Выявить влияние генетических факторов в виде семейной отягощенности алкогольной зависимостью и полиморфизма генов белка-переносчика дофамина (SlC6a3(Dat1) и фермента дофамин-бета-гидроксилазы (Dbh) на риск развития тяжелых осложнений синдрома отмены алкоголя (СоА). Материал и методы. 266 стационарных пациентов с алкогольной зависимостью, мужчин, были изучены в двух группах сравнения: 130 случаев СоА с осложнениями (судорожные припадки (СП), делирий (Дл), их сочетание (СП+Дл) и 136 СоА без осложнений. Генотипирование ДнК проводили по полиморфным локусам генов (SlC6a3(DAT1) (40-bp Vntr 3' utr, rs27072) и DBH (rs1611115). информацию о семейной отягощенности, развитии и течении заболевания получали с помощью структурированного интервью. Результаты. Пациенты с осложнениями СоА чаще имеют семейную отягощенность (Со) (54,6 и 33%, p=0,001) и отличаются более ранним возрастом к периоду первой пробы алкоголя (ВППА) (16±3,53 и 17±1,66, p=0,001). логистическая регрессия подтвердила, что Со увеличивает риск развития осложнений СоА в целом (р=0,001) и Дл (р=0,003), а ВППА независимо увеличивает риск осложнений в целом (р=0,008), СП (р=0,04), Дл (р=0,032), (Дл+СП) (р=0,048) и каждый год отсрочки первых проб алкоголя снижает риск на 18-30%. Полиморфные варианты взаимодействуют с ВППА, снижая его негативное влияние на: риск СП -т Dat (rs27072) (р=0,04), риск (Дл+СП) (р=0,023) и Дл (р=0,06) -т Dbh (rs1611115). Аллель т Dat (rs27072) связан с ВППА (p=0,007) и увеличивает риск (Дл+СП) (р=0,036), но снижает риск СП (р=0,038) и риск Дл (р=0,021), но только при взаимодействии с Со. Аллель А9 Dat Vntr связан с СП (p=0,009), однако только у носителей А9 с Со повышен риск СП (р=0,004) и осложнений СоА (р=0,001). также А9 DatVntr увеличивает риск Дл (р=0,048), а при наличии большого количества больных алкогольной зависимостью в семье эффект становится более выраженным (р=0,014). Взаимодействие генов а9 DATVNTR х т DBH (rs1611115) снижает риск Дл (р=0,055). независимо от вариантов осложнений СоА, аллель т Dbh (rs1611115) связан с наличием продуктивной психопатологии (p=0,039). Заключение. Генетические факторы в виде семейной отягощенности и полиморфных вариантов генов DAT и DBH увеличивают риск развития осложнений СоА, их взаимодействие имеет максимальный эффект, имеется определенный протективный эффект генетических вариантов в отношении влияния поведенческого фактора риска -раннего возраста больного к периоду первых проб алкоголя. результаты исследования могут быть использованы в целях эффективной профилактики при условии проведения дополнительных исследований.Ключевые слова: зависимость от алкоголя, генетика, наследственность, алкогольные ...
At least 50% of factors predisposing to alcohol dependence (AD) are genetic and women affected with this disorder present with more psychiatric comorbidities, probably indicating different genetic factors involved. We aimed to run a genome-wide association study (GWAS) followed by a bioinformatic functional annotation of associated genomic regions in patients with AD and eight related clinical measures. A genome-wide significant association of rs220677 with AD (p-value = 1.33 × 10−8 calculated with the Yates-corrected χ2 test under the assumption of dominant inheritance) was discovered in female patients. Associations of AD and related clinical measures with seven other single nucleotide polymorphisms listed in previous GWASs of psychiatric and addiction traits were differently replicated in male and female patients. The bioinformatic analysis showed that regulatory elements in the eight associated linkage disequilibrium blocks define the expression of 80 protein-coding genes. Nearly 68% of these and of 120 previously published coding genes associated with alcohol phenotypes directly interact in a single network, where BDNF is the most significant hub gene. This study indicates that several genes behind the pathogenesis of AD are different in male and female patients, but implicated molecular mechanisms are functionally connected. The study also reveals a central role of BDNF in the pathogenesis of AD.
Controlling the epidemic of overweight and obesity is one of the major challenges to modern healthcare. One of the possible causes of overweight and obesity can be food addiction manifesting as overeating and other eating disturbances (ED).We conducted a cross-sectional study to test the association between addiction-related ANKK1/DRD2 gene Taq1A polymorphism (rs1800497) and ED in overweight patients. Overall, 527 outpatients (469 (89,0%) females, 58 (11,0%) males; mean ± SD: BMI — 35,8±7,3 kg/m2; age — 46,7±11,8 years) of European ancestry were included in the study. Eating Disorder Examination — Questionnaire (EDE-Q), Eating Attitudes Test (EAT-26) and Dutch Eating Behavior Questionnaire (DEBQ) were used to assess the ED. We also used Hospital Anxiety and Depression Scale (HADS), Hamilton Rating Scale for Depression (HRDS), Hamilton Anxiety Rating Scale (HARS) and Yale-Brown Obsessive-Compulsive Scale (YBOCS) to assess depressive, anxiety and obsessive-compulsive disturbances respectively. DNA was extracted from blood samples and polymorphism rs1800497 was detected by RT-PCR.According to the dominant genetic model, carriers of minor T allele (“CT+TT” group) in contrast to wild-type allele homozygous patients (“CC” group) were younger (p=0,075) and demonstrated higher scores of EDE-Q (p=0,085) and emotional eating subscale of DEBQ (p=0,063). Moreover, among them the proportion of patients with high ED risk was significantly increased vs. CC group: 1) EAT-26 score: 15,2% vs. 9,0%, OR=1,82 (CI95% (1,054-3,134), p=0,03; 2) EDE-Q score: 46,6% vs. 37,5%, OR=1,45 (CI95% (1,015-2,072), p=0,041. BMI was correlated with emotional eating subscale of DEBQ score in “CT+TT” group only (ρs =0,199, p=0,004). Correlation analysis showed weaker connections between ED and affective disturbances in “CT+TT” group.Our results show that carriers of the addiction-related T allele of ANKK1/DRD2 gene Taq1A polymorphism comprise the at-risk group for eating disturbances in overweight individuals. This may support the role of food addiction in overweight and obesity.
