Клинические рекомендации уже давно вошли в число рабочих инструментов современного врача, помогая ему быстро ориентироваться в наиболее эффективных доказанных методах лечения и профилактики различных заболеваний, а также адаптировать эти методы к конкретным задачам своих больных и добиваться максимальной персонализации лечения. Клинические рекомендации составляются профессиональными некоммерческими ассоциациями и одобряются научным советом МЗ РФ, при этом нередко одна рекомендация готовится двумя или даже тремя ассоциациями. Особенность предлагаемых вашему вниманию рекомендаций в том, что в профилактику и лечение ожирения вовлекаются не только эндокринологи, но и терапевты, кардиологи, гинекологи, гастроэнтерологи и врачи многих других специальностей. Мультидисциплинарная рабочая группа представляет этот проект в многопрофильном журнале с целью объединения усилий нескольких профессиональных ассоциаций, что связано с необходимостью уделить внимание не только самому ожирению, но и коморбидным состояниям. Мы надеемся на конструктивную критику и разностороннее обсуждение проблемы на страницах нашего журнала.
The effects of pharmaceutical excipients on the quality of medicinal products are discussed. It is emphasized that excipients must be studied before the development of a product starts, including compliance with production standards, investigations of their influences on the physical, pharmacological, and microbiological characteristics of products, and influences on the therapeutic efficacy and safety of medicinal formulations (release of active ingredient, bioavailability, etc.). Aspects considered in the present review indicate the need for a wide approach to assessment of the roles of excipients in determining the required pharmaceutical and therapeutic properties of medicinal products.
COVID-19 has had a significant impact on health care systems, including drug use. The present study aimed to evaluate the patterns of community supply of antimicrobials from community pharmacies during the COVID-19 pandemic in five cities of Russia. In a cross-sectional study, a random sample of pharmacies reported all episodes of antimicrobials supply during a one-week period. Patterns of supply (age and gender of customer, drug name and formulation, prescription availability, indication, etc.) were analyzed. Altogether, 71 pharmacies took part in the study and 5270 encounters were recorded. In total, 4.2% of visits resulted in supply of more than one antimicrobial agent and 5.2% were for parenteral formulations. The rate of prescription-based purchase in participated cities varied from 40.5 to 99.1%. Systemic antibiotics and antivirals accounted for the majority of supplies (60.5 and 26.3%, respectively). Upper respiratory tract infections were reported as the indication for antimicrobials usage in 36.9% of cases, followed by skin and soft tissue infections (12.1%) and urinary tract infections (8.7%); COVID-19 accounted for 8.4% of all supplies. Amoxicillin with clavulanic acid, azithromycin and amoxicillin were indicated as the top three antimicrobials purchased for upper respiratory tract infections, and azithromycin, umifenovir and levofloxacin were the top three for COVID-19. In general, a high rate of drugs dispensing without prescription was revealed. Antibiotics for systemic use remained the most common antimicrobials, whereas presumably viral upper respiratory tract infections were the main reason for their purchase. COVID-19 infection itself was responsible for a small proportion of the supply of antimicrobial agents, but systemic antibiotics accounted for more than a half of supplies.
Aim. To assess the effect of therapy with sodium glucose co-transporter type 2 inhibitor dapagliflozin in patients with heart failure with reduced ejection fraction (CHrEF) on the state cardiovascular mortality target indicators.Material and methods. All adult Russian patients with NYHA class II-IV HFrEF (left ventricular ejection fraction ≤40%) were considered as the target population. The characteristics of patients in the study corresponded to those in the Russian Hospital HF Registry (RUS-HFR). The study suggests that the use of dapagliflozin in addition to standard therapy will be expanded by 10% of the patient population annually in 2022-24. Cardiovascular mortality modeling was performed based on the extrapolation of DAPA-HF study result. The number of deaths that can be prevented was calculated when using dapagliflozin in addition to standard therapy. Further, the contribution of prevented deaths with dapagliflozin therapy to the achievement of federal and regional cardiovascular mortality target indicators (1, 2 and 3 years) was calculated.Results. The use of dapagliflozin in addition to standard therapy for patients with NYHA class II-IV CHrEF with the expansion of dapagliflozin therapy by 10% of the patient population annually will additionally prevent 1729 cardiovascular death in the first year. This will ensure the implementation of cardiovascular mortality target indicators in Russia in 2022 by 11,8%. In the second year, 3769 cardiovascular deaths will be prevented, which will ensure the implementation of target indicators in 2023 by 17,2%. In the third year, 5465 cardiovascular deaths prevented, which will ensure the implementation of implementation of target indicators in 2024 by 18,7%.Conclusion. The use of dapagliflozin in addition to standard therapy for patients with NYHA class II-IV CHrEF will ensure the implementation of implementation of target indicators in 2024 by 18,7%.
