Резюме Цель: изучить эффективность компрессорных небулайзеров в терапии респираторного поражения дыхательных путей у детей разных возрастных групп. Материал и методы: основную группу составили 48 детей-средний возраст 9,7 ± 4,3 лет, которым в ходе лечения была применена небулайзерная терапия, в контрольную группу вошли 41 ребенок-средний возраст 10,8 ± 4,7 лет, которым ингаляционная терапия не проводилась. Наряду с общепринятой терапией, детям основной группы проводили сеансы ингаляционной терапии с использованием компрессорного небулайзера в двух режимах в зависимости от клинической симптоматики пациента: при бронхитах курс составлял 5-6 дней, при пневмонии-до 8-9 дней. С использованием небулайзера вводились физиологический раствор, ипратропия бромид, беродуал, пульмикорт, лазолван в режиме постоянного потока, а в случаях диагностированного риносинусита использовался режим пульсирующей подачи аэрозоля. Результаты: при анализе клинической картины было выявлено, что у всех пациентов процесс принимал торпидное течение с длительным малопродуктивным кашлем, вязкой мокротой и стойко сохраняющимися аускультативными данными при незначительных сдвигах в общем анализе крови. Анализ полученных данных применения различных режимов лечения нозологических форм заболеваний у обследованных детей показал, что у пациентов основной группы клиническая симптоматика сочетания заболеваний легочной патологии и риносинуситов купировалась при применении режима пульсирующей подачи в сроки 6,1 ± 0,7 дней (р<0,05), в случаях изолированных вариантов бронхитов и пневмонии применение режима постоянного потока позволило сократить сроки госпитализации детей до 82 ± 7,4% от нормативных показателей койко-дней. Заключение: проведенные собственные исследования по эффективности ингаляционной терапии у детей выявили широкий диапазон клинического воздействия с помощью компрессорного небулайзера независимо от возраста пациента: клиническая симптоматика сочетания заболеваний легочной патологии и риносинуситов купировалась при применении режима пульсирующей подачи в более короткие сроки в сравнении с группой контроля без ингаляционной терапии, что позволило сократить сроки госпитализации детей в стационаре.
Резюме Введение. Кашель оказывает влияние на качество жизни и является наиболее частой причиной обращения пациентов за медицинской помощью. У детей установление патофизиологической причины состояния, связанного с кашлем, имеет особое значение для постановки адекватного диагноза и назначения правильной терапии. Цель. Оценить эффективность и безопасность применения бутамирата в комплексном лечении дошкольников с острым бронхитом. Материалы и методы. Инициативное исследование проводилось в соответствии с требованиями Хельсинкской декларации и включало 63 пациента с симптомом сухого кашля в возрасте 3-7 лет с диагнозом «острый бронхит». Эффективность терапии оценивали с применением балльной системы. В основной (1) группе-21 ребенок-использовали ненаркотический противокашлевой препарат центрального действия бутамират по 5 мл сиропа 3 раза в сутки перед едой 4,7 ± 0,6 дня. В основной (2) группе-21 ребенок-применяли бутамират по 25 капель внутрь 4 раза в сутки 4,9 ± 0,8 дня. В группе сравнения-21 ребенок-применяли противокашлевые средства согласно протоколу лечения острого бронхита. Результаты. У 80,3 ± 5,4% пациентов основных (1 и 2) групп к четвертому дню лечения бутамиратом по отношению к группе сравнения улучшилось самочувствие (0,5 ± 0,07 балла, р = 0,032), уменьшилась интенсивность кашля (0,1 ± 0,01 балла, р = 0,015), нивелировался сухой кашель (0,3 ± 0,02 балла, р = 0,023), сократился кашлевой период при изнуряющем кашле (0,2 ± 0,01 балла, р = 0,017). Статистически значимых различий между обеими основными группами не выявлено. Бутамират ни в одном случае не вызывал угнетения дыхания, нарушений когнитивных функций, не оказывал седативного действия и лекарственной зависимости. В группе сравнения у пациентов самочувствие улучшалось менее интенсивно, характеристики сухого кашля сохранялись более длительно. Выводы. Бутамират является эффективным и безопасным средством в терапии сухого кашля при остром бронхите у дошкольников.
Introduction. Cough accompanies an acute infectious process in the respiratory tract. The article focuses on the symptomatic treatment of cough using a modern herbal preparation with complex action and standard therapy.Goal. To study the effects of a herbal preparation with complex action in the treatment of cough in children.Materials and methods. The study included 72 patients of both sexes (mean age 14.6 ± 2.3 years) with a symptom of unproductive/unproductive cough, against the background of acute bronchitis or community-acquired pneumonia.Results and discussion. When using a herbal preparation with complex action was used in ¾ of the children, cough decreased by the 3rd day and in most patients it stopped by the 10 th day. The severity of daytime and nighttime coughs decreased by 3.0–3.5 points, the number of seizures decreased to 2–4 per day, the duration of seizures also decreased significantly, the median of this indicator in the main group reached 0 points. In both groups, the proportion of patients with seizure frequency of no more than 4 per day increased. While taking a herbal preparation, a more pronounced decrease in all indicators of cough by 3.5 times was noted throughout the treatment, and the corresponding effects developed 3 days ahead of time against the background of a full course of therapy. The average value of the efficacy and safety assessment given by the medical researchers to the action of the herbal preparation with complex action was 3.6 points. Analysis of objective and subjective data based on the results of evaluating case histories and individually developed questionnaires for patients on drug tolerance showed equivalence in terms of excellent and good results. Against the background of the full course of treatment with the herbal preparation, a 1.8 times more pronounced decrease in the characteristics of the cough syndrome was noted.Conclusion. The drug has shown high efficacy and safety, which makes it possible to recommend it as a symptomatic therapy for cough in school-age children as part of a combination or monotherapy.
