The article discusses the role of telmisartan in the treatment of arterial hypertension in patients with metabolic syndrome. Telmisartan is second-generation type 1 angiotensin II receptor blocker, which has unique pleiotropic effects due to partial affinity for receptors that activate the proliferation of subtype γ peroxisomes (PPARγ) located in adipose tissue. The interrelation of metaflamation, a specific chronic inflammatory process with pathogenetic mechanisms of development of cardiovascular diseases, including arterial hypertension, is also described in study. The role of the adiponectin peptide is considered, which synthesis is stimulated by partial PPARγ receptor agonists (as mentioned above -telmisartan). It has a positive effect on fat and carbohydrate metabolism, as well as cardioprotective properties. The conclusion contains the results of numerous randomized studies and meta-analyzes confirming the high efficacy of telmisartan in the treatment of arterial hypertension in patients with morbid obesity.
Aim. To evaluate the effectiveness of telmisartan as a component of triple antihypertensive therapy in patients with obesity and refractory arterial hypertension.Material and methods. The study included 30 patients with obesity and refractory arterial hypertension. All patients received an angiotensin II receptor blocker (ARB) or an angiotensin-converting enzyme inhibitor (ACE inhibitor) in an adequate dosage as part of a triple antihypertensive therapy that also includes a calcium antagonist and thiazide diuretic. Participants were randomly divided into two equal groups. In the main group, telmisartan (Dr. Reddy’s Laboratories) was prescribed at a dosage of 80 mg/day instead of the previously taken ARB or ACE inhibitor. In the control group, patients continued to receive a previously prescribed ARB or an ACE inhibitor. The primary end point was a decrease in mean daily systolic and/or diastolic blood pressure (SBP and DBP) according to 24-hour blood pressure monitoring at 10 mm Hg and more than 20 weeks after the start of the study. The secondary end point was a decrease in the concentration of inflammatory markers and an increase in the level of adiponectin by 15%.Results. After 20 weeks, in the telmisartan group, we noted a significant decrease in the average daily SBP and DBP: in the main group from 145,9±5,4/95,6±4,8 mm Hg to 134,8±3,0/84,9±4,2 mm Hg, in the control group with 147,2±4,9/96,4±5,6 mm Hg to 142,4±4,3/96,9±62 mm Hg. We also determined an increase in the serum adiponectin concentration from 9,3±5,6 μg/ml to 13,4±6,6 μg/ml in the main group and a decrease from 8,8±5,2 μg/ml to 8,6±5,6 μg/ml in the control group, as well as a decrease in the concentration of highly sensitive C-reactive protein (main group from 8,8±3,0 to 6,0±2,8 mg/l and the control group from 7,4±3,8 to 6,9±4,9 mg/l) and interleukin-6 (main group from 16,2±6,1 to 12,3±2,8 pg/ml, control group from 22,5±4,2 to 19,9±5,2 pg/ml).Conclusion. Telmisartan can be considered as a drug of choice as part of multi-component therapy in the treatment of patients with obesity and refractor arterial hypertension.
ВведениеСогласно современным представлениям под рези-стентной артериальной гипертензией (АГ) понимают клиническую ситуацию, в которой при одновременном назначении трех и более антигипертензивных ле-карственных препаратов различных классов (один из которых -диуретик) не удается достичь целевого ар-териального давления (АД) (<140/90 мм рт. ст.). При этом важно, чтобы все лекарственные препараты были назначены в оптимальных терапевтических дозах [1][2][3]. Обследование и ведение пациентов с резистентной АГ включает в себя не только исключение симптома-тических АГ, но и ряд других этапов: исключение псев-дорезистентной АГ, оптимизация лекарственной тера-пии и образа жизни и др. Только после этого форму-лируются показания к новому немедикаментозному ме-тоду лечения -ренальной денервации. Возможности терапии резистентной артериальной гипертензииНа сегодняшний день в арсенале кардиолога на-считывается около 40 антигипертензивных препаратов, однако и при использовании рациональных комбина-ций далеко не всегда удается достичь целевого АД.Даже на фоне рациональной фармакотерапии це-левого АД удается достичь не более чем у 35% боль-ных. Точная распространенность резистентной АГ не-известна. По данным крупных центров её частота мо-жет достигать 10-13% [4,5], и этот показатель остается высоким даже после исключения псевдорезистент-ной АГ, связанной с гипертонией «белого халата», ошибками при изменении АД и т.д.Многие годы единственным способом нормализо-вать АД у больных с истинной резистентной АГ было на-значение полифармакотерапии с одновременным ис-пользованием 4-6 и более препаратов. Однако такая тактика лечения нередко приводила к развитию серь-езных побочных эффектов.Как известно, симпатическая нервная система (СНС) является одним из основных прессорных механиз-мов, а ее активация -ключевым фактором развития эс-сенциальной АГ. Гиперактивация СНС и прогресси-рующее повышение АД взаимно усиливают друг дру-га и участвуют в формировании порочного круга. По-вышенная активность СНС больше всего затрагивает поч-ки. При гиперактивации СНС в почке происходит ва-зоконстрикция, снижаются кровоток и фильтрация, усиливается секреция ренина, увеличивается реаб-сорбция натрия и воды. Афферентная импульсация от почек поступает сначала в боковые рога спинного мозга, а затем в сосудистый центр в центральной нервной системе, способствуя повышению АД. ВведениеВ России и зарубежных странах артериальная ги-пертензия (АГ) считается одним из самых распростра-ненных факторов риска сердечно-сосудистых заболе-ваний. Согласно данным Всемирной организации здравоохранения причиной 7,5 миллионов (12,8%) случаев смерти во всем мире является АГ [1]. По не-которым прогнозам распространенность АГ к 2025 г. бу-дет достигать 29% [2]. Данные федерального монито-ринга за 2003-2010 гг. свидетельствуют, что у 39,7% взрослого населения Российской Федерации уровень артериального давления (АД) выше целевых значений, при этом знают о наличии заболевания 81,1% больных [3]. К сожалению, даже адекватная, тщательно подо-бранная медикаментозная терапия не всегда позволяет добиться нормализации АД и избежать осложнений, что влечет за собой потерю трудоспособности, инва-лидизацию, а в масштабах страны -большие эконо-мические и социальные потери [4]. По результатам ис-следования ЭССЕ-РФ распространенность АГ в РФ за 2012-2013 гг. возросла до 44% [5].