В статье представлены данные о распространенности и особенностях течения инфекционных заболеваний, связанных с вирусом Эпштейна – Барр (ВЭБ). ВЭБ – ДНК-содержащий вирус семейства герпесвирусов, серологические маркеры которого выявляют почти у 90% взрослых людей во всем мире. Вирус легко распространяется через глубокие поцелуи, что является основной причиной первичной ВЭБ-инфекции у подростков – инфекционного мононуклеоза. Инфекционный мононуклеоз (ИМ, моно) также известен как железистая лихорадка. У пациентов часто развивается недомогание, головная боль и лихорадка, затем присоединяются тонзиллит и фарингит. ИМ продолжается несколько недель, а далее самокупируется, однако в среднем у 1% пациентов могут развиваться тяжелые осложнения: обструкция дыхательных путей из-за воспаления ротоглотки, менингоэнцефалит, стрептококковый фарингит, гемолитическая анемия и тромбоцитопения. The article presents data on the prevalence and course of infectious diseases associated with the Epstein – Barr virus (EBV). EBV is a DNA-containing virus of the herpesvirus family whose serological markers are detected in almost 90% of adults worldwide. The virus spreads easily through deep kissing, which is the main cause of primary EBV infection in adolescents - infectious mononucleosis. Infectious mononucleosis (IM, mono) is also known as glandular fever. Patients often develop malaise, headache, and fever, followed by tonsillitis and pharyngitis. IM lasts for several weeks and then resolves itself; however, in an average of 1% of patients, severe complications may occur: airway obstruction due to inflammation of the oropharynx, meningoencephalitis, streptococcal pharyngitis, hemolytic anemia, and thrombocytopenia. Other less common complications of EBV include oral hairy leukoplakia, airway obstruction due to lymphoid hyperplasia, lymphoproliferative disorders, Burkitt’s lymphoma, tumors in HIV-infected patients, Hodgkin’s lymphoma, T-cell lymphoma, and carcinoma of the head and neck. Primary EBV infection can in some cases lead to chronic multisystem disease in patients who do not have a specific genetic cause for poor virus control.The determination of specific serological markers and other indicators of EBV infection is important for the earliest diagnosis of precancerous process, since many EBV-associated neoplasms are not clinically evident at the early stages of the process.
Прогрессирующий семейный внутрипеченочный холестаз (ПСВХ) – это редкая форма наследственной патологии печени, которая обусловлена дефектами секреции желчи, непрерывно прогрессирует и способствует формированию фиброза и цирроза печени. Частота данной патологии составляет 1 на 50 000–100 000 новорожденных. Начинается заболевание, как правило, в младенчестве, но прогрессирование процесса может быть и в подростковом возрасте. Выделяют три типа ПСВХ: при ПСВХ 1-го и 2-го типа нарушается секреция солей желчных кислот, а при ПСВХ 3-го типа изменена экскреция фосфолипидов. Диагностика основана на клинических проявлениях, результатах холангиографии, УЗИ, гистологии печени и специфических тестах. Терапия урсодезоксихолевой кислотой должна быть начата во всех случаях для предотвращения прогрессирования повреждения печени. Тем не менее, большинство пациентов с ПСВХ в конечном итоге являются кандидатами на трансплантацию печени. Таким образом, ранняя диагностика ПСВХ позволяет своевременно определять тактику ведения пациентов с целью предотвращения необратимого исхода заболевания. Progressive familial intrahepatic cholestasis (PFIC) is a rare form of hereditary diseases of the liver, which (due to defects in bile secretion) constantly progresses and promotes the formation of liver fibrosis and cirrhosis. Its incidence is estimated as 1 to 50.000–100.000 newborns. The onset of the disease is generally in infancy, but the diagnosis may be delayed until adolescence. Three types of PFIC are distinguished: in PFIC types I, II, the secretion of bile salts deteriorates, whereas in PFIC type III, the excretion of phospholipids is altered. Diagnosis is based on clinical manifestations,cholangiography, liver ultrasonography, and histology, as well as on specific tests. Ursodeoxycholic acid therapy should be initiated in all patients to prevent the progression of liver damage. However, the majority of PFIC patients are ultimately candidates for liver transplantation. Thus, early diagnosis of PFIC allows timely determination of patient management tactics in order to prevent an irreversible outcome of the disease.
