Anemia of chronic disease (ACD) is one of the most frequent forms of anemia is often observed in patients with infections, cancer and chronic inflammatory or autoimmune diseases. The underlying mechanisms are complex and include dysregulation of iron homeostasis and erythropoietin production, impaired proliferation of erythroid progenitor cells and reduced life span of red blood cells. Moreover, ACD is often superimposed by malnutrition, bleeding and renal failure. ACD is mediated through inflammatory cytokines and characterized by low serum iron (hypoferremia) and often increased reticuloendothelial stores of iron. ACD is usually normocytic, normochromic anemia, but it can become microcytic and hypochromic as the disease progresses. Hepcidin, the main regulator of iron homeostasis and its synthesis, is inhibited by iron deficiency and stimulated by inflammation. In many patients the disease is associated with several extrapulmonary manifestations regarded as the expression of the systemic inflammatory state of chronic obstructive pulmonary disease (COPD). Recent studies showed that anemia in patients with COPD is more frequent than expected, with its prevalence ranging from 8 to 33%. Systemic inflammation may be an important pathogenic factor, but anemia in COPD can also be the result of a number of factors, such as the treatment with certain drugs (angiotensin-converting enzyme inhibitors or theophylline), endocrine disorders, acute exacerbations and oxygen therapy. Anemia in COPD patients is strongly associated with increased functional dyspnea, decreased exercise capacity and is an independent predictor of mortality. Treatment options to correct anemia used in other chronic diseases, such as congestive heart failure, cancer or chronic kidney disease have not been explored in COPD (i.e. erythropoietic agents, iron supplements or combined therapy). It is not known whether treating the underlying inflammation could improve hematological characteristics. It is important to develop basic diagnostic modalities for this group of patients and formulate methods of anemia correction.
Asthma is a serious health problem affecting all age groups. Melatonin or its agonists are commonly used to treat many diseases, but there are conflicting data on asthma therapy. This paper analyzes researches on the possible use of melatonin in the therapy of asthma. Melatonin is a potent antioxidant and a vasodilator, but in some experiments, it can act as a pro-oxidant and a vasoconstrictor, which may depend on the duration of use. It has been suggested that circadian rhythms should be corrected in asthmatics to optimize the desired effects of drugs and to reduce the severity of their adverse reactions. Disordered diurnal variations in the salivary levels of melatonin and cortisol are detectable in patients with asthma and may be implicated in its pathogenesis. In addition, the conflicting data on the effect of melatonin on the development of asthma are associated with an incomplete view of the factors influencing the level of melatonin. Thus, to study the effects of melatonin, it is necessary to take into account the greatest possible factors that may influence the level of melatonin and the course of asthma: a daily diet in terms of the use of caffeine, alcohol, sleep-wakefulness pattern, sleep quality proper, and drowsiness during the daytime, social burden, the level of anxiety and stress resistance, and to investigate the levels of endogenous melatonin or its derivatives, immune status, oxidative stress intensity, etc. Obviously, the use of melatonin in the therapy of asthma can be considered, by applying a personalized approach.

Экзо генный мелатонин нормализует интенсивность окислительного стресса и уменьшает выраженность одышки при ХОБЛ, снижает уро вень 8 изопростана в 1,6 раза, ингибирует фосфорилирование киназы ERK и экспрессию Sp1. Мелатонин эффективно подавляет экспрессию MUC5AC, что может быть связано с ингибированием фосфорилирования сигнального пути MAPK. Мелатонин значитель но улучшает качество сна у больных ХОБЛ с сопутствующими нарушениями сна. При этом у пациентов не наблюдается дневной сон ливости и нарушения функции дыхания, в то время как при воздействии обычных снотворных средств может ухудшиться гипоксемия ночью, а в тяжелых случаях и привести к дыхательной недостаточности. Очевидно, мелатонин может представлять собой потенциаль ное лекарственное средство в терапии хронических заболеваний дыхательных путей, таких как БА и ХОБЛ. Background: Chronic obstructive pulmonary disease (COPD) may frequently be complicated by sleep disorders, which worse quality of life and lead to other adverse consequences. The objective of the study was to analyze clinical course of the disease and quality of life in elderly patients with both COPD and sleep disorders. Methods and Results:The study included 50 patients with moderate and severe COPD in age from 60 to 75 years (mean age, 66.12±0.76 years). Patients were divided into 2 groups randomly. For patients in Group 1, 3 mg of melatonin was prescribed orally 30-40 minutes before bedtime for 14 days on the background of standard COPD therapy. Patients of Group 2 received only standard COPD therapy. After 12 months of follow-up, the number of exacerbations and hospitalizations for COPD significantly decreased in Group 1: from 3.74±0.19 to 1.91±0.20 times a year (P=0.0000) and from 2.08±0.18 to 1.35±0.18 (P=0.0046). In Group 2, the dynamics of these parameters was not statistically significant. In addition, along with the improvement of sleep quality in COPD patients during treatment with melatonin, both state and trait anxiety scores and depression level improved. In Group 1, SF-36 scores (PF, RP, BP, and GH) have also significantly improved.Conclusion: Correction of sleep disorders by melatonin in elderly patients with COPD improved the effectiveness of COPD treatment, and reduced the frequency and duration of exacerbations and the number of outpatient visits and hospitalizations.
Development of apoptosis is accompanied by a decrease in transcripts of NADP-isocitrate dehydrogenase (NADP-IDH, E.C. 1.1.1.42.), and also alterations of catalytic properties from the rat liver enzyme in comparison with control. Administration of thioctic acid is increased the level of expression towards normal values. Enzyme preparations NADP-IDH were obtained from rat liver at norm conditions, after introduction of tumor necrosis factor and thioctic acid action under apoptosis. Molecular weight of homogenous preparations of NADP-IDH purified from livers of control and experimental rats was 112±5,8 kDa, however Km values and pH-optimum changed in apoptosis. Regulation of NADP-IDH activity under effect of some intermediates of the tricarboxylic acid cycle also differed in these groups.
