Цель. Оценка диагностической значимости данных однофотонной эмиссионной компьютерной томографии с одномо-ментным проведением рентгенологической компьютерной томографии (ОФЭКТ-КТ) после терапии радиоактивным йодом в сравнении с данными ультразвукового исследования (УЗИ) мягких тканей шеи и компьютерной томографии (КТ) у пациентов с дифференцированным раком щитовидной железы (ДРЩЖ), которым первоначально было проведено хирургическое лечение, и роли гибридного исследования в рестадировании ДРЩЖ. Материал и методы. Проанализированы результаты исследований 263 пациентов, обследованных с ноября 2011 г. по апрель 2014 г., в возрасте от 10 до 78 лет, 190 женщин и 73 мужчины. По результатам послеоперационного гистологиче-ского исследования у 225 человек был выявлен папиллярный рак щитовидной железы (ЩЖ), у 38 -фолликулярный рак ЩЖ. Всем пациентам были проведены: УЗИ мягких тканей шеи, компьютерная томография органов грудной клетки без внутривенного контрастирования, сцинтиграфия мягких тканей шеи с 99m Tc-пертехнетатом, после чего выполнена тера-пия радиоактивным йодом активностью от 3,0 до 5,5 ГБк. В среднем на 3-5-е сутки после радиойодтерапии пациентам выполнялись планарная сцинтиграфия всего тела и ОФЭКТ-КТ. Результаты. После радиойодтерапии и проведения ОФЭКТ-КТ у 36,7% пациентов изменена категория N, у 11% -кате-гория M, поставленные ранее по результатам УЗИ, КТ и сцинтиграфии. Частота регионарных шейных метастазов, не выявляемых эхографически и заподозренных по данным сцинтиграфии всего тела, составила 43,9%. В большинстве случаев имело место микрометастазирование, а также поражение лимфатических узлов в пре-и паратрахеальных обла-стях, плохо доступных при УЗИ. Заключение. Данные ОФЭКТ-КТ после терапии радиоактивным йодом позволяют более точно определить стадию забо-левания у пациентов с ДРЩЖ. The purpose of the study was to assess the diagnostic value of SPECT/CT data after radioiodine therapy in comparison with the data of soft tissue neck ultrasonography (US) and computed tomography (CT) in patients with differentiated thyroid carcinoma (DTC) and thus to investigate the role of SPECT/CT data in the restaging of DTC. Material and methods. We analyzed the results of 263 patients who were examined from November 2011 to April 2014, at the age of 10 to 78 years (190 female and 73 male). According to postoperative histological tests, 255 patients had papillary thyroid cancer and 38 patients had follicular thyroid cancer. Soft tissue neck US, CT of the chest without intravenous contrast and scintigraphy with 99mTc-pertechnetate in all patients were performed and then they were treated with radioiodine (dose, 3.0-5.5 GBq).
