Изучить спектр и динамику антибиотикорезистентности Acinetobacter baumannii, оценить механизмы устойчивости к карбапенемам и чувствительность к комбинациям антибиотиков. Материал и методы. Для 305 клинических изолятов A.baumannii определена чувствительность к антибиотикам автоматизированным методом. Детекция генов карбапенемаз выполнена методом ПЦР в реальном времени. Чувствительность к комбинациям антибиотиков определена методом тестирования бактерицидности разных комбинаций. Выводы. Показано увеличение доли A.baumannii в этиологической структуре инфекционной патологии. Отмечено увеличение антибиотикорезистентности. Выявлено наличие генов карбапенемаз ОХА-40. Не обнаружено изолятов, устойчивых к колистину. Комбинации меропенем-колистин, амикацин-колистин, левофлоксацин-колистин проявляли бактерицидную активность в отношении 100% изолятов. ВведениеAcinetobacter baumannii -условно-патогенный микроорганизм, вызывающий инфекционные заболевания на фоне иммуносупрессии. У пациентов отделений реанимации и интенсивной терапии, травматологических и ожоговых отделений ацинетобактеры могут вызывать пневмонию, инфекции кровотока, катетер-ассоциированные и раневые инфекции, инфекции мочевыводящих путей [1]. С начала 2000-х годов во многих странах мира отмечается увеличение частоты возникновения ацинетобактерных инфекций, сопровождающихся быстрым распространением устойчивости возбудителей к антимикробным препаратам (АМП) [1, 2]. Для определения уровней антибиотикорезистентности ацинетобактеров используются международные согласительные критерии: мультирезистентность (MDR -multidrug resistance) -нечувствительность по крайне мере к одному АМП в трех и более категориях АМП, экстремальная резистентность (XDR -extensively drug resistance) -нечувствительность по крайне мере к одному АМП во всех категориях АМП, за исключением 1-2 категорий, панрезистентность (PDRpandrug resistance) -нечувствительность ко всем АМП во всех категориях [3]. Важной причиной развития MDR и XDR у A.baumannii является продукция β-лактамаз класса D, способных разрушать карбапенемы (карбапенемазы ОХА-23, ОХА-40, ОХА-56), а также метало-β-лактамаз (МБЛ) [4,5]. Продукция карбапенемаз часто ассоциирована с множественной устойчивостью к АМП разных групп -цефалоспоринам, фторхинолонам, аминогликозидам. В настоящее время только полимиксины сохраняют приемлемую микробиологическую активность в отношении многих карбапенемрезистентных штаммов A.baumannii [6].
Цель исследования -оценить этиологическую структуру инфекций кровотока при различных патологических состояниях и определить чувствительность их возбудителей к антибактериальным лекарственным средствам.Материал и методы. Проведено мультицентровое клиническое исследование по анализу 5892 клинических образцов крови от пациентов с различными патологическими состояниями, находящихся на стационарном лечении в многопрофильных лечебно-профилактических организациях здравоохранения. Микробиологическое исследование крови на стерильность выполняли классическим бактериологическим методом с использованием двухфазной среды либо с использованием автоматического гемокультиватора. Идентификацию выделенных гемокультур и определение их чувствительности к антибактериальным лекарственным средствам выполняли с использованием автоматического микробиологического анализатора.Результаты и обсуждение. Всего выделено 789 клинически значимых гемокультур, в их числе 471 (59,7%) штамм грамположительных бактерий, 290 (36,8%) штаммов грамотрицательных бактерий и 28 (3,5%) штаммов грибов рода Candida. Общая высеваемость составила 13,4% и была в 1,4 раза выше при использовании гемокультиватора по сравнению с выполнением исследования традиционным бактериологическим методом (соответственно 17,9 и 12,6%).Показано преобладание коагулазонегативных стафилококков у пациентов с инфекциями нижних дыхательных путей (45,5%), лихорадкой неясной этиологии (40,5%), инфекциями области хирургического вмешательства (34,5%), термическими и химическими ожогами (45,8%), эндокардитами (34,5%). Энтеробактерии с большей частотой выделены при бактериемии у больных со злокачественными новообразованиями (43,5%), инфекциями мочевыделительной системы (30,8%), сепсисом (39,2%). Грамотрицательные неферментирующие бактерии (Pseudomonas aeruginosa, Acinetobacter spp.) с наибольшей частотой идентифицированы у пациентов с онкопатологией (19,6%), ожоговой болезнью (27,1%), инфекционным эндокардитом (27,6%). Показаны высокие уровни резистентности инвазивных штаммов K. pneumoniae, A. baumannii, P. aeruginosa к карбапенемам, цефалоспоринам III-IV поколения, аминогликозидам, фторхинолонам.Заключение. Отмечены возросшая роль K. pneumoniae и грамотрицательных неферментирующих бактерий в этиологии инфекций кровотока, а также высокие уровни их резистентности к антибактериальным лекарственным средствам. Показаны большая эффективность и скорость выполнения микробиологического исследования при использовании автоматической системы для гемокультивирования в сравнении с традиционной методикой.Финансирование. Aim. Within the microbiological monitoring program, to study the prevalence of carbapenemase-producing K. pneumoniae in the healthcare organizations of the Gomel region and assess their level of resistance to antibacterial drugs. Materials and methods. For 91 clinical isolates of Klebsiella pneumoniae with multiple antibiotic resistance, isolated in Gomel and Gomel region, carbapenemase genes were detected by real-time PCR and sensitivity to antibacterial drugs was determined. Results. 68 carbapenemase producers were revealed: KPC — 1 isolate, OXA-48 — 47 isolates, NDM — 20 isolates. Carbapenemase producers were found in 11 Gomel health organizations and 8 central district hospitals of the regional centers of the Gomel region. All of them had an associated resistance to most antibiotics and retained sensitivity to colistin (91.2% sensitive isolates) and tigecycline (98.5%). Conclusion. The spread of carbapenemase-producing K. pneumoniae isolates in healthcare organizations makes it very difficult to conduct effective antibiotic therapy for patients and requires the introduction of appropriate infection control measures aimed at limiting their circulation in the hospital environment.
Objective: to assess the etiological structure of bacteremia in various pathological conditions. Material and methods. To check blood sterility, we analyzed results of 4548 blood tests of patients with lower respiratory tract infections, urinary tract infections, endocarditis, burns, cancer, fever of unknown etiology, sepsis. Cultural blood analysis was performed by means of the traditional bacteriological method or using an automated hemocultivator. Results. We isolated 477 clinically significant blood cultures. Gram-positive microorganisms prevailed over gram-negative in terms of the isolation rate, and fungemia was observed considerably most rarely (4.4%). Microbial inoculation found by the automated method was 1.5 times as high as that found by the traditional two-phase medium (14.7 and 9.5%, respectively). The etiological structure of microorganisms isolated as blood cultures in various nosological forms was assessed. Conclusion. We have revealed the prevalence of coagulase-negative staphylococci in patients with fever of unknown etiology (53.7 %), lower respiratory tract infections (39.5 %), endocarditis (31.4 %), burns (30.5 %) compared to other nosological forms. Enterobacteria were isolated with higher frequency in bacteremia in patients with malignant tumors (30.0 %) and urinary system infections (29.4 %), and gram-negative non-fermenting bacteria - with the highest specific weight in the isolated hemoculture associated with burn disease (31.6 %) and urinary system infections (29.4 %), enterococci - with greater frequency in bacteremia in patients with endocarditis (23.5 %).
