Евдокимова», Москва, Россия Обоснование. Гипофизарный нанизм является орфанным заболеванием, при котором необходимо патогенетическое лечение. В отечественной литературе отсутствуют исследования, посвященные медико-экономической эффективности терапии дефицита гормона роста (ДГР) препаратами рекомбинантного гормона роста (рГР). Цель. Расчет эффективности затрат терапии рГР у детей с ДГР в Российской Федерации в рамках программы «7 высокозатратных нозологий». Методы. Изучены данные 50 детей с диагнозом ДГР, проживающих в 4 субъектах РФ и получавших лечение рГР в рамках программы «7 высокозатратных нозологий». Оценивали объемы потребления препарата и экономическую составляющую: стоимость лечения и мониторинга в течение ≥6 лет. Приростная стоимость рассчитана как разница между программой, при которой ребенок с ДГР обеспечен лечением рГР и мониторингом, и альтернативной программой, когда ребенок с подтвержденным ДГР получает финансовую, социальную помощь и медицинское обследование. Результаты. Медиана стоимости лечения одного ребенка рГР в дозе 0,033 мг/кг/сут за весь анализируемый период (в среднем 6,95 года) составила 437,5 тыс. руб., или 8,12 тыс. долл. США. Медиана стоимости лечения одного ребенка в год составила 63,6 тыс. руб/год, или 1,09 тыс. долл. США/год. С учетом стоимости лечения и мониторинга комплексное ведение одного пациента составило в среднем 68,4 тыс. руб. в год (470,7 тыс. руб. за 6,95 года); на долю мониторинга приходилось 7,05%. Стоимость всего комплекса выплат и обследований для одного ребенка-инвалида составила 178,97 тыс. руб. в год, или 1243,86 тыс. руб. за 6,95 года (доля медицинского обследования -1,2%). При расчете стоимостной разницы между программой, при которой ребенок с ДГР обеспечен лечением и мониторингом, и альтернативной программой, при которой ребенок с ДГР не обеспечен лечением, получает соответствующие выплаты по инвалидности и медицинское обследование, приростная стоимость составила 110,6 тыс. руб. на ребенка в год (773,18 тыс. руб. за 6,95 года). Заключение. Исследование показало, что лечение детей с ДГР в рамках программы «7 высокозатратных нозологий» является экономически эффективным как для пациента, так и для общества и государства в целом. Так, приростная стоимость между двумя программами за 6,95 года составила 110,6 тыс. руб. в год, или 773,18 тыс. руб. Результаты исследования свидетельствуют о важности всестороннего учета эффектов и затрат при оценке эффективности медицинских программ, особенно в случае орфанных заболеваний.Ключевые слова: гипофизарный нанизм, лечение, мониторинг, экономическая эффективность, дети.Rationale. Background. Growth hormone (GH) therapy for growth hormone deficient (GHD) children in Russia is funded by the state. This warrants the need for a full understanding of the disease and its treatment with recombinant GH (rGH) including its clinical, epidemiological and economical aspects. Aims — to estimate the incidence and the prevalence of GHD in children and adults in the Russian Federation.Material and methods. In Russia, GHD patients are registered in both the official medical statistics (MS) and a special registry for patients suffering of «high expenditure dieseases». Both the official MS data and the GHD patients’ registry is used for analysis.Results. According to MS the prevalence of GHD in Russia among children is 14,38 per 100 000. Including the adult population and adult patients with GHD, the prevalence is calculated at 4,94 per 100 000 total population. Calculations based on the registry show similar, yet not identical results: 14,57 per 100 000 children and 4,56 per 100 000 total population. The incidence of GHD among children results at 2,45 per 100 000 children/year according to MS, according to the registry — 2,06 per 100 000 children/year. Several regions of Russia demonstrate an extremely high or low prevalence/incidence of GHD among children. The prevalence of GHD among adults is much lower (statistics — 2,68 per 100 000 adults, registry — 2,16 per 100 000 adults). These circumstances warrant closer investigations and possible corrections of both diagnostics of GHD and patient monitoring.Conclusions. The differences between the MS and the registry revealed here show the necessity for correcting the rules of conducting the registry. Today there are no clear exclusion criteria. Considering the epidemiology of GHD shown by the official MS, the dependence of GH therapy outcome on a timely diagnosis we suggest to widen the functional use of the federal registry, which has the potential to become a useful tool in GH therapy control and patient monitoring.
Полногеномное и полноэкзомное секвенирование имеет общепризнанную клиническую значимость для диагностики орфанных заболеваний. Вследствие развития технологии секвенирования и последующего снижения затрат на тестирование, данные методы все чаще используются не только в клинических целях, но и в качестве инструмента скрининга здоровых индивидов. При проведении исследования по поводу подозрения на генетическое заболевание в семье и при тестировании здорового человека «по желанию» могут быть выявлены «случайные находки» с разной клинической значимостью. Однако в ряде профессиональных руководств появились рекомендации по систематическому поиску клинически значимых вторичных результатов - так называемому оппортунистическому скринингу. В соответствии с этим активно обсуждаются этические проблемы оппортунистического скрининга в контексте соразмерности пользы и рисков для тестируемого человека, информированного согласия и защиты его автономии и справедливости. Этические проблемы в связи с клиническим использованием анализа генома в Российской Федерации мало изучены. В данной обзорной статье описываются упомянутые проблемы и на основе международного опыта. Whole-genome and whole-exome sequencing have universally recognized clinical importance for the diagnosis of orphan diseases. Due to the development of sequencing technology and the subsequent reduction in diagnostic costs, these methods are increasingly used not only for clinical purposes, but also as a screening tool in healthy individuals. The study for suspected genetic disease in the family or testing a healthy person «optional» can be revealed «incidental findings» with different clinical significance. However, a number of professional guidelines recommend a systematic search for clinically significant secondary results - the so-called opportunistic screening. Accordingly, the ethical problems of opportunistic screening actively discussed, particularly in the context of the proportionality of the benefits and risks for the tested person, informed consent and the protection of autonomy and justice. Ethical problems of genetic counseling in modern economic and technical aspects in the Russian Federation have been little studied. This review article describes the problems mentioned, provides international experience and recommendations for solving the aforementioned problems.
