Сахарный диабет (СД) 1 типа – хроническое заболевание, имеющее специфические особенности в каждом возрастном периоде, что следует обязательно учитывать для достижения хороших показателей гликемического контроля. В настоящее время имеется немало публикаций, посвященных вопросам ориентированного перевода пациентов из педиатрической системы медицинского наблюдения во взрослую, при этом большинство из них имеет недостаточную доказательную базу, так как основывается на субъективном мнении экспертов или клиническом опыте конкретного лечебного учреждения. Согласно определению, принятому Society of Adolescent Medicine (Обществу медицины для взрослых пациентов), под термином «transition» (с англ. – «переход») понимается целенаправленный, спланированный и своевременный переход от педиатрической, семейно-ориентированной системы медицинской помощи к взрослой, пациент-центрической. Известно, что некорректный перевод может обернуться выпадением пациента из системы медицинского наблюдения, к тому же ошибки при переводе оказывают отрицательный эффект на общую заболеваемость и летальность у молодых взрослых с СД 1 типа. Так, исследования выявили увеличение частоты как тяжелых гипогликемий, так и диабетического кетоацидоза в первый год после перехода во взрослую сеть. Помимо повышенного риска острых осложнений СД 1 типа, многие авторы связывают переход во взрослую сеть с возрастающей вероятностью появления/прогрессирования хронических осложнений диабета, обусловленных ухудшением метаболического контроля. Для снижения подобных рисков перевод подростков с СД 1 типа во взрослую сеть должен быть своевременным, непрерывным и подготовленным, а необходимость разработки региональных структурированных программ перевода может быть обоснована целью сохранения здоровья и улучшения качества жизни большого числа пациентов. Type 1 diabetes mellitus (T1D) is a chronic disease with each age period specific features, which should be taken into account for achieving glycemic targets. There are many publications about the transition of patients from pediatric medical system in the adult's service, but most of them still have insufficient evidence based on subjective expert opinion or on clinical centered experience. According to the definition adopted by the Society of Adolescent Medicine, the term «transition» refers to a purposeful, planned and timely transition from a pediatric, family-oriented system of medical care to an «adult», patient-centered system. It is known that incorrect transition can lead to patient's «falling out» of medical supervision system and errors in transition have a negative effect on the overall morbidity and mortality, frequency of severe hypoglycemia and diabetic ketoacidosis in the first year after the transition to the adult's service in young adults with T1D. In addition to the increased risk of acute complications of T1D, many authors associate the transition with an increasing occurrence/progression of chronic complications of diabetes due to the deterioration of metabolic control. To reduce such risks, the transition of adolescents with T1D to the adult's service should be well-timed, continuous and prepared, and now it's time to develop regional structured transfer programs for preserving health and improving the quality of life for a large number of our patients.
ЦЕЛЬ: по состоянию на 1 февраля 2023 года в Санкт-Петербурге на учете в Городском детском эндокри- нологическом центре состоит 2418 детей с сахарным диабетом 1 типа. С июля 2022 года все дети старше 4 лет с сахарным диабетом 1 типа обеспечиваются датчиками системы флэш-мониторирования гликемии. Нами проанализированы данные мониторирования с целью оценки показателей гликемического контроля у детей с сахарным диабетом 1 типа, проживающих в Санкт-Петербурге. МАТЕРИАЛЫ И МЕТОДЫ: исследование проводилось на базе СПбГБУЗ ДГМКЦ ВМТ имени К.А.Раухфуса. Оценены данные мониторирования 648 детей с длительностью заболевания не менее 1 года в возрасте от 4 до 17 лет (включительно), из них: 326 мальчиков (50,3%) и 322 девочки (49,7%). Дети были разделены на следующие возрастные группы – 4-6 лет (n=85 13,1%), 7-10 лет (n=198 30,6%), 11-15 лет (n=279 43,1%) и 16-17 лет (n=86 13,3%). Контроль показателей гликемии осуществлялся системой флэш-мониторирования. Оценивалось время в целевом диапазоне (TIR), время выше целевого диапазона (TAR), время ниже целево- го диапазона (TBR) и коэффициент вариации (CV). Параметры TIR, TAR и TBR рассчитывались автоматически и оценивались согласно Международному Консенсусу по использованию систем непрерывного монито- рирования гликемии и времени в целевом диапазоне по отчетам стандартного амбулаторного профиля глюкозы (АПГ). Отчеты АПГ были получены на онлайн платформе LibreView. Статистический анализ проводился с использованием программы StatTech v. 3.0.9 (разработчик - ООО «Статтех», Россия). РЕЗУЛЬТАТЫ: дети в возрасте 4-6 лет имели достоверно более высокие показатели TIR и низкие пока- затели TAR по сравнению с детьми других возрастных групп. TIR у детей в возрасте 4-6 лет 80% (69;86) до- стоверно выше (p<0,001), чем в группах 7-10 лет - 57% (48;68), 11-15 лет - 62% (43;75) и 16-17 лет -54% (43;62). TAR в возрасте 4-6 лет составило 11% (6;17) достоверно меньше (p<0,001), чем в группах 7-10 лет - 34% (24;43), 11-15 лет - 29% (15;50) и 16-17 лет - 33% (24;44). При оценке показателя TBR были получены следую- щие статистически значимые различия – максимальное значение (p<0,001) зафиксировано в возрастной группе 16-17 лет - 10% (5;16), в возрасте 4-6 лет 7% (3;11), группах 7-10 лет - 6% (2;10), 11-15 лет - 4% (2;8). CV был достоверно ниже (p<0,001) также в группе 4-6 лет 33% (30;38), чем в остальных группах. Максимальное значение CV отмечалось в группе 16-17 лет и составило 53% (51;57) ВЫВОДЫ: ухудшение показателей гликемического контроля у детей в возрасте 7-10 и 11-15 лет про- исходит за счет повышенной частоты гипергликемических эпизодов при относительно низкой частоте гипогликемий, в то время как в возрасте 16-17 лет уменьшение времени в целевом диапазоне происходит за счет как гипер- так и гипогликемий. Достижение целевых значений гликемического контроля у детей в возрасте 4-6 лет может быть объяс- нено их большей зависимостью от родителей меньшей самостоятельностью в принятии терапевтических решений.