Резистентной артериальной гипертензией (РАГ) на-зывают состояние, при котором адекватная комбини-рованная терапия тремя антигипертензивными пре-паратами, один из которых диуретик, в сочетании с из-менением образа жизни и отказом от вредных привычек не приводит к достижению целевого АД (<140/90 мм рт. ст.). Важно отметить, что все препараты должны на-значаться в оптимальных терапевтических дозах [6][7][8].На сегодняшний день РАГ можно назвать наиболее изучаемой проблемой современной «гипертензиоло-гии». Изучение эпидемиологии затруднено прежде всего тем, что не всем пациентам исходно назначают адекватную антигипертензивную терапию [9]. По дан-ным отечественных исследований целевых значений АД на фоне лечения достигают не более 15% больных [10]. Кроме того, больных РАГ редко включают в крупные мно-гоцентровые исследования, направленные на изучение моно-или двукомпонентной терапии. В итоге -истинная распространенность резистентности неизвестна. То же относится и к статистике, касающейся основных факторов риска РАГ [11].Согласно результатам исследования NHANES (The National Health and Nutrition Examination Survey), посвя-щенного оценке питания и состояния здоровья населения США, в 2007-2008 Несмотря на возможности современной фармакотерапии, более 80% пациентов с артериальной гипертензией не достигают целевого артериального давления. По-мимо очевидных причин -низкой приверженности пациентов лечению и применение нерациональных схем терапии, существуют и другие объективные факторы рис-ка резистентности. Рассмотрены три основные модифицируемые причины резистентной АГ, значение которых обычно недооценивается при работе с пациентами с труд-но контролируемой гипертензией: ожирение, синдром обструктивного апноэ сна и первичный гиперальдостеронизм. Понимание этих механизмов резистентности и критериев их диагностики позволит улучшить результаты лечения пациентов с резистентной артериальной гипертензией. Aim. To evaluate the course of resistant arterial hypertension in patients with visceral obesity, to identify predictors of unsatisfactory shot-term and long-term treatment outcomes, to optimize therapy and improve adherence to treatment. Materials and methods. A total number of 90 individuals with a history of refractory or resistant arterial hypertension and visceral obesity were a subject of intensive study. The prospective analysis group consisted of 30 patients with an individualized management plan each, whereas the retrospective group of real clinical practice included 60 participants. At baseline, all patients were taking antihypertensives like ACE inhibitors or angiotensin II receptor blockers (ARBs)/angiotensin-converting enzyme inhibitors, calcium channel blockers (CCBs), and a diuretic. After the initial examination, therapy was individually optimized for each patient in accordance with current clinical guidelines. Most patients in the retrospective group received ARBs valsartan or losartan, CCBs amlodipine, the diuretics indapamide and torasemide, the -blockers bisoprolol and metoprolol, the 2-agonist moxonidine, and the mineralocorticoid receptor antagonist spironolactone. Patients in the prospective group were prescribed ARBs telmisartan and azilsartan, the CCB lercanidipine, thiazide and thiazide-like diuretics indapamide and chlorthalidone, the -blockers nebivolol and carvedilol, the 1-blocker doxazosin, and spironolactone. A re-examination was performed 2 months later. Subsequently, regular communication was maintained with participants of the prospective group during 8 months using a messenger. Communication with patients of the retrospective group was not maintained. All the patients were then asked to self-report their health status by conducting a telephone survey. Results. After 2 months, according to the data of the follow-up, in the retrospective group the target values of mean daily SBP and DBP were observed in 35 and 36.7% of patients, though the statistics among the patients in the prospective group were 66.7 and 60%, respectively. After 10 months, according to the results of the interviews, the target values of SBP and DBP were observed in 10 and 18.3% of patients, though the statistics among the patients in the prospective group were 93.3 and 96.7%, respectively. In the retrospective group, 78.3% of patients changed the previously selected therapy, in the prospective group this figure was only 20%. In the retrospective group, anthropometric data did not change, while in the prospective group, weight and waist circumference significantly decreased (p0.05). Conclusion. Maintaining regular contact with patients and a well-rounded treatment strategy with individualized choice and dosage of medications with an emphasis on modern metabolically neutral drugs with a prolonged duration of action led to better BP control, increased adherence to therapy and indicated significant weight loss among the patients from the prospective group.
