Aortic esophageal fistula (AEF) is a rare but extremely life-threatening condition requiring immediate surgical treatment. The mortality rate among such patients may exceed 60 %, including after surgical treatment. Etiological and pathogenetic mechanisms of AEF are complex and various, but in most cases, they are associated with chronic aortic diseases. The second group of etiological factors of AEF includes trauma of esophagus wall by foreign bodies, malignant neoplasms of the esophagus or mediastinum. AEF may also occur as a result of surgical interventions on the aorta and esophagus. The difficulties of early diagnosis are primarily associated with non-specific clinical manifestations of AEF and the lack of unified protocol for the examination of patients with the first-time upper gastrointestinal tract bleeding. The described reasons in some cases can lead to an inaccurate diagnosis, which entails a loss of time. Among the instrumental diagnostic methods, the most informative is the combination of esophagogastroduodenoscopy and computed tomography of the chest with intravenous contrast enhancement. Each of these methods has its advantages and disadvantages, and allows to identify a number of direct and indirect signs of pathological communication between the aorta and the esophagus.
ель исследования. Определение возможности компьютерной томографии в выявлении характера и тяжести повреждений грудного отдела аорты. Материалы и методы. Проанализированы результаты компьютерной томографии 15 пострадавших с закрытой тяжелой сочетанной травмой и травмой грудного отдела аорты, находившихся на лечении в НИИ СП им. Н.В. Склифосовского с 2006 по 2017 гг. Большинство пострадавших были мужчины 73% (n=11), средний возраст 41,3+ 7,6 (22-79) лет. Причиной травмы в 12 случаях была автоавария, в двух-падение с высоты более трех метров, в одном-падение с высоты собственного роста. Результаты. В оценке степени тяжести повреждения аорты придерживались классификации, предложенной Американской Ассоциацией Торакальных Хирургов в 2013 г. В нашем исследовании пациентов с 1 степенью не было; 2 степень-ложная аневризма малых размеров (меньше 50 % окружности аорты) была у 8 пострадавших; 3 степень-ложная аневризма больших размеров (больше 50% окружности аорты) выявлена у 3 пострадавших, 4 степень-полный разрыв аорты отмечен у 4 пострадавших. Одновременно с травмой грудной аорты у 9 пациентов были выявлены ушибы легкого, у 8-множественные переломы ребер, у 1-перелом грудины. Кровоизлияние в клетчатку средостения отмечено у 8 пострадавших, гемоперикард-у 3. Гемоторакс был у 12 пациентов. Признаки черепно-мозговой травмы выявлены у 6 пациентов, спинальной травмы-у 3, травмы опорно-двигательного аппарата-у 4, повреждения паренхиматозных органов брюшной полости отмечены у 4 пострадавшего (печени-у 3, почки-у 1), у одного пациента были получены признаки разрыва двенадцатиперстной кишки, у другого-был диагностирован разрыв левого купола диафрагмы, у одного пациента одновременно с разрывом грудной аорты была выявлена ложная аневризма собственной печеночной артерии. Обсуждение. У пациентов со стабильной гемодинамикой использование классификации тяжести повреждения аорты по результатам КТ дает возможность определить степень стабильности аорты, прогнозировать риск ее разрыва и выбрать вид оптимального хирургического вмешательства (раннее или отсроченное). Пациентам с травмой аорты и одновременными сочетанными повреждениями других областей, на фоне управляемой гипотонии, имеется возможность выполнить эндопротезирование аорты и оперативное вмешательство других областей. Выводы. При обследовании пациентов с тяжелой сочетанной травмой целесообразно проводить одномоментно компьютерно-томографическое исследование нескольких анатомических областей. Оценка структур средостения у пострадавших с тяжелой сочетанной травмой должна проводиться на основании данных компьютерной томографии груди с обязательным внутривенным введением контрастного препарата. Для повышения точности метода необходимо выполнять построение многоплоскостных и трехмерных реформаций, позволяющих лучше визуализировать весь объем повреждений грудной аорты. Relevance. The variety of options for combining injuries and disorders occurring in the body with a combined injury, the need for rapid decision-making on treatment require rapid diagnosis of the entire volume of injuries and determining the severity of the injury, but there is still no single protocol for examining such victims. The aim of the work was to present a modern CT study protocol for patients with concomitant trauma.Materials and methods. The results of computed tomography of 30 patients with severe concomitant trauma were analyzed. All the victims were admitted in the first hours after the injury. Men were 30 (67 %), women – 10 (33 %), the average age was 41.3 ± 7.6 (22–79) years. Traditional multiphase full-body MSCT was performed in 20 patients, in 10 patients a special protocol was used for the study of the "whole body" by the method of divided bolus.The results of the study. Craniocerebral injuries were detected in 15 (50 %), spinal injuries – in 7 (23.3 %). Simultaneous injuries to the chest and abdomen were detected in 19 (63.3 %) of the victims. When subjectively evaluated, the quality of the images obtained using the full-body MSCT protocol with bolus separation and the standard multiphase protocol for the diagnosis of traumatic injuries was equivalent. The average radiation load per patient with traditional multiphase full-body MSCT is 66 % higher than with the split-bolus protocol.Conclusions. Full-body MSCT using a split bolus is a modern technique that fully meets the diagnostic conditions for severe combined trauma and, with a decrease in the radiation dose, allows you to determine all possible injuries in the victim in a single study.
Lung ultrasound demonstrates a high diagnostic value in the assessment of lung diseases.Aim. To determine the diagnostic accuracy of lung ultrasound compared to chest computed tomography (CT) in the diagnosis of lung changes in COVID-19. Materials and methods. The retrospective study included 45 patients (28 men) aged 37 to 90 years who underwent polypositional lung ultrasound with an assessment of 14 zones. The study compared lung echograms with chest CT data in assessing the prevalence of the process and the nature of structural changes. The diagnostic accuracy, sensitivity, and specificity of lung ultrasound in comparison with CT scans were determined, 95% confidence intervals (CI) were calculated.Results. In 44 patients (98%), CT revealed pathological changes with subpleural localization in both lungs. Of these, in 30 cases, the inflammation was limited only to the subpleural parts, and in 14 cases, the changes spread to the basal parts of the lungs, while ultrasound revealed changes at the depth of the lesion no more than 4 cm. The lesion of 10–11 zones according to lung ultrasound corresponds to CT 1–2 degrees, the lesion of 13–14 zones — CT 3–4 degrees. The sensitivity of ultrasound to detect lung changes of various types was ≥ 92%. The highest sensitivity of 97.9% (95% CI: 92.8–99.8%) was determined for small consolidations on the background of interstitial changes (degree 1A+, 1B+), which corresponded to “crazy-paving” pattern on CT. The specificity depended on the nature of the changes and varied from 46.7 to 70.0%. Diagnostic accuracy was ≥ 81%, the maximum values of 90.6% (95% CI: 85.6–94.2%) were obtained for moderate interstitial changes (grade 1A) corresponding to ground-glass opacity (type one) according to CT data.Conclusion. The sensitivity of ultrasound to detect lung changes in COVID-19 is more than 90%. Lung ultrasound has some limitations: inability to determine the prevalence of the process clearly and identify centrally located areas of changes in the lung tissue.
