Составить алгоритм прогнозирования риска развития пароксизма фибрилляции предсердий у больных ишемической болезнью сердца на фоне постинфарктного кардиосклероза (ПИКС). Материал и методы. Проведен углубленный анализ ряда лабораторно-функциональных показателей (показателей перекисного окисления белков, липидов, антиоксидантной защиты и кожной микроциркуляции) у 85 пациентов с фибрилляцией предсердий (ФП). Все пациенты были разбиты на 4 группы: пациенты с пароксизмальной формой ФП без ПИКС (27 человек), пациенты с пароксизмальной формой ФП с ПИКС (12 человек), пациенты с постоянной формой ФП без ПИКС (26 человек) и пациенты с постоянной формой ФП с ПИКС (20 человек). Результаты. Создана математическая модель для прогнозирования развития фибрилляции предсердий у больных ПИКС с расчетом коэффициента вероятности фибрилляции предсердий. Методами корреляционного и факторного анализов установлено, что наибольшую ценность для прогнозирования пароксизма фибрилляции предсердий у больных ПИКС имеют два фактора: суммарный уровень продуктов глубокого окисления белков и показатель микроциркуляции при лазерной допплеровской флоуметрии. Диагностическая чувствительность разработанной прогностической модели составила 85,7%, специфичность теста-93,7%, диагностическая эффективность теста-90%. Заключение. В ходе исследования была доказана клинико-диагностическая и прогностическая ценность комплексного исследования показателей окислительно-восстановительных реакций и показателей микроциркуляции в сочетании со стандартными лабораторно-инструментальными исследованиями у больных ФП. Разработанная математическая модель позволяет прогнозировать развитие фибрилляции предсердий у пациентов с ПИКС.
Представлен первый случай пренатальной диагностики оро-фацио-дигитального синдрома I типа (OMIM 311200) у плода с частичной моносомией Хр22.2. Причиной оро-фацио-дигитального синдрома I типа являются мутации в гене OFDI, случаи заболевания вследствие хромосомных микроделеций, затрагивающих ген OFDI встречаются редко. При ультразвуковом исследовании плода во втором триместре беременности выявлены врожденный порок развития головного мозга и множественные эхографические маркеры хромосомной и синдромальной патологии. Биологический образец плода (пуповинная кровь) получен в 21 неделю гестации посредством кордоцентеза. Молекулярно-цитогенетический анализ ДНК, выделенной из лимфоцитов пуповинной крови плода, с применением матричной сравнительной геномной гибридизации выявил частичную моносомию региона Хр22.2, размером 2,6 млн п.н.: arr[hg19] Xp22.2(11135472_13798048)×1. Наличие перестройки, а также ее de novo происхождение, подтверждено количественной ПЦР в режиме реального времени. The first case of prenatal diagnosis of type I oro-facio-digital syndrome (OMIM 311200) in fetus with partial monosomy Xp22.2 is presented. The cause of type I oro-facio-digital syndrome is mutations in the OFDI gene, cases of disease due to chromosomal microdeletions affecting the OFDI gene are rare. Ultrasound examination of the fetus in the second trimester of pregnancy revealed congenital defect of brain development and multiple echographic markers of chromosomal and syndrome pathology. An umbilical cord blood was obtained in 21 weeks of gestation by cordocentesis. aCGH revealed partial monosomy Xp22.2 at 2.6 Mb in size: arr [hg19] Xp22.2(11135472_13798048) × 1. The presence of microdeletion, as well as its de novo origin, was confirmed by the quantitative real time PCR. The use of high-resolution molecular cytogenetic analysis significantly expands the possibilities of syndrome monogenic pathology diagnosis in the prenatal period.
