Статья посвящена редкой патологии из группы болезней накопления с аутосомно-рецессивным типом наследования-муколипидозу II и IIIA типов. Заболевание отличается большим фенотипическим сходством с мукополисахаридозом. Цель работы: анализ генофенотипических показателей у российских больных с муколипидозом II и IIIA типов. Активность лизосомных ферментов в плазме (β-глюкуронидазы, общей гексозаминидазы и N-ацетил-альфа-D-глюкозаминидазы) измерялась по стандартной методике с использованием хромогенных и флюорогенных субстратов. Геномная ДНК лейкоцитов периферической крови выделялась с помощью набора реактивов Preb 100 (DIAtom TM). Амплификация всех экзонов гена GNPTAB проводилась методом полимеразной цепной реакции (ПЦР) с последующим прямым нерадиоактивным секвенированием по Сэнгеру. Обследованы 50 больных в возрасте от 1,5 до 10 лет. Клиническая симптоматика болезни включала: Гурлер-подобный фенотип, задержку роста, поражение скелета, сердца и сосудов, ЦНС. Муколипидоз II типа (I-клеточная болезнь) отличался более тяжелым течением. Клинический диагноз был подтвержден результатами лабораторных методов исследования: нормальными показателями почечной экскреции гликозаминогликанов (ГАГ), высокой (в 5-15 раз выше нормы) активностью лизосомных гидролаз в плазме крови и выявлением мутаций в гене GNPTAB. Полностью генотипированы 35 пробандов. У 8 больных выявлено только 8 мутантных аллелей; у 7-мутации не обнаружены. Найдено 6 новых мутаций в экзонах 1 (p.I31N; p.Q36P), 10 (p.L398P), 11 (p.W446X) и 13 (p.S738X; c.2250delT), в том числе частая для российских больных мутация p.S738X (21% аллелей). Наиболее частой (31,4% аллелей) в российской когорте пациентов оказалась известная мелкая делеция c. 3503_3504delTC, приводящая к сдвигу рамки считывания. Представлено клиническое наблюдение ребенка с муколипидозом II типа (I-клеточная болезнь) с типичной симптоматикой заболевания, обусловленной двумя нонсенс-мутациями гена GNPTAB: p.S738X/p.R375X. В заключение подчеркивается, что идентификация мутаций гена GNPTAB обеспечивает прогнозирование тяжести течения болезни, понимание механизмов ее развития, что будет способствовать разработке методов патогенетического лечения, улучшению качества жизни больных и эффективному медико-генетическому консультированию.
Mucopolysaccharidosis type II (MPS II; Hunter syndrome) is an X-linked hereditary disorder associated with a deficiency of iduronate2-sulfatase (IDS). IDS deficiency provokes the accumulation of dermatan sulfate and heparan sulfate in different tissues. Clinical manifestations of MPS II are heterogeneous and involve different organs. Two phenotypes are distinguished: attenuated or severe; classification is based on central nervous system impairment signs. The review provides data on the current treatments opportunities for Hunter syndrome and perspectives for development of new therapeutic approaches. Current treatment includes intravenous enzyme replacement therapy (ERT), hematopoietic stem cell transplantation, and symptomatic treatment. Intravenous enzyme replacement therapy does not promote the enzyme to penetrate the blood-brain barrier which leads to the treatment failure for neurological signs and symptoms; hematopoietic stem cell transplantation has high risk of post-transplantation complications but can improve some neurological problems. Intrathecal ERT, substrate reduction, pharmacological chaperones, and gene therapy are currently under investigation as therapies for severe form of MPS II. Development of new approaches to treatment of Hunter syndrome and other hereditary diseases is extremely vital.
Mucopolysaccharidosis I-Hurler (MPS I-H) is the most severe form of a metabolic genetic disease caused by mutations of IDUA gene encoding the lysosomal α-L-iduronidase enzyme. MPS I-H is a rare, life-threatening disease, evolving in multisystem morbidity including progressive neurological disease, upper airway obstruction, skeletal deformity and cardiomyopathy. Allogeneic hematopoietic stem cell transplantation (HSCT) is currently the gold standard for the treatment of MPS I-H in patients diagnosed and treated before 2–2.5 years of age, having a high rate of success. Enzyme replacement therapy (ERT) with human recombinant laronidase has also been demonstrated to be effective in ameliorating the clinical conditions of pre-transplant MPS I-H patients and in improving HSCT outcome, by peri-transplant co-administration. Nevertheless the long-term clinical outcome even after successful HSCT varies considerably, with a persisting residual disease burden. This review will focus on all these critical issues related to the management of MPS I-H.
Статья посвящена редкой патологии из группы болезней накопления с аутосомно-рецессивным типом наследования - муколипидозу IIIA типа у больных, представителей коренного населения Республики Тыва. Обследованы 10 детей с этим редким заболеванием. Показаны основные клинические симптомы болезни; представлены 3 последовательных этапа диагностических мероприятий, на основании которых у всех больных была обнаружена новая нуклеотидная замена с.3169T>G; Cys1057Gly в экзоне 16 гена GNPTAB. Выявленная замена обнаружена у всех больных в гомозиготном состоянии. Высказано предположение, что данный вариант замены является следствием эффекта родоначальника и его носительство характерно только для тувинцев. Для эффективного медико-генетического консультирования тувинских семей рекомендуется популяционное обследование коренного населения Республики на носительство мутантного аллеля гена GNPTAB. The article is devoted to a rare pathology from the group of accumulation diseases with an autosomal recessive type of inheritance - type IIIA mucolipidosis in patients of the indigenous population of the Republic of Tyva. Ten children with this rare disease were examined. The main clinical symptoms of the disease are shown; 3 consecutive stages of diagnostic measures are presented, on the basis of which a new nucleotide substitution c. 3169T> G was found in all patients; Cys1057Gly in exon 16 of the GNPTAB gene. Revealed replacement was found in all patients in a homozygous state. It has been suggested that this option of replacement is a consequence of the effect of the ancestor and its carriage is characteristic only of persons of Tuvan nationality. For effective medical and genetic counseling of Tyva families, a population survey of the indigenous population of the Tuvan Republic on the carriage of the mutant GNPTAB gene is recommended.

Hypophosphatasia is a rare genetic disease caused by deficiency of tissue-specific alkaline phosphatase due to mutation in the ALPL gene. The article covers all the features of epidemiology, etiology and pathogenesis, detailed stages of differential diagnostics. Treatment guidelines for pediatric patients are provided, they are based on the principles of evidence-based medicine. Special attention was given to the only effective method of hypophosphatasia management —enzyme replacement therapy (ERT). This material is the clinical guideline draft for the management of patients with hypophosphatasia prepared by the Union of Pediatricians of Russia and the Association of Medical Geneticists.
