a 25-year-old patient who first came to a psychiatric clinic due to the obsessive-compulsive disorder that developed due to the pandemic.
Цель исследования -оценить эффективность и безопасность различных схем терапии депрессии в зависимости от клинического варианта биполярного аффективного расстройства (БАР) и выбрать оптимальные схемы лечения депрессии при БАР I типа (БАР I) и БАР II типа (БАР II) (80,0; 72,7 и 33,3% соответственно). При этом частота возникновения симптомов транзиторной гипомании в этих группах достовер-но не различалась (20,0; 27,3 исследования -60,6; 92,9; 77,8 и 69,2% соответственно), у них реже отмеча-лись симптомы противоположного полюса на фоне лечения (18,2; 7,1; 0,0 и 15,4% соответственно). The depressive patients with BADII generally responded better to different therapy regimens (the reduction in depressive symptoms was 52.08, 58.82, 58.40, and 53.98% in the AD, NL, AD + NT, and AD + NL groups; the remission index by the end of the investigation was 60.6, 92.9, 77.8, and 69.2%, respectively); these patients were seen to have less frequently symptoms of an antipole during their treatment (18.2, 7.1, 0.0, and 15.4%, respectively и 8,3% соответственно). Пациенты с депрессией в рамках БАР II в целом лучше отвечали на различ-ные схемы терапии (степень редукции депрессивной симптоматики -52,08% у получавших АД, 58,82% -НЛ, 58,40% -АД + НТ, 53,98% -АД + НЛ; показатель ремиссий к концу Заключение. У пациентов с БАР введение в схему терапии АД ускоряет выход из депрессии любой тяжести и значительно повы-шает эффективность лечения. В зависимости от клинической картины депрессии может применяться как монотерапия АД (БАР II), так и комбинация АД + НТ и/или НЛ (БАР I, БАР II). Включение в схему терапии
Депрессии у больных шизофренией в период ремиссии встречаются довольно часто, однако их генез и клинические особенности изучены недостаточно. Цель исследования-оценка клинических особенностей депрессий, возникающих у больных приступообразной шизофренией в период ремиссии. Пациенты и методы. Обследовано 88 больных с диагнозом «приступообразная шизофрения в стадии лекарственной ремиссии», находящихся в состоянии непсихотической депрессии. Всего проанализировано 200 депрессивных эпизодов. Состояние больных оценивалось в соответствии с диагностическими критериями депрессивных расстройств по МКБ-10. Для количественной оценки использовали психометрические шкалы: шкалу депрессии у больных шизофренией Калгари (Calgary Depression Schizophrenia Scale, CDSS), субшкалу оценки негативных симптомов (Negative Subscale PANSS). Результаты. Выделено два типа постшизофренических депрессий: эндогенные и реактивные. Эндогенные депрессии подразделяют на постпсихотические, развивающиеся в период становления ремиссии, и отсроченные депрессивные эпизоды; реактивные депрессии-на нозогенные и психогенные. Эндогенные депрессии представляют собой этап шизофренического процесса, реактивные-депрессивную реакцию больного шизофренией на психотравмирующую ситуацию. Последние возникают у пациентов с меньшей выраженностью шизофренического дефекта, более высоким уровнем критического отношения к болезни и более качественной социально-трудовой адаптацией. Эндогенные депрессии в среднем имеют большую продолжительность и тяжесть, включают в себя, помимо тревожных симптомов, астенические и апатические, а реактивные депрессии-только тревожные и тоскливые проявления. Заключение. Знание особенностей депрессий при шизофрении позволит проводить более точную их диагностику и назначать адекватную терапию.
