В статье представлены результаты деятельности Регионального сосудистого центра (РСЦ) г. Грозного по оказанию специализированной помощи больным с нарушениями мозгового кровообращения с 2014 по 2016 гг. Цель исследования: анализ структуры заболеваемости инсультом и результатов лечения больных с инсультом в Чеченской Республике после начала функционирования регионального сосудистого центра. Материал и методы: Проведено ретроспективное исследование 1028 больных с инсультом, поступивших в региональный сосудистый центр г. Грозного с января по июнь 2014 г. и с июля по декабрь 2015 г. Изучены их эпидемиологические характеристики, вид нарушения мозгового кровообращения, объем диагностических процедур, тактика лечения и исходы, а также дальнейшая преемственность в лечении и реабилитации.
В статье рассматривается одна из актуальнейших медико-социальных проблем - проблема черепно-мозгового травматизма. Целью исследования в данной работе явилось выявление факторов, ведущих к развитию неблагоприятных исходов при черепно-мозговой травме (ЧМТ), и на основании этого разработка соответствующих рекомендаций. Материалы и методы исследования включали ретроспективный анализ документации 117 больных с тяжелой ЧМТ, поступивших в ГКБ №1 в 2014 и в 2015 годах. Регистрировались такие параметры как: пол, возраст, уровень сознания при поступлении, данные компьютерной томографии, показатели гемодинамики, неврологический статус, длительность комы и исход. На основании проведенного исследования факторы риска неблагоприятного исхода при ЧМТ были разделены на модифицируемые и немодифицируемые. Среди немодифицируемых факторов наибольшее значение относительного риска имели пол, возраст, уровень сознания по ШКГ и характер первичных повреждений на КТ. Среди модифицируемых факторов выделены - сроки доставки в стационар, коррекция гемодинамики и электролитных нарушений, объем и сроки операций при ЧМТ с масс-эффектом.
