Цель -исследование воздействия метода хрономагнитотерапии на качество жизни пациентов с ЗОДА. Пациенты и методы. Больные основной группы (30 человек с остеоартрозом (ОА), 30 -с ревматоидным артритом (РА), 20 -с анкилозирующим спондилитом (АС), 6 -с подагрическим артритом (ПА), 6 -с псориатическим артритом (ПсА), 6 -с реактивным артритом (РеА)) получали по 10 ежедневных процедур хроно-магнитотерапии с помощью аппарата Мультимаг («Касимовский приборный завод», Рязань) в условиях ФГБУН «НИИ КиЭР». Больные группы сравнения (20 человек с ОА, 27 -с РА, 15 -с АС, 4 -с ПА, 4 -с ПсА, 4 -с РеА) получали подобные процедуры хрономагнитотерапии в условиях климатобальнеологического курорта Геленджик (СКК «Вулан», Краснодарский край). Результаты. В основной группе для пациентов с ОА характерно наиболее выраженное увеличение показателей ролевого физического и социального функционирования, интенсивности боли, с АС -физического функци-онирования и общего состояния здоровья, с РеА -ролевого эмоционального функционирования и жизненной активно-сти. Положительной оказалась динамика у больных ПсА группы сравнения по таким параметрам, как физическое, ролевое физическое и ролевое эмоциональное функционирование. В этой же группе у пациентов с ОА, АС и РеА отмечается улучшение показателей качества жизни, характеризующих как физический, так и психический компонент здоровья. За-ключение. Метод хрономагнитотерапии на этапе реабилитации пациентов с ЗОДА повышает уровень преимущественно физического здоровья, расстройства которого вызывают психосоматические нарушения. Хрономагнитотерапия в услови-ях климатобальнеологического курорта у пациентов с ЗОДА показала себя более эффективной, улучшая параметры как физического, так и психического здоровья. Субъективная оценка состояния здоровья самим больным может использо-ваться как маркер эффективности проводимой терапии и стать частью констатации ремиссии заболевания.Ключевые слова: хрономагнитотерапия, качество жизни, опросник ревматические болезни.
Introduction. The nutrition of the Nenets in the Arctic zone of Western Siberia is mainly based on traditional products (raw (thermally unprocessed) local fish and venison), which is the most important factor of survival in the Arctic. Storing up food products for an extended period is not relevant for the Nenets. Climate change and industrial development in the Arctic primarily affect the seasonality of consumption. Changing the calendar of fish and venison harvesting leads to greater use of imported and canned local food, increasing the length of the periods when local food completely disappears from the diet of the Nenets. Material and methods. During the expeditions to the national villages of the Yamal-Nenets Autonomous Okrug, cross-sectional screening studies were performed, in which 985 Nenets people took part. Nutritional analysis was conducted using the questionnaire method based on an album of portions and dishes. Retrospective information was collected on the seasonality of consumption of reindeer products and local fisheries (the length of the period when unserved fish and venison are available). Results. The consumption of reindeer and river fishery products is seasonal. The duration of the season of venison consumption was from 150±14.0 to 180±21.0, and raw fish - from 120±14.0 to 200±21.0 days. Despite the high level of consuming reindeer products per season, the average annual consumption of venison by Nenets in the Arctic zone of Western Siberia was 240 g/day/person that is less than in the central regions of the Russian Federation and industrially developed countries. In the studied area, the average annual consumption of fish (171.5 g/day/person) is three times higher than the average consumption in the Russian Federation. However, increased consumption of traditional food is typically only during the season of fishing or slaughtering reindeer. The average annual consumption reduced; during the off-season, the share of easily digestible carbohydrates in the diet increased. Conclusion. The creation of fish and venison stocks in villages and their year-round sale to the population is a necessary step in ensuring the food security of the Nenets people.
Проведена оценка отдаленных результатов применения метода низкочастотной магнитотерапии (НМТ) при проведении восстановительного лечения больных остеоартрозом (ОА). Обследовано 83 пациента с ОА (16 мужчин и 67 женщин в возрасте от 28 до 73 лет), проходящих курсовое (21 день) лечение в санаторно-курортных условиях. Все больные ОА были разделены на две группы, сопоставимые по половому составу, возрасту, длительности, активности заболевания и фактору фоновой терапии: основную (n=50) -с применением метода НМТ, и контрольную (n=33) -группа плацебо. Результаты изучения лечебного воздействия НМТ свидетельствуют о высокой эффективности данного метода и характеризуются достоверными изменениями индекса WОMАС (в основной группе уменьшение суммарного индекса WОMАС на 64,8%, в контрольной группе -на 28,6%), увеличением сроков ремиссии заболевания (ремиссия более 9 месяцев отмечалась у 18% пациентов из основной группы и у 6,1% из контрольной группы), снижением частоты рецидивов и частоты приема нестероидных противовоспалительных препаратов больными ОА. Применение НМТ на санаторно-курортном этапе соответствует условиям целенаправленной реабилитации больных ОА и позволяет улучшить отдаленные результаты терапии больных данного профиля. The results of the study of the therapeutic effects of LFMT demonstrate the high efficiency of this method and are characterized by significant changes of WOMAC index (in the primary group -decrease in total WOMAC index by 64.8%, in the control group -28.6%), the increase in terms of disease remission (remission more than 9 months was observed in 18% of patients in the primary group and 6.1% in the control group), a decrease in the frequency of relapses and frequency of intake of NSAID in OA patients. Application of LFMT on sanatorium stage corresponds to the conditions of targeted rehabilitation of patients with OA and improves long-term results of treatment of patients of this type. ASSESSMENT ОF LONG-TERM RESULTS OF LOW-FREQUENCY MAGNETIC
In modern sanatorium-resort organizations there is an urgent need of effective innovative physiotherapeutic methods usage along with basic resort treatment, that is especially important for patients’ resort treatment duration reduction. Extracorporeal shock wave therapy (ESWT) is evidence-based physiotherapeutic method. The aim of the study is to evaluate the high clinical efficacy of ESWT method in comparison with traditional methods of treatment at the sanatorium-resort complex «Vulan». Materials and methods: ESWT method effectiveness study was carried out in 111 people with shoulder joints and periarticular tissues pathology of various etiology aged 30-79 years (average age 45.5 years), disease duration from 1 year to 10-12 years (average 4.5 years). Results: According to the Swanson Score, in patients of the main group (n=57) «pain score» improved from 3.5 (before) to 8.3 (after), that is 2.3 times. “Activity in everyday life” raised from 4.7 to 8.6 (1.7 times), the volume of active movements increased from 3.2 to 7.9 (2.5 times). According to the Likert scale, patients reported a subjective improvement in their own well-being by 4.6 point. According to VAS scale a decrease in pain syndrome intensity was recorded (2.2 times in average). In patients of comparison group, who received basic resort treatment, positive changes were less significant. Conclusion: Objective and subjective data obtained in this study indicates a high clinical effect of ESWT in a complex health-resort treatment of patients with humeroscapular pain syndrome.
