Esophageal strictures are the most common complications of bullous epidermolysis in children. Strictures cause the development of dysphagia that prevents oral alimentation and receipt of an adequate amount of nutrients that is accompanied by a violation of nutritional status, weight loss, and delayed physical development of a child. Disturbed swallowing can also cause aspiration syndrome, pneumonia, and airway obstruction. To eliminate dysphagia and reduce the risk of complications, it is necessary to restore esophageal patency. The article describes the results of X-ray with assisted balloon dilatation in 19 children with bullous epidermolysis and dysphagia. It is shown that this method of esophagus recanalization allows to effectively and safely restore oral alimentation of children already in the first day after intervention with a gradual expansion of the diet.

Развитие новых медицинских технологий и поиск малоинвазивных методов лечения ряда урологических заболеваний заставляет по-новому взглянуть на тактику лечения обструк-ции лоханочно-мочеточникового сегмента у детей.Цель исследования. Определение эффективности баллонной дилатации высокого давления (БДВД) лоханочно-мочеточникового сегмента (ЛМС) у детей с гидронефрозом.Материалы и методы. В исследование были включены дети в возрасте от 2 месяцев до 14 лет с доказанным нарушением оттока мочи в области лоханочно-мочеточникового сегмента на основе кли-нико-инструментального исследования. Детям проводилась баллонная дилатация высокого давления лоханочно-мочеточникового сегмента с его последующим стентированием. Стент удалялся спустя 4-6 недель. Оценка результатов проводилось через 3-6 месяцев после удаления стента.Результаты. Баллонная дилатация высокого давления ЛМС показала общую эффективность 73,2%. Во время деривации мочи стентом была отмечена отчетливая декомпрессия чашечно-лоханочной си-стемы. Миграции стентов и наличие осложнений, включая инфекционные, не отмечались. У 11 паци-ентов (26,8%) после удаления стента был отмечен отрицательный результат, что потребовало проведе-ния стандартного оперативного лечения по методике Хайнеса-Андерсона.Заключение. The development of new medical technologies and the search for minimally invasive methods for the treatment of a number of urological diseases makes us take a fresh look at the tactics of treating ureteropelvic junction obstruction in children.The goal was to determine the efficacy of high-pressure balloon dilatation (HPBD) of ureteropelvic junction (UPJ) in children with hydronephrosis.Material and Methods. The study included children from different regions of the Russian Federation aged from 2 months to 14 years with a proven ureteropelvic junction obstruction (UPJO) based on a clinical and instrumental examination. Children underwent HPBD of the UPJO zone with its subsequent stenting. The stent was removed after 4-6 weeks. Evaluation of the results was carried out 3-6 months after the stent removal.Results. The HPBD of the UPJO zone showed a total efficiency of 73%. During the derivation of urine by the stent, clear decompression of the renal collective system was found. Migration of stents and other complications, including urinary tract infections, were not noted. There were an increase in the level of obstruction in 11 (26,8%) patients after stent removal, which required routine surgical treatment by the Heines-Anderson pyeloplasty.Conclusion. Inferior gluteal artery pseudoaneurysms are rare, and most of the cases are due to blunt (pelvic fractures, femur’s dislocations) or penetrating trauma. Herein, we present a case of traumatic blunt rupture of an inferior gluteal artery pseudoaneurysm in adolescent 15th years old. Patient went through the complex examination and endosurgical treatment: endovascular occlusion of inferior gluteal artery and endoscopic evacuation of the large hematoma. Gastrointestinal hemangioma (GIH) is a benign vascular formation located in different parts of the intestinal tube. The clinical significance of hemangiomas of the gastrointestinal tract is usually associated with the development of complications such as gastrointestinal bleeding, invagination, intestinal obstruction or bowel perforation. Aim of the study. Demonstration of a rare clinical observation of jejunal cavernous hemangioma as a cause of recurrent intestinal bleeding. Material and methods. A boy, 11 years old, admitted to the Institute with complaints of chronic anemia and recurrent intestinal bleeding of unknown etiology. There was made a complex diagnostic search, including laboratory and instrumental methods, with a video capsular study of the gastrointestinal tract and angiography of the abdominal cavity. Results. At the base of the video capsular study of the gastrointestinal tract, the vascular formation of the jejunum, which is a possible source of intestinal bleeding, is suspected. The vascular structure of the formation is confirmed by angiography. During laparoscopy, a cavernous hemangioma with a diameter of about 1.5 cm was detected. Hemangioma was located in the wall of the jejunum and prolapsed both into the lumen of the intestine and from the side of the serous membrane. Under micro laparotomic access in the umbilical region, there was performed a resection of the jejunum area with the imposition of the anastomosis "end to end". A morphological diagnosis was confirmed. The postoperative period was uneventful. Conclusion. The cavernous hemangioma of the jejunum is rare and difficult to diagnose the cause of recurrent intestinal bleeding. The use of modern diagnostic and surgical technologies ensures timely and mini-invasive treatment, which leads to rapid recovery of the patient and ensures the cost-effectiveness of treatment.
Цель -определить возможность использования баллонных катетеров, применяемых в кардиохирургии при проведении баллонной дилатации высокого давления (БДВД) для лечения обструкции пузырно-мочеточникового сегмента (ПМС) у детей. Материал и методы. С 2019 по 2021 г. 27 мальчикам и 24 девочкам с диагнозом «первичный обструктивный мегауретер» выполнена БДВД ПМС. Средний возраст на момент оперативного вмешательства -19 мес. Период наблюдения за пациентами составил 1 год. Пациенты при анализе данных были разделены по возрасту на 2 группы: от 1 мес до 1 года, от 1 года до 4 лет. Результаты. Как показали наши исследования, в качестве критериев эффективности лечения можно использовать протяженность стенозированного участка мочеточника, диаметр стеноза и его площадь, определяемые в процессе выполнения БДВД на основании прицельной ретроградной пиелографии. Общая эффективность лечения первичного обструктивного мегауретера с помощью метода БДВД ПМС составила 84%. В 1-й возрастной группе (от 1 мес до 1 года) установлена эффективность 93%, во 2-й (от 1 года до 4 лет) -75%. Положительные результаты лечения получены при протяженности стенозированного участка до 1,8 мм, диаметре стеноза >0,6 мм и площади стеноза <96,6%. При площади стеноза >99% успех от баллонной дилатации не достигнут. Заключение. Использование баллонных катетеров оправдало себя в траслюминарном способе лечения обструктивного мегауретера у детей младшей возрастной группы. Эффективность предложенного нами метода лечения составила 84%. При выполнении БДВД мы также рекомендуем выполнять высокочастотную прицельную пиелографию для оценки характеристик стенозаэто определяет показания к использованию метода БДВД и повышает его результативность.Финансирование.
