In 1997, the International Expert group on chronic obstructive pulmonary disease (COPD) has come to decision to initiate the Global Initiative for COPD (GOLD). The first GOLD report was published in 2001 and provided the basis for many national programs for COPD diagnosis and management. Research data accumulated in the next decade allowed to perform complete revision of global strategy in 2011 (updated 2013), considering personalized approach to prognosis and treatment selection in an individual patient. At the end of2016, anew comprehensive reassessment of GOLD report was released. The ABCD assessment tool has been refined; the role of spirometry in COPD management has been updated. Regular assessment of inhaler technique to improve the disease control has been emphasized. New data on therapy with long-acting β2-agonists/long acting anticholinergic combinations and «triple therapy» have been added. Classification of severity of COPD exacerbations was provided and impact of concomitant diseases was stressed. The section on pulmonary rehabilitation has been amplified.
ОбзорыПредставление о хронической обструктивной болез ни легких (ХОБЛ) как о гетерогенном заболевании на сегодняшний день общепризнано. Несмотря на гетерогенность, воспаление дыхательных путей и бронхоконстрикция играют главную роль в пато физиологии ХОБЛ и являются важнейшими мише нями для фармакологического воздействия. Вопрос о месте комбинаций, содержащих ингаляционные глюкокортикостероиды (иГКС), в терапии ХОБЛ провоцирует горячие диспуты в научных кругах. С одной стороны, доказано их влияние на снижение частоты обострений, с другой -существует риск в виде развития нежелательных явлений. Как наи лучшим образом соотнести пользу и риск при выбо ре терапии конкретному пациенту? Есть ли биомар керы, позволяющие сделать наиболее оптимальный выбор? В обзоре, опубликованном в журнале Expert Opinion on Pharmacotherapy, рассматриваются разно сторонние аспекты пользы / риска группы препа ратов, содержащих иГКС и длительно действующие β 2 агонисты (ДДБА) [1]. Молекулярные аспекты взаимодействия ГКС с клеткойГКС представляют собой жирорастворимые молеку лы, которые легко проникают сквозь клеточную мембрану в цитоплазму клетки, где связываются с цитоплазматическими глюкокортикоидными ре цепторами (ГР). Активированные комплексы гор мон-рецептор направляется в ядро. Соединяясь по парно, они образуют гомодимеры, связывающиеся со специфическими последовательностями ДНК -стероид чувствительными элементами, располо РезюмеХроническая обструктивная болезнь легких (ХОБЛ) является актуальной проблемой современного здравоохранения. Патогенез ХОБЛ связан с воспалением, поэтому в арсенале препаратов для лечения этого заболевания существенную роль играют противовоспалитель ные препараты -ингаляционные глюкокортикостероиды (иГКС). Доказаны благоприятные эффекты иГКС на исходы болезни, одна ко их использование может сопровождаться повышением риска развития нежелательных явлений, в частности, пневмонии. В статье приведен обзор результатов исследований, в которых продемонстрированы долгосрочные положительные эффекты применения ком бинаций иГКС и длительно действующих β2 агонистов (ДДБА) у пациентов с различной степенью тяжести ХОБЛ, в т. ч. новой комби нации для однократного применения -флутиказона фуроата / вилантерола. Также рассмотрены данные о пневмонии как наиболее час то обсуждаемом неблагоприятном эффекте применения иГКС / ДДБА, приведены последние доказательства о класс специфичности данного эффекта. Кроме того, рассмотрены последние доказательства в пользу применения нового биомаркера, потенциально способ ного выделить фенотип пациентов, для которых назначение иГКС содержащих препаратов является наиболее оправданным, -таким биомаркером может быть уровень эозинофилов в крови. Конгресс18-21 октября 2016 г. в Москве прошел XXVI Нацио нальный конгресс по болезням органов дыхания. Большой интерес участников вызвали сложные во просы ведения пациентов с хронической обструк тивной болезнью легких (ХОБЛ) -возможности ранней диагностики, основы персонализированного подхода к выбору терапии, пути улучшения выпол нения пациентами рекомендаций врача, особенно сти различных устройств доставки ингаляционных препаратов. Активная дискуссия проходила по пово ду позиции препаратов разных фармакологических классов в современных схемах терапии ХОБЛ.В рамках Конгресса прошел симпозиум «Терапия хронической обструктивной болезни легких (ХОБЛ): практика, основанная на доказательствах». Симпози ум проходил под председательством заведующего ка федрой пульмонологии ФГБОУ ДПО «Российская медицинская академия последипломного образова ния» Минздрава России профессора А.И.Синопаль никова и медицинского руководителя глобально го респираторного подразделения компании ЗАО «ГлаксоСмитКляйн Трейдинг» (GSK) д ра Адама Хо кинса (Великобритания). Финансовую поддержку симпозиума осуществляла компания GSK. Обозна чая глобальные проблемы в лечении пациентов с ХОБЛ, д р Хокинс отметил, что существующие кли нические рекомендации (GOLD, 2011(GOLD, -2016 ставят перед врачом дилемму, предлагая одни и те же вари анты терапии больных с совершенно разным тече нием заболевания. Подобный вопрос возникает при лечении пациентов, относящихся к группам высоко го риска (C и D) по классификации GOLD.Для наглядности представлены 2 клинических наблюдения, каждое из которых можно отнести к группе высокого риска обострений и ускоренного прогрессирования заболевания. В первом случае у относительно молодого пациента с выявленными умеренными спирометрическими нарушениями (объем форсированного выдоха за 1 ю секунду (ОФВ1) после ингаляции бронхолитического препа рата -64 %долж.) и жалобами на одышку легкой степе ни (1 балл по шкале mMRC)) отмечены повторные обострения ХОБЛ; больной госпитализирован по поводу такого обострения несколько месяцев назад. Другой пациент -пожилой человек с тяжелой сте пенью ограничения воздушного потока (ОФВ 1 -32 % долж.) и одышкой при минимальных физических нагрузках, но без обострений в течение последнего года наблюдения. Для обоих пациентов в клиничес ких рекомендациях предложен выбор препаратов 2 фармакологических классов:• ингаляционные глюкокортикостероиды (иГКС) в комбинации с β2 агонистами длительного дей ствия (ДДБА); • длительно действующие антихолинергические препараты (ДДАХП). В выборе лечения бронхиальной астмы (БА) основ ным ориентиром является достижение контроля над заболеванием и предупреждение риска будущих не благоприятных событий с использованием ступенча того подхода, предложенного современными реко мендациями [1]. Доказательная база современных клинических рекомендаций формируется прежде все го по результатам рандомизированных двойных сле пых плацебо контролируемых клинических иссле дований (РКИ), в которых предусмотрены строгие критерии отбора пациентов и распределение их слу чайным образом в группы активной терапии и конт роля, что исключает влияние на результат любых воз можных факторов, кроме исследуемого препарата [2]. Выявлено, что в крупной разнородной популяции больных БА лече ние комбинацией вилантерола и флутиказона фуроата (ВИ / ФФ) 22 / 92 мкг или 22 / 184 мкг 1 раз в день в порошковом ингаляторе Эллипта обеспечивало лучший контроль над заболеванием по сравнению со стандартной терапией, назначаемой и изменяемой по усмотрению врача. Уровень контроля над БА в группе ВИ / ФФ стабильно поддерживался на протяжении 12 мес., при этом изменения риска развития серьезных нежелательных явлений не выявлено. Результаты не зависели от терапии, которую исходно получали больные (ингаляционные глюкокортикостероиды или их комбинации с длительно действующими β2 агонистами), а также от статуса курения и количества обострений БА за прошедший год.
