Иммунофенотипирование бластов в костном мозге (КМ) является одним из основных методов дифференциальной диагностики лимфомы / лейкоза Беркитта (ЛБ) и острого лимфобластного лейкоза из В-линейных предшественников (ВП-ОЛЛ). Однако определение поверхностной экспрессии тяжелых и легких цепей иммуноглобулинов может быть затруднено. Поэтому целью исследования стало формирование дополнительных иммунофенотипических критериев дифференциальной диагностики ЛБ и ВП-ОЛЛ. Проведено ретроспективное сравнение данных иммунофенотипирования клеток КМ пациентов с верифицированными диагнозами ЛБ и ВП-ОЛЛ. Исследуемую группу составил 21 пациент с ЛБ с наличием бластов в КМ, а группу сравнения-84 ребенка с ВП-ОЛЛ. Был определен ряд особенностей иммунофенотипа ЛБ, который может помочь при оценке результатов проточной цитометрии КМ при дифференциальной диагностике В-клеточных лейкозов. Было показано, что по экспрессии различных поверхностных маркеров ЛБ существенно отличается от ВП-ОЛЛ, поэтому даже при неоднозначном или негативном результате определения экспрессии поверхностного иммуноглобулина М дифференциальная диагностика этих двух нозологий возможна при комплексной оценке иммунофенотипа. В частности, при ЛБ для опухолевых клеток характерно отсутствие экспрессии миелоидных маркеров и CD34. При обнаружении бластных клеток с таким фенотипом необходимо обратить пристальное внимание на долю CD20позитивных клеток, которая достоверно выше при ЛБ, чем при ВП-ОЛЛ. Полученный алгоритм анализа данных при использовании порогового уровня в 87 % CD20-позитивных бластов позволил в исследованной нами выборке абсолютно точно отделить все случаи ЛБ от ВП-ОЛЛ.
Вопросы гематологии/онкологии и иммунопатологии в педиатрии № Одна из форм наследственной тромбоцитопении (НТ) -тромбоцитопения, ассоциированная с мутацией в гене MYH9, кодирующем синтез тяжелой цепи немышечного миозина IIA. Она объеди-няет группу заболеваний: аномалия Мея-Хегглина, синдром Эпштейна, синдром Фехтнера, син-дром Себастьяна, которые характеризуются изолированной тромбоцитопенией с большими или гигантскими формами тромбоцитов, проявляющейся геморрагическим синдромом различной сте-пени выраженности, и развитием негематологических проявлений в виде повышенного риска раз-вития нейросенсорной тугоухости, ранней катаракты и прогрессирующей нефропатии. Обнару-жение больших форм тромбоцитов в мазках периферической крови и специфических включений в нейтрофилах при иммунофлюоресцентном окрашивании -простые методы ранней диагности-ки этой формы НТ, а молекулярно-генетическое исследование помогает в поиске специфиче-ской мутации. В статье представлен клинический случай: ребенок, 9 лет, с НТ, ассоциированной с мутацией в гене MYH9, с характерными клиническими проявлениями, подтвержденной с помо-щью молекулярно-генетического исследования. Показано, что применение агонистов тромбо-поэтиновых рецепторов (рТПО) может быть эффективным методом профилактики кровотечений при данной форме НТ. Inherited thrombocytopenia caused by mutations in MYH9 gene, is one of the forms of inherited platelet disorders. Currently, it comprises a group of diseases previously known as: Mey-Hegglin anomaly, Epstein syndrome, Fechtner syndrome, Sebastian syndrome. These syndromes are characterized by macrothrombocytopenia, and development of non-hematological manifestations such as sensorineural hearing loss, cataract and progressive nephropathy. Finding of large forms of platelets in blood smears and specific inclusions in neutrophils in immunofluorescence staining are simple methods for early diagnosis of this form of inherited thrombocytopenia, molecular-genetic analysis helps to detect the specific mutations. This article is described a clinical case of a 9-years old child with MYH9-related inherited thrombocytopenia, having typical clinical picture and confirmed by molecular-genetic analysis. It illustrated that the administration of agonist TPO-receptors may be effective for prevention of bleeding in this form of inherited thrombocytopenia. MYH9-related inherited thrombocytopeniaE
The length of the end of chromosomes – telomeres is a dynamic and constant characterizes cellular aging process. Measurement of telomere length (TL) and its connection with the diagnosis, course, prognosis and treatment of a number of diseases in recent years, is of great interest for researchers. The methods used for this are varied and are primarily by molecular biological. Variability of methodological approaches, the lack of a common standard of measurement TL measurements in absolute and relative terms: all this leads to results that cannot be compared with each other. The aim of this work is the development of data conversion algorithm is obtained by flow cytometry in absolute telomere length, expressed in kilobases (kb). The study was conducted from venous blood of children 2–3 years of age (8 healthy children and 3 patients with dyskeratosis congenita). As control material used cell line 1301 (ATCC, UK). Determination of relative telomere length was performed by using a set FlowFISH Telomere PNA Kit / FITC (DakoCytomation, Glostrup, Denmark). RTL code and the absolute length of telomeres were calculated for each study participant. Data on the length of patients telomere were compared with data on TL healthy children of appropriate age. RTL average value for healthy children 2–3 years was 30.1, and an absolute telomere length – 15.56 kb. Telomere length patients with dyskeratosis congenita was significantly shorter (RTL 1.4–4.3, the absolute value of TL 0.3–2.4 kb). The algorithm conversion molecular fluorescence equivalent in absolute TL is universal and can be used in any laboratory equipped with a flow cytometer. The resulting data TL can be compared with the data expressed in absolute units and obtained by any other method.
The diagnosis of rare hematological disorders requires a comprehensive clinical and laboratory investigation with careful interpretation of all test results. Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is one of such rare entities. We have performed a retrospective analysis of the results of immunophenotyping, cytomorphology and cytogenetics of bone marrow tumor cells from 5 patients with BPDCN aged from 8 to 51 years. The study was approved by the Independent Ethics Committee of the Dmitry Rogachev National Medical Research Center of Pediatric Hematology, Oncology and Immunology. No specific characteristics of blasts were found. No correlation with the treatment and outcomes was noted as well: 3 patients died of progression or relapse (2 and 1, respectively). Bone marrow immunophenotyping is probably the most valuable laboratory test which allows physicians to establish the proper diagnosis in the absence of skin lesions. Flow cytometry immunophenotyping is the only technique used to determine the antigen profile that enables us to distinguish normal plasmacytoid dendritic cells from tumor ones by the presence (or absence) of the expression of CD2, CD7, CD38, CD56, CD303 etc. In the present paper, we provide a detailed description of five cases of BPDCN and main methods for flow cytometry data analysis. The parents of the patients agreed to use the information, including photos of children, in scientific research and publications.
