Microcirculatory hemodynamic indexes (HI) were assessed in patients with moderate and severe COVID-19. In both groups, a significant increase in the absolute spectral indexes (HI1, HI2, and HI3) and the ratio of low-frequency to high-frequency component (HI1/HI3) was revealed. In the group of severe infection, only the “slow” index (low-frequency HI1) of microcirculatory hemodynamics was significantly lower. The oscillatory indices MAYER1-3 and RESP1-3 were reduced in patients of both groups. The aggravation of the disease course was accompanied by depression of the low-frequency index HI1. Regulatory shifts compensate for disturbances in microcirculatory processes in moderate COVID-19, but severe course was associated with their decompensation.
Introduction. Despite the advances in modern anesthesiology, it is impossible to guarantee a safe course of anesthesia, and even with planned surgical interventions, there is a risk of death. At present, there is no unanimity in approaches to assessing perioperative risk, and many systems for determining this risk have not been validated in Russia. The question of the contribution of pre-operative factors to the likelihood of an unfavorable outcome also remains open, which requires large multicenter national studies. Objectives. To assessment the predictive value of preoperative factors in determining the risk of death and complications based on the analysis of data obtained during the first year of the STOPRISK study. Materials and methods. An analysis of data on perioperative indices of 3002 patients operated on the abdominal and pelvic organs from 30 centers in 21 cities of Russia participating in the STOPRISK study is presented. Results. The mortality rate in the study was 0.47 %, the rate of postoperative complications was 3.9 %. Most often, an unfavorable outcome developed after upper abdominal and colorectal surgery. Despite the fact that the severity of surgery and the ASA class are independent predictors of an unfavorable outcome, the use of these parameters allows to predict postoperative mortality (AUROC = 0.85) and (with age) postoperative complications (AUROC = 0.77) with limited accuracy. Conclusions. Thus, the probability of an unfavorable outcome can be estimated using factors such as the severity of surgery and the initial physical status, but their predictive value for determining the risk of mortality is clearly insufficient, and even less is their ability to assess the risk of postoperative complications. As shown by literature data, inclusion in model additional risk factors allows to increase the accuracy of the forecast, however, given the peculiarities of the structure of comorbidities and their impact on outcome in the studied population, we need further evaluation of their contribution to perioperative risk. Also, taking into account the peculiarities of the occurrence of some concomitant diseases, further research is required to identify a significant impact on mortality and postoperative complications.
Тяжелое течение COVID-19, сопровождаемое острой дыхательной недостаточностью, острым респираторным дистресс-синдром и полиорганной недостаточностью, наиболее часто наблюдается у пожилых (старше 65 лет), а также у сравнительно молодых пациентов с сопутствующими заболеваниями -сахарным диабетом, гипертонической болезнью, нарушениями сердечной, почечной или печеночной деятельности. При этом у больных возникает чрезмерная воспалительная реакция, которая сопровождается развитием «цитокинового шторма», разбалансированием и последующим истощением Т-клеточного иммунитета. Успешная терапия COVID-19 строится на восстановлении функций поврежденной иммунной системы, в том числе ликвидации «цитокинового шторма». С этой целью рекомендовано применение блокатора рецептора моноклонального антитела к IL6 (RIL6) тоцилизумаба (TCZ). Приведены сведения об успешном применении TCZ у больных тяжелой формой COVID-19, а также указаны его существенные недостатки -развитие неблагоприятных реакций -лимфоцитопении, тромбоцитопении, увеличения уровня IL6, аланин-и аспартатаминотрансферазы и др. Одновременно приведены обоснования для применения у таких больных иммуномодулятора тималина, способного ликвидировать «цитокиновый шторм», а также нормализовать состояние иммунной системы и предотвращать синдром диссеминированного внутрисосудистого свертывания. Приводятся типичные случаи лечения больных с тяжелыми формами COVID-19 при использовании на фоне стандартной терапии TCZ и тималина раздельно и совместно.