Background: Only a little percent of chronically ill patients was found to follow physicians prescriptions. One of the reasons for this issue is misunderstanding of recommendations due to inappropriate interpretation of medical terms, contained in medical advice. Aims: The study is aimed to evaluate the quality of patient interpretation of the most frequently used medical terms in diabetes mellitus field (DM) and to evaluate the impact of misunderstanding on diabetes control. Materials and methods: 13 endocrinologists composed 2 lists of the most frequently used terms one list for DM type 1 and one for DM type 2. We selected 10 terms for DM type 1 and 10 terms for DM type 2, mentioned by the most of participated doctors, and created 2 kinds of questionnaire for patients. Patients were to explain the terms in written if they were aware of terms meaning. Three independent researchers evaluated every answer according to a 0 to 10 scale, where 0 was for totally incorrect or no answer, and 10 was for a completely correct answer. Patients also filled in the forms about their social and demographic parameters. Statistical analysis was conducted with the use of Wilcoxon Test and linear regression model. Results: 89 patients with DM type 1 (27% men, HbA1c (meanSD) 7,951,77%) and 86 patients with DM type 2 (27% men, HbA1c (meanSD) 8,111,91%) were included into the study. Patients with type 1 DM received a greater overall score for understanding the terms than those with type 2 DM (p 0.0001) 57.8422.66 and 39.3322.02 from 100, respectively. 38 (42.7%) participants with DM type 1 reported that they know all 10 terms, but only 15 (16.8%) respondents understand terms correctly. In the group of type 2 DM patients 9 (10.5%) of all answered yes for all the terms, but really know terms only 2 (2.3%) participants. In both groups, the total score of the terms knowledge did not correlate with the HbA1c level (р=0.698 and р=0.319 for type 1 and type 2 DM groups, respectively). Conclusion: The most of patients with DM do not understand relevant medical terms properly. Some patients are in the wrong belief that they have no misunderstandings with their consulting doctors. However, terms understanding does not influence on glycemic control (HbA1c level). During the medical consultation, endocrinologists should check if a patient understands their advice properly to improve understanding and compliance of patients.
BACKGRAUND: A closed-loop glucose control system or artificial pancreas consists of three components a Continuous Glucose Monitor (CGM), infusion pumps to deliver hormone(s) and a sophisticated dosing algorithm to control hormone delivery. In the past years, numerous studies with closed-loop system devices were conducted with gradual shift to out-of-hospital environment and with lengthening study duration. AIMS: To compare efficacy and safety of closed-loop insulin pump use in children with type 1 diabetes mellitus in compare with conventional insulin treatment (continuous subcutaneous insulin infusion (CSII) with our without CGM) based on randomized control trials data (RCT). METHODS: In the systematic review we have include 28 randomized controlled trials results indexed in PubMed, Medline databases published till 15 June 2017. The efficacy on metabolic control in this study evaluated by the proportion of time within target range (preferably 70 to 180 mg/dl if reported) and mean (median) glucose based on sensor measurements, and the safety evaluated by time in hypoglycemia (below 70 mg/dl if reported). RESULTS: Increased time in range in the night period was observed in all RCT. Only 3 RCT showed decrease of the time in range within 24 h evaluation period. In one RCT the significant positive differences have been shown in the time in range for dual hormone closed-loop glucose control system in compare with insulin-only artificial pancreas. Mean glycaemia and glucose variability changes were not in the same manner in different RCT, both in the night only and in 24 h estimation period. Night hypoglycemia duration decreased in most RCT with closed-loop control in compare with CSII, and increased only in 2 RCT. When all-day estimation period the time in hypoglycemia changed not in the same manner in different RCT. Valuable methodology differences of the glycaemic control estimation within observed RCT brought significant complications in the data analysis and made impossible the results quantitative estimation to prepare a metaanalysis. CONCLUSIONS: Much work has been done to develop effective and safe artificial pancreas, but not all RCTs confirmed advantages of closed-loop glucose control in compare with CSII in children and adolescents in real life. More research with prospective randomized control design required to prove benefits of closed-loop glucose control. Further RCTs should have an uniform methodology for glycemic control assessment and long duration that will allow to use cumulative measures in a closed-loop efficacy estimation (HbA1c).
