Incorporation of an IL-1i into a therapy regimen for secondary OA in RA patients during basic therapy could not only improve the functional status of patients, but also decrease activity of the underlying disease according to the DAS 28.

BackgroundAmong the complications in patients with RA often can be found secondary osteoarthritis, atherosclerosis, systemic osteoporosis1,2.ObjectivesThe aim of the study was to estimate the prevalence of secondary osteoarthritis of the knee in patients with rheumatoid arthritis and to reveal it association with the parameters of basic disease.Methods565 RA patient's histories (diagnosis according to the criteria of ACR/EULAR) from the rheumatology in-patient clinic with the mean age 48.4±10, 85.4% female and duration of the disease 3-15 years were reviewed. The features of secondary OA of the knee in patients with RA were assessed with the use of questioning (clinical symptoms, anamnesis of the disease), examination (ultrasonography (US) and X-ray of the joints).ResultsWe revealed secondary OA of the knee in 71% of patients with RA. Secondary OA was diagnosed predominantly in anti-CCP pos. patients (68,6%), with DAS 28>5,3 (in present and in the anamnesis) and with the duration of the disease 7-15 years. The more statistical significant associations were observed between DAS 28 and US, X-ray (corr. r=0,734, p<0,01), the duration of the disease and US and X-ray (r=0,671, p<0,05), US, X-ray and morning stiffness (r=0,526, p<0,05).ConclusionsThese findings demonstrate the high prevalence of secondary osteoarthritis of the knee in patients with rheumatoid arthritis. This comorbid medical condition associates with the increase of activity, morning stiffness and duration of underlying disease (RA).ReferencesPeat G, et al. This article focuses on the distant blood pressure monitoring for patients with arterial hypertension. As numerous studies show, even slightly elevated blood pressure significantly raises the risk of cardiovascular complications. And, vice versa, a 5 mmHg decrease in blood pressure reduces the lethality risk. Therefore, it is not enough to prescribe the right medication but also it is of paramount importance to monitor patients’ compliance with the treatment. Clinical observation of patients with arterial hypertension is an effective tool for the prevention of cardiovascular complications. However, to date, the coverage of follow-up and the achievement of blood pressure targets in patients with arterial hypertension is one of the most problematic aspects. Distance monitoring of blood pressure opens more opportunities for the doctor’s involvement, timely assessment and adjustment of the medication. The results of domestic and foreign research show high efficacy of the distance blood pressure monitoring. Positive results regarding the achievement of target blood pressure after 3 months are shown when using the technology of blood pressure monitoring and distance counseling of patients with arterial hypertension. In particular, the article considers the technology of mobile health care (mHealth), which is a more flexible platform for a patient’s continuous self-care.

The aim of the study is to evaluate the efficacy of complex therapy of secondary osteoarthritis in patients with rheumatoid arthritis with the use of inhibitor of interleukin-1, including the dynamics of inflammatory markers.Materials and methods: 248 patients with secondary osteoarthritis in rheumatoid arthritis were involved in the trial. The participants were divided into 4 groups: patients of group I took inhibitor of interleukin-1 (diacerein) in combination with laser therapy and methotrexate; inhibitor of interleukin-1 in complex with methotrexate took patients from group II; group 3 — laser therapy + methotrexate and patients of group IV took only methotrexate. The efficacy of therapy we estimated in 6 months.Results: The constructed model surfaces indicated the decreased levels of IL-1, COMP and DAS 28 in group 1 till 6,68±0,37 pg/ml (p<0,001), 16,92±0,8 ng/ml х 10² (p<0,001) и 2,06±1,19 (p<0,05) accordingly in comparison with groups III and IV. Also the model surfaces revealed the interdependency of all these indicators. The control group (IV) reacted to the treatment by decreasing the indicators as well. However, the dynamics of the changes was significantly less. In patients of groups I and II the levels of ESR and CRP decreased to 13,95±0,52* (*p<0,001) and 10,97±0,43* (*p<0,001); 16,53±0,63* (*p<0,001) and 12,81±0,77* (*p<0,001) accordingly.Conclusions: In comparison analysis we noted statistical significant advantages (p<0,01) of the use of diacerein with methotrexate regarding the dynamic of IL-1 and COMP, ESR, CRP in patient’s serum, which is accompanied by the reduction of basic disease activity on DAS 28.
