ÂâåäåíèåС проблемой длительного кашля у детей, или рецидивирующего бронхообструктивного синдрома, при котором традиционная терапия малоэффективна, педиатры в своей практике сталкиваются достаточно часто. По статистике, примерно в 30 % случаев причиной этому является микроаспирационный синдром. Аспирация ведет к химическому повреждению слизистой оболочки бронхов, развитию бронхоспазма, размножению бактериальной флоры (нередко кишечной), развитию бронхита [1,2]. Аспирационный (ингаляционный) бронхит -сборное понятие, которое объединяет Êë³í³÷íà ïåä³àòð³ÿ / Clinical Pediatrics ® различные формы патологии бронхов, возникшие на почве аспирации жидкостей (при аспирации твердых предметов принято говорить об инородных телах трахеи и бронхов). У детей чаще всего наблюдается систематическая (привычная) аспирация пищи, приводящая к развитию затяжного или рецидивирующего бронхита, нередко осложняющегося пневмонией. Случайная острая аспирация небольшого количества жидкости обычно ведет к развитию приступа кашля, удаляющего аспирированный материал [3,4].Причины аспирации у детей многообразны. В статье приведены данные по исследованию распространенности синдрома микроаспирации среди детей раннего возраста, которые находились в городском детском пульмонологическом отделении по поводу затяжного течения обструктивного бронхита. Была изучена структура причин микроаспирации у детей раннего возраста (2010-2015 гг.). Выделены наиболее значимые причины возникновения микроаспирационного синдрома в зависимости от возраста детей. Для более эффективного сбора анамнеза использовали модифицированный опросник для родителей. В группе риска по наличию микроаспирационного синдрома в первую очередь находятся дети с перинатальным поражением центральной нервной системы, рожденные раньше срока, с морфологическими и физиологическими дефектами носоглотки, с мышечной дистонией. Проявлением микроаспирации могут быть частые срыгивания, рвота во время кашля, вытекание пищи через нос, связь приступов кашля с кормлением, усиление или появление кашля в положении лежа, приступы ночного кашля или удушья, длительное сохранение покашливания после приступа. Педиатрам рекомендуется внимательно наблюдать за детьми во время каждого кормления, так как дисфагия может быть непостоянной. Микроаспирационный синдром является сложным в диагностике и требует комплексного подхода, чаще всего диагностические манипуляции носят инвазивный характер. Своевременная диагностика и лечение микроаспирационного синдрома позволят снизить уровень респираторной заболеваемости у детей раннего возраста и уменьшить продолжительность течения обструктивных бронхитов. Цель исследования-изучение особенностей синдрома одышки у больных гипертонической болезнью. Материал и методы. Было обследовано 80 больных гипертонической болезнью с синдромом одышки в возрасте от 35 до 62 лет, из них с � стадией гипертонической болезни-35 человек� со �� стадией-�5. �собен� � стадией гипертонической болезни-35 человек� со �� стадией-�5. �собен� стадией гипертонической болезни-35 человек� со �� стадией-�5. �собен� �� стадией-�5. �собен� стадией-�5. �собенность группы состояла в том, что помимо жалоб, типичных для ГБ, больные отмечали одышку, выраженность которой не соответствовала степени функциональных изменений сердечно�сосудистой системы. Проводилось исследование функционального состояния сердечно�сосудистой и дыхательной систем, включающее ЭКГ, УЗИ сердца, спирографию, оценку газового и кислотно�основного состояния крови, капнографию. Капнографическое исследование проводилось с помощью газоанализатора «Нормокап�200�OXY» фирмы «Датекс» (Финляндия). Психофизиологическая оценка степени одышки осуществлялась с применением шкалы Борга и визуальной аналоговой шкалы. Для выявления гипервентиляционных состояний и определения степени их тяжести применялся психологический опросник департамента пульмонологии Наймигенского университета методом анкетирования с оценкой симптомов по ��балльной шкале. Результаты и их обсуждение. Выявлено, что у �1 больного гипертонической болезнью (26 пациентов с � стадией, 15-со �� стадией) с гипокапническим типом вентиляции отмечались жалобы на одышку, усиливающуюся при физической нагрузке, характеризуемую больными как «чувство нехватки воздуха», «дыхательный дискомфорт», «стеснение в грудной клетке», гаспы, сухой кашель. Гиперкапнический тип дыхательных нарушений у больных гипертонической болезнью был диагностирован у 12 больных. У 7 больных гипертонической болезнью �� стадии отмечалась лабильность типов вентиляции в раз� �� стадии отмечалась лабильность типов вентиляции в раз� стадии отмечалась лабильность типов вентиляции в разные моменты исследования, а у � больных гипертонической болезнью � стадии и у 16 больных �� стадии выявлен нормокапнический тип вентиляции с разнообразными дыхательными нарушениями. Заключение. У половины обследованных больных причиной синдрома одышки явилось наличие гипервентиляционного синдрома.
