Среди страдающих СД в мире ежегодно более 600 тыс. человек пол-ностью теряют зрение. Распространенность катаракты, по дан-ным Государственного регистра сахарного диабета в Россий-ской Федерации, достигает 26%, встречается в более молодом возрасте и коррелирует с длительностью основного заболева-ния. Однако фактическая распространенность осложнений СД выше официально зарегистрированной в 3-4 раза [1,2]. В настоящее время исследователей многих стран волнуют вопросы о том, как свести к минимуму осложнения после хи-рургии катаракты, достичь высоких показателей послеопера-ционной остроты зрения и ускорить реабилитацию пациентов. На сегодняшний день сахарный диабет (СД) является одной из важнейших медико-социальных проблем. Рост численности населения, распространенность ожирения и сидячего образа жизни при увеличении ее продолжительности способству-ют росту числа людей, страдающих данным заболеванием. В настоящее время катаракта остается ведущей причиной сле-поты во всем мире и занимает второе место среди причин низкого зрения при СД.На современном этапе развития офтальмохирургии факоэмульсификация с имплантацией эластичной заднекамерной ин-траокулярной линзы (ИОЛ) является стандартом хирургии катаракты. Совершенствование хирургической методики позво-лило существенно снизить частоту послеоперационных осложнений. Однако в связи с исходно имеющимся поврежде-нием гематоофтальмического барьера риск развития экссудативно-воспалительных реакций у больных СД значительно повышается. Одним из наиболее тяжелых осложнений, влияющим на раннее восстановление и приводящим к снижению остроты зрения в послеоперационном периоде, является кистозный макулярный отек.Это определяет важность совершенствования методов профилактики, поиска оптимальной схемы медикаментозной тера-пии, позволяющей предотвратить развитие данного осложнения после хирургии катаракты у больных СД.КЛЮЧЕВЫЕ СЛОВА: сахарный диабет; катаракта; кистозный макулярный отек; диабетический макулярный отек Nowadays, diabetes mellitus is an important public health problem. Cytokines are polypeptides which are produced by a broad range of cells of the body. They play a key role in the formation and regulation of protective reactions. The researchers attempt to find predictors for the development of exudative-inflammatory reaction after surgery. It led to study of the eyes immunological status. It is known that a change of the pro- and anti-inflammatory cytokines balance leads to the blood-ocular barrier dysfunction that increases the inflammatory reaction. It is interesting the blood-ocular barrier has already damaged in diabetic patients. The paper examines the epidemiology, pathogenesis and possible methods of prevention the postoperative cystoid macular edema, presents investigations describing the change in the local cytokine status in patients with diabetes and the association between cytokines concentrations and the severity of diabetic retinopathy. The manuscript has shown possibilities of multiplex analysis, emphasized the features of individual cytokines and angiogenic factors, highlighted cytokines as predictors of macular edema after cataract surgery. The study of the cytokines role in the development of ophthalmic pathology led to the creation the new pathogenetically oriented eye diseases therapy. Further studies of the cytokine status in patients with macular edema after phacoemulsification are needed to determine the possible treatment methods.
