The growth in the cancer morbidity rate over the last 10 years has exceeded 17% [5].
Introduction. The high prevalence of diseases of the circulatory system of workers in various sectors of the economy, the lack of data on the effects of harmful production factors on the formation of cardiovascular diseases in petrochemical industry workers justify the importance of conducting special studies on these petrochemical complex plants with the aim of assessing cardiovascular risk with the subsequent development of preventive measures. Material and methods. The objects of the study were workers in the production of monomers (ethylene, propylene, ethylbenzene, isoprene, divinyl) of petrochemical enterprises. Hygienic studies of the working conditions of workers included an assessment of the factors of the working environment and the labor process using generally accepted methods. Comprehensive clinical examination of workers included periodic examinations (n = 2634) with calculation of total and relative cardiovascular risk using the SCORE system, using questionnaire data, a Reeder L questionnaire and an in-depth clinical diagnostic medical examination (n = 101) with the definition total cardiovascular risk. Results. The most significant occupational factors of the labor process for workers in the main profession - panmen were established to include the combination of a chemical factor with the occupational noise and labor intensity, with an overall assessment of working conditions 3.2. There were revealed a high prevalence of arterial hypertension and other factors of cardiovascular risk. A higher dependence of the arterial hypertension level on age and length of service in the group of panmen was established when compared to the mechanicians of instrumentation and automation. Using mathematical models of multiple regression, the likely dynamics of the level of arterial hypertension and the left ventricular myocardial mass index were calculated under the influence of increasing age and other risk predictors. Conclusion. The results of a clinical study showed workers with severe arterial hypertension, a high and very high risk of death from cardiovascular disease on the SCORE scale, a high and very high overall cardiovascular risk, signs of dyslipidemia, obesity, high stress levels to require close attention. It is important to timely diagnose lesions of target organs, to conduct rational prevention aimed at reducing risk factors. According to the results of the study, there were developed preventive measures aimed at reducing cardiovascular morbidity and cardiovascular risk, prolonging the longevity of workers in petrochemical industries.
The estimation of professional risk on the basis of a priori and a posteriori criteria at five enterprises of
the fuel and energy complex located in the Volga and Urals Federal Districts was carried out. Based on
the conducted studies, a conceptual model for assessing risk management in oil producing, oil refining
and petrochemical enterprises was developed, tested in real conditions and demonstrated high efficiency.
We have compared working conditions and health state of workers engaged in low- and large-tonnage
ethylbenzene-styrene manufacturing. It has been shown that working conditions of low-tonnage workers
are of сlass 3.3. They cause chronic occupational diseases as well as a high degree of work-related chronic
diseases. Working conditions of large-tonnage workers are of сlass 3.1. Careful medical examination has
not revealed any changes in «critical» organs and systems.
Our aim was to study how significant psychosocial factors are in occupational stress and cardiovascular diseases evolvement in workers employed at petrochemical production; we also intended to work out a set of preventive measures. Our hygienic and social-psychological research enabled us to detect factors causing stress evolvement in workers employed at petrochemical production. These factors included chemical impact, noise, unfavorable microclimate, labor hardness and labor intensity. High level of risk for their own lives and responsibility for safety of others, as well as work under time deficiency conditions with increased responsibility for the final results, were the most significant psychosocial factors for workers. In the course of questioning we detected that 74 % machine operators, 63 % tool men working with controllers and automatic devices, and 57 % repairmen mentioned having stress at work. Here 38 % workers gave a subjective estimation of their professional activity as having apparent "stress nature". The questioning revealed that 48 % workers with various occupations had increased parameters as per anxiety scale (HADS); 23 % workers had increased parameters as per depressions scale (HADS). Primary hypertension was the most widely spread nosologic form among chronic noninfectious diseases; it was found in 46.1 % operators and in 45.2 % repairmen dealing with processing stations repair. 30.1 % tool men working with controllers and automatic devices had average occupational causation of primary hypertension by production factors. We detected direct relation between hyperlipidemia and age and working period. We created foundation for preventive measures and worked out a program aimed at increasing resistance to stress at corporate and individual level. It will provide significant social effect and later on economic one. To overcome social stress we need to create safe working conditions at workplaces and to increase labor motivation based on career development possibilities.
