Обоснование. В силу исторических обстоятельств Абхазия является одним из немногих регионов бывшего СССР, где до настоящего времени не было проведено исследований по обеспеченности питания населения йодом и не осуществляются мероприятия по профилактике йодного дефицита. Целью настоящего исследования была оценка обеспеченности питания йодом населения Абхазии (дети школьного возраста и беременные женщины) для выработки стратегии профилактических мероприятий. Методы. Всего в этих трех районах Абхазии было обследовано 222 человека, включая 151 ребенка школьного возраста (8-12 лет) и 61 беременную женщину в возрасте от 19 до 39 лет, у которых было исследовано содержание йода в моче, а также использование йодированной соли на уровне домохозяйств. Результаты. Результаты проведенного обследования показали крайне низкий уровень обеспеченности питания йодом на всей территории Абхазии. Так, у школьников медианная концентрация йода составляла всего 29,1 мкг/л, а у беременных женщин-26,5 мкг/л. При этом в некоторых кластерах медианная концентрация йода была ниже 20 мкг/л, что свидетельствует о возможности тяжелого йодного дефицита. Выводы. Результаты проведенного исследования дают основание заключить, что в Абхазии сложилась критическая ситуация в отношении обеспеченности питания населения йодом. Для устранения дефицита йода в питании по примеру государств Закавказья (Азербайджан, Армения, Грузия) в Абхазии необходимо принять срочные меры по частичному или полному запрету импорта, продажи и использования обычной соли. Due to historical reasons, Abkhazia is one of the few regions of the former Soviet Union where status of iodine nutrition of the population is still unknown and activities for the prevention of iodine deficiency are not carried out. Aim. To assess the status of iodine nutrtition of Abkhazian population (schoolchildren and pregnant women). Materials and methods. In total, 222 subjects were surveyed in Abkhazia, including 151 schoolchildren (age 8-12 years) and 61 pregnant women (aged 19-39 years). The content of iodine has been studied in the urine, and the use of iodized salt has been assesses at the household level. Results. The results of the survey revealed medium to severe iodine deficiency in Abkhazia. In schoolchildren median urinary iodine concentration was 29.1 mcg/l, and in pregnant women-26.5 mcg/l. In some clusters the median urinary iodine concentration was below 20 mcg/l indicating the possibility of severe iodine deficiency. Conclusion. Results of the study point to a critical situation with regard to status of iodine nutrition of the population in Abkhazia. To eliminate iodine deficiency, based on experience of other Transcaucasian states (Armenia, Azerbaijan, Georgia), Abkhazia should take urgent measures to introduce partial or complete ban on the import, sale and use of common (non-iodized) salt.
Йодный дефицит является важной медико-социальной проблемой для Кыргызстана. Результаты широкомасштабных исследований, проведенных под эгидой ВОЗ, ЮНИСЕФ и Глобальной сети по йоду (ГСЙ), показали, что за последние 10-15 лет охват йодированной солью и обеспеченность питания населения йодом улучшились [1]. Появление доступных ме-тодик, таких как определение концентрации йода в моче (КЙМ) и оценка объема щитовидной железы (ЩЖ) с помощью ультразвукового сканирования (УЗИ), помноженное на бескрайние просторы России, открыло невозделанное поле для эпидемио-логических исследований. Аналогичные процессы происходили и в других частях света, и, применяя марксистскую терминологию, происходящее в то время можно было бы назвать "первоначальным накоп лением капитала", если под капиталом пони-мать новые знания о природе и распространенности йодного дефицита.Опять же в силу бескрайности просторов Россия остается одной из немногих стран в мире, в которых в течение последних 25 лет не было проведено обще-национального исследования распространенности йодного дефицита. Вместо этого во многих регионах страны были выполнены (иногда неоднократно) авто номные проекты, включающие исследование КЙМ у школьников (реже -у других групп насе-ления) с расчетом медианной КЙМ (мКЙМ) или, просто говоря, "медианы йодурии". Несмотря на не-достатки такого подхода (отсутствие единых крите-риев выбора кластеров и целевых групп населения), уже к началу нулевых годов исследования были про-ведены во всех федеральных округах и большинстве крупных регионов и показали, что практически на всей территории Российской Федерации потребле-ние йода было недостаточным [1]. Снижение мКЙМ выявили даже там, где его меньше всего ожидали, -на берегу Черного моря и у жителей острова Сахалин, за исключением небольшой популяции корейцев, традиционно использующих в питании много море-продуктов и морских водорослей.Наиболее тяжелый йодный дефицит был выяв-лен в Туве, где среди воспитанников интернатов и домов ребенка были обнаружены недавние случаи йододефицитного кретинизма, а мКЙМ была ниже 20 мкг/л.