ВведениеОстрая общая холодовая травма (ООХТ) часто встречается в регионах Сибири и на Крайнем Севере, где поражение организма холодом является краевой патологией. Ежегодно регистрируется значительный уровень летальности в результате действия холода [13]. Вместе с тем патогенез и танатогенез холодовой травмы требуют уточнения.Течение ООХТ во многом зависит от функциональ ного состояния центров вегетативной иннервации, раз дражение которых может происходить под влиянием терморецепторов кожи и сосудов, а также от прямого воздействия на них охлажденной кровью [13,14]. В свою очередь, холодовой фактор приводит к измене нию вегетативного статуса, что проявляется вегетатив ным дисбалансом и, как следствие, возникновением жизнеугрожающих нарушений [3,4,10,11].В настоящее время в клинической медицине для анализа активности нейрогенной регуляции сосудов, обусловленной симпатическим тонусом, широко ис пользуется неинвазивный метод лазерной допплеров ской флоуметрии (ЛДФ) [1,5]. Применяя амплитуд ночастотный анализ колебаний кровотока, оценивая при этом тонус сосудов в целом, можно изолированно оценить влияние нейрогенного компонента тонуса вегетативной системы, что предоставляет в этом от ношении значимые диагностические возможности [1,9]. Aim. To evaluate the activity of the 4-1BB/4-1BBL signaling pathway in patients with influenza A (H1N1) virus-associated pneumonia.Materials and Methods. Here we enrolled 85 patients (41 males and 44 females, median age 48 (36-62) years) with influenza A (H1N1) virus-associated pneumonia. Among the exclusion criteria were unstable hemodynamics, BMI > 30, diabetes mellitus, HIV, tuberculosis, and cancer. Control group consisted of 15 healthy donors. The diagnosis of influenza A / H1N1 was confirmed by a positive PCR test. Pneumonia was diagnosed according to the Federal Clinical Guidelines «Community-acquired pneumonia in adults». Severity of pneumonia was evaluated by using CURB-65 and SMART-COP scales as well as IDSA/ATS criteria. Plasma concentration of 4-1BB (CD137 or TNFRSF9, an inducible costimulatory receptor expressed on activated T cells and antigen-presenting cells) was determined by flow cytometry.Results. Patients with moderate and severe influenza A (H1N1) virus-associated pneumonia had 1.5- and 2.4 fold-increased concentration of plasma 4-1ВВ as compared with the healthy controls.Conclusion. The 4-1BB/4-1BBL signaling pathway, involved in multiple immune reactions, is associated with the severity of influenza A (H1N1) virus-associated pneumonia.
Background. The spread of the new coronavirus infection COVID-19 has already become one of the main problems of national healthcare systems around the world. Until now, it has not been possible to find drugs with sufficient etiotropic activity for COVID-19, and therefore, it is important to determine new points of application for pathogenetic therapy in relation to this pathology. Aim. To identify the predictors of an unfavorable outcome of a severe course of COVID-19 infection to determine the prognosis of the clinical course and optimize treatment tactics using succinates. Material and methods. A retrospective observational study of 46 cases of treatment with a severe form of the disease on the basis of a monohospital for the treatment of patients with a new coronavirus infection was conducted. All patients had comobrid pathology (median Charlson index 3 points). The most common ones were: encephalopathy of mixed genesis, diabetes mellitus, coronary heart disease, arterial hypertension, alimentary-constitutional obesity. We assessed the relationship between indicators of initial status and mortality in patients, and indicators with a statistically significant relationship were selected as predictors. Statistical processing of the results was carried out in the IBM SPSS v. 23.0, ROC analysis was used to find the relationship between quantitative predictors and lethal outcome. Results. Among the treated parameters, the most significant influence on the risk of death was found in arterial anion gap (odds ratio 28.78; p 0.017) and fibrinogen level (odds ratio 22.20; p 0.01). To a lesser extent, the level of urea, aspartate aminotransferase, and the Charlson comorbidity index had an effect on the prognosis of a lethal outcome. The identified predictors of an unfavorable outcome of a severe course of COVID-19 infection can be used to predict the clinical course and build treatment tactics based on the received information. Conclusion. Predictors of poor outcomes in severe COVID-19 infections include arterial anion gap and fibrinogen levels, and to a lesser extent, urea levels, aspartate aminotransferase levels, and the Charlson comorbidity index.