Background: Atrial fibrillation is one of the most common and heavy disorders of heart rhythm, which affects 0,4% of the general population and more than 5% among people aged 65 and older. The prevalence has increased six times in the last 25 years. AF occurs more often in men (10 times), less-in women (4 times). Therefore atrial fibrillation has lately become an issue of intensive clinical research. The challenges deal with a recurrent character of episodes, their unpredictiveness as well as a question of feasibility of attesting AF as a permanent disease. The answers to these questions may determine a choice of conservative or surgical treatment, which contributes to quality of life and life expectancy in patients. Materials and methods: The study included 85 patients with atrial fibrillation and 30 somatically healthy individuals. All subjects underwent a comprehensive study of the protein, lipid oxidative stress and microcirculation indicators. Findings: It was established that the formation of the clinical course of is-chemic heart disease is interconnected with the intensification of the processes of peroxidation of proteins, lipids and a change in superoxide dismutase activity. It has been proven that in patients with AF (both paroxysmal and permanent forms) in combination with a previous infarction, there is a change in peripheral microhemodynamics: a decrease in tissue perfusion and inhibition of active modulating mechanisms of tissue microcirculation. Conclusion: According to the correlation and factor analysis, two factors have been identified that have a significant interrelation with the development of paroxysmal AF in patients with previous myocardial infarction: the level of advanced oxidation protein products and the microcirculation index. Our mathematical model for the risk of paroxysmal AF in patients with previous myocardial infarction has a level of statistical significance of 0,020, which indicates the reliability of the prediction results. В последнее время происходит активное внедрение высокоразрешающих методов диагностики для выявления хромосомных аномалий у человека. Так, использование микрочиповой диагностики (aCGH) позволяет выявлять хромосомные аномалии у 6 % плодов с нормальным кариотипом, определённым стандартными цитогенетическими методами. При этом для пренатальной диагностики aCGH используется в значительно меньших объемах. В первую очередь это обусловлено получением большого объема информации и сложностями её интерпретации. В настоящей работе приводятся результаты использования aCGH для пренатальной диагностики в НИИ медицинской генетики Томского НИМЦ. Исследованы 44 образца плодного материала методом aCGH. В 20 случаях (45 %) идентифицирован геномный дисбаланс, сбалансированный кариотип был выявлен в 21 образце (48 %). В трех случаях (7 %) результат не был получен вследствие недостаточного количества или низкого качества биологического материала. Среди 20 случаев с хромосомными аномалиями было 4 образца с анеуплоидиями и 16 образцов с вариациями числа копий ДНК (CNV), из них 13 делеций и 10 дупликаций. Все CNV, выявленные в результате исследования, не представлены или представлены единичными случаями в базе данных DGV. По клинической значимости выявленные CNV распределились следующим образом: 5 аномалий с неясной клинической значимостью, 5 вероятно патогенных и 11 патогенных. Патогенные CNV были представлены 7 делециями и 4 дупликациями, идентифицированными в 10 образцах. Кроме «целевых» патогенных CNV, выявляются и так называемые случайные патогенетически значимые CNV. Случайные находки характерны для всех широкогогеномных и полногеномных исследований и вызывают сложности при медико-генетическом консультировании, связанные с информированием пациентов и возможным прерыванием беременности. В настоящем исследовании было выявлено по пять CNV (24 %), относящихся к группам случайных патогенных и с неясной клинической значимостью. Результаты настоящей работы свидетельствуют о необходимости использования современных молекулярно-цитогенетических методов для пренатальной диагностики хромосомных аномалий. При этом необходимо учитывать возникновение новых методических и этических вопросов, которые наиболее остро стоят при использовании этих методов в пренатальной диагностике. Recently, there has been an active introduction of high-resolution diagnostic methods for detecting chromosomal abnormalities in humans. Thus, the use of microarray diagnostics allows detecting 6% more chromosomal abnormalities in fetuses with a normal karyotype based on the results of standard cytogenetic methods. At the same time, as part of the prenatal diagnosis, aCGH is used in much smaller amounts. This is primarily due to the receipt of a large amount of information and difficulties with its interpretation. This paper presents the results of using the aCGH method in prenatal diagnosis at the Research Institute of Medical Genetics of the Tomsk National Research Medical Center. In the framework of prenatal diagnosis, 44 samples of fetal material were examined using the aCGH method. Prenatal diagnosis using microarray in 20 cases (45%) allowed to identify chromosomal abnormalities, a balanced karyotype was detected in 21 samples (48%). In three cases (7%) the result was not obtained due to insufficient quantity or low quality of the biological material. Among the 20 cases with chromosomal abnormalities, 4 samples with aneuploidy and 16 samples with CNV. In 16 samples of fetal material without aneuploidy, 13 deletions and 10 duplications were identified. All the adjustments included in this list are not represented or represented by isolated cases in the Database of Genomic Variants. When analyzing the clinical significance of the CNV, were identified 11 pathogenic, 5 probably pathogenic and 5 CNV with uncertain significance. Pathogenic rearrangements were represented by 7 deletions and 4 duplications identified in 10 samples. In addition to the “Primary Finding” of pathogenic CNVs, so-called “Incidental Finding” of pathogenetically significant rearrangements are also detected. The identification of “Incidental Finding” is characteristic of all genome-wide studies and leads to the emergence of ethical issues related to informing patients and the possible termination of pregnancy. In the present study, five “Incidental Finding” pathogenic CNVs and five rearrangements with uncertain significance were identified. The results of this work indicate the need to use modern molecular cytogenetic methods for the prenatal diagnosis of chromosomal abnormalities. It is necessary to take into account the emergence of new methodological and ethical issues that are most acute when using these methods in prenatal diagnosis.
