The article presents the analysis of the health status of children born prematurely during the first year of their life. There is the comparison of morbidity rates for five years of the operation of the catamnesis center of Kazan. Profoundly premature children demonstrated the highest level of morbidity with more frequent pathologies of the central nervous system, respiratory organs, eyes and anemia of premature children. In 2017 there was a decrease in the frequency and severity of respiratory diseases (bronchopulmonary dysplasia with chronic respiratory insufficiency 1.6 times), the nervous system (severe ischemic and hypoxic-hemorrhagic lesions of CNS 2.7 times), eyes and its adnexa (a complicated retinopathy of premature children 1.7 times) in extremely premature infants.
The article presents the clinical evidence of safety and efficacy of mucolytics and expectorants in the treatment of both productive and non-productive coughs in respiratory infections in childhood in all stages of the infectious-inflammatory process. The authors set a goal to study the efficacy and safety of mucolytics and expectorants in children with respiratory infection in the pediatric practice. Materials and methods. A total of 96 children undergoing inpatient treatment for various variants of acute respiratory infection were examined. The average age of the patients was 7.6 ± 0.9 years old (from 3 to 14 years old). All patients were divided into 3 groups. Primary outcomes. Using the investigational drugs in children with various forms of acute respiratory infection made it possible to quickly reduce the intensity and frequency of cough by Day 3-5 from the onset of the disease and therapy, and transform cough into productive wet cough. The study showed rapid positive dynamics in cases, when acetylcysteine was prescribed from the first days of the disease, with a consistent cough therapy using herbal products. Both investigational drugs showed no adverse drug effects during the study. The results of the analysis brings us to the conclusion that a multimodal approach to cough treatment provides a comprehensive effect, which potentiates the treatment effect and allows us to recommend them in everyday pediatric practice as effective symptomatic agents to treat inflammatory diseases of the respiratory tract. This method helps to prevent polypharmacy, minimizing the drug load on the child.
Резюме Цель: изучить эффективность компрессорных небулайзеров в терапии респираторного поражения дыхательных путей у детей разных возрастных групп. Материал и методы: основную группу составили 48 детей-средний возраст 9,7 ± 4,3 лет, которым в ходе лечения была применена небулайзерная терапия, в контрольную группу вошли 41 ребенок-средний возраст 10,8 ± 4,7 лет, которым ингаляционная терапия не проводилась. Наряду с общепринятой терапией, детям основной группы проводили сеансы ингаляционной терапии с использованием компрессорного небулайзера в двух режимах в зависимости от клинической симптоматики пациента: при бронхитах курс составлял 5-6 дней, при пневмонии-до 8-9 дней. С использованием небулайзера вводились физиологический раствор, ипратропия бромид, беродуал, пульмикорт, лазолван в режиме постоянного потока, а в случаях диагностированного риносинусита использовался режим пульсирующей подачи аэрозоля. Результаты: при анализе клинической картины было выявлено, что у всех пациентов процесс принимал торпидное течение с длительным малопродуктивным кашлем, вязкой мокротой и стойко сохраняющимися аускультативными данными при незначительных сдвигах в общем анализе крови. Анализ полученных данных применения различных режимов лечения нозологических форм заболеваний у обследованных детей показал, что у пациентов основной группы клиническая симптоматика сочетания заболеваний легочной патологии и риносинуситов купировалась при применении режима пульсирующей подачи в сроки 6,1 ± 0,7 дней (р<0,05), в случаях изолированных вариантов бронхитов и пневмонии применение режима постоянного потока позволило сократить сроки госпитализации детей до 82 ± 7,4% от нормативных показателей койко-дней. Заключение: проведенные собственные исследования по эффективности ингаляционной терапии у детей выявили широкий диапазон клинического воздействия с помощью компрессорного небулайзера независимо от возраста пациента: клиническая симптоматика сочетания заболеваний легочной патологии и риносинуситов купировалась при применении режима пульсирующей подачи в более короткие сроки в сравнении с группой контроля без ингаляционной терапии, что позволило сократить сроки госпитализации детей в стационаре.
1920-1953 гг. Кафедра педиатрии-одна из первых кафедр Казанской государственной медицинской академии. Датой ее основания является включение детской больницы, организовать которую было поручено профессору Е.М. Лепскому, в состав Клинического казанского института (в последующем Казанского ГИДУВа), что произошло 15 июня 1920 г. (рис. 1). Создание больницы предшествовало созданию кафедры, которое произошло в 1919 г., когда Казан-ский губздрав поручил Е.М. Лепскому организовать детскую больницу и предоставил для нее одно из лучших, просторных зданий в центре Казани-бывший особняк купца Оконишникова на ул. Комлева (рис. 2). Со свойственной ему энергией Ефим Моисеевич горячо взялся за выполнение этого поручения. Время было трудное: еще не закончилась гражданская война и шли бои на поль ском фронте; в стране царила разруха, испытывалась нужда буквально во всем; не хватало подготовленных медицинских кадров.
