У 62 хворих на ЕГ ІІ стадії з ІІ-ІІІ ступенем зростання АТ та 21 контрольної (нормотензивної) групи проводили добове моніторування АТ (ДМАТ) з використанням апаратного комплексу “Solvaig” (Україна). На час дослідження ДМАТ обстеженими проводився забір добової сечі. За даними ДМАТ визначали середні за активний, пасивний та добовий періоди величини систолічного (САТ), діастолічного (ДАТ) та середнього динамічного АТ (СДТ), а в добовій сечі - концентрацію іонів натрію та розраховували його екскрецію. Обстеження у хворих проводилось на фоні комбінованої двохкомпонентної антигіпертензивної терапії з використанням тіазидовго діуретика у фіксованій дозі (гіпотіазид 25 мг). Встановлено, що більшість обстежених хворих за циркадіанною структурою АТ мали ритм «не діперів» - ND (61,6%) та NP(12,9%), на відміну від контрольної (нормотензивної) групи де серед 21 обстеженого – 18 володіли добовим ритмом АТ – Dipper. СДТ у хворих наростав в оберненій залежності до величини добового індексу (ДІ). За даними коефіцієнту пресорного натрійурезу (КПН) встановлено, що тільки у хворих на ЕГ ІІ ст. з добовим ритмом АТ – Dipper він наближається до норми (1,5). У групах «не діперів» він значно нижчий від норми (ND – 1,30; NP – 1,28). Виявлені зміни вказують на недостатнє дозування діуретичної складової у комбінованому АГ лікуванні та перевагу у цих хворих об’ємзалежного варіанту ЕГ. Для оптимізації дози діуретика необхідно титраційним методом (по 12,5 мг гіпотіазиду) збільшити його дозу до досягнення КПН - 1,5.
