Одним из очевидных преимуществ ингаляци онной анестезии является простота дозирования ос новного препарата. Эта простота обеспечивается способом доставки и фармакокинетикой, в частнос ти, севорана: в равновесном состоянии Et anesth при мерно соответствует концентрации препарата в ЦНС -органе мишени для любого анестетика, гип нотика и центрального анальгетика [1]. В этой связи, анестезиологами широко используется понятие ми нимальной альвеолярной концентрации (МАК)концентрации ингаляционного анестетика в конце выдоха, при достижении которой у 50% пациентов отсутствует двигательная реакция на стандартный болевой раздражитель (кожный разрез) [2]. МАКвесьма важная константа, характеризующая анесте Цель исследования -обоснование необходимости введения новой константы -МАК угнетения дыхания и определение ее величины. Материал и методы. Исследования выполнены у 43 пациентов в возрасте 20-45 лет, с нормальной мас сой тела и физическим статусом ASA I II. Всем больным проводили вводный наркоз севораном, индукция исключала использование наркотических анальгетиков, закиси азота и мышечных релаксантов. После достижения MAClm уста навливали ларингеальную маску. Повышали концентрацию севорана на испарителе, добиваясь изменения Et anesth . со скоростью 0,2 об% в мин до достижения Et anesthугнетения дыхания . Результаты. Обоснована необходимость введения новой константы ингаляционной анестезии -МАК угнетения дыхания , которая, в соответствии с концепцией МАК, рас сматривается как «концентрация ингаляционного анестетика, вызывающая необходимость в проведении вспомога тельной вентиляции у 50% пациентов». МАК угнетения дыхания составила 1,8 МАК. Это существенно больше и МАК и 1,3 МАК -концентрации анестетика, при которой возможно выполнение любой операции у 90% больных. Вместе с тем, полученная величина, 3,7 об%, несколько меньше МАК барр (4,07 об%), а значит, у малого количества больных моно наркоз севораном в условиях спонтанного дыхания окажется неадекватным, или его проведение будет сопряжено с риском развития гипоксии. Заключение. Последовательно развиваемая нами концепция анестезии с сохраненным спонтанным дыханием, диктует необходимость введения новой константы ингаляционной анестезии -МАК угнетения дыхания . Ее величина для севофлурана составляет 3,7 об%. Subjects and methods. Forty three patients aged 20-45 years, who had normal weight and ASA physical status I-II, were examined. All the patients underwent induction of sevorane anesthesia without narcotic analgesics, nitrous oxide, and myorelaxants. A laryngeal mask was installed after MAClm was achieved. Цель исследования-оптимизировать анестезиологическую тактику периоперационного ведения больных пожилого возраста при операциях на нижнем этаже брюшной полости путем применения сочетанной анестезии с сохраненным спонтанным дыханием без миоплегии. Материал и методы. Исследования выполнены у 142 х пациентов в возрастной группе старше 65 лет. Пациенты разделены на 2 е группы: 1 я-ингаляционная анестезия с тотальной миоплегией и ИВЛ в режиме CMV; 2 я-ингаляционная анестезия без миоплегии с сохраненным спонтанным дыханием или ВИВЛ в режиме PS. Результаты и обсуждение. Методика ВИВЛ позволяет провести сочетанную анестезию с сохраненным спонтанным дыханием у пожилых пациентов при операциях на нижнем этаже брюшной полости и магистральных сосу дах у 89% больных без риска получить неадекватный транспорт кислорода в условиях его (кислорода) относительно повышенного потребления. ИВЛ, проводимая для протезирования функции внешнего дыхания в условиях сочетанной анестезии и тотальной миоплегии, вызывает снижение сердечного индекса (на 58,8%; p<0,05), увеличение ОПС (на 50% и более; p<0,05) и увеличение внутрилегочного шунта в три раза (p<0,05). Проведение сочетанной анестезии без миоплегии и ИВЛ предупреждает развитие индуцированных изменений СИ, ОПС и Qs/Qt. Различия достоверны в те чение всего периода наблюдения (интраоперационный этап и 9 часов п/о периода). Больные старшей возрастной груп пы более чувствительны к повреждающему действию ИВЛ, у них же более выражен предупреждающий эффект моди фицированного метода анестезии. Заключение. Исключение миоплегии и ИВЛ в рамках проведения сочетанной анестезии предупреждает развитие ИВЛ индуцированных изменений СИ, ОПС и Qs/Qt. Subjects and methods. Examinations were made in 142 patients aged over 65 years. The patients were divided into 2 groups: 1) inhalation anes thesia under total myoplegia and controlled mechanical ventilation (MV); 2) inhalation anesthesia without myoplegia with preserved spontaneous breathing or assisted mechanical ventilation (AMV) in the pressure support (PS) mode. Results and discussion. The AMV procedure allows mixed anesthesia with preserved spontaneous breathing in elderly patients during operations on the lower abdomen and great vessels in 89% of the patients without a risk for inadequate transport of oxygen under its relative uptake. MV used to maintain external respiratory function under mixed anesthe sia and total myoplegia causes a decrease in cardiac index (CI) by 58.8% (p<0.05) and increases in total peripheral resis tance (TPR) by 50% or more (p<0.05), and intrapulmonary shunt threefold (p<0.05).
