During 2007-2012 years the ?Diabetes mellitus? sub-programme reached the performance targets with the following results. Development of the State Diabetes Register covering all regions of the Russian Federation. The Register facilitates the continuous evaluation and planning of diabetes care in this country, analysis of epidemiological dynamics (including incidence and mortality in diabetes population) and human resources management.Alteration of the demographics of the national diabetes population, specifically: - an increase in life expectancy among T1DM patients: up to 56.7 years (+3.8 years) in males and up to 60.8 years (+4.1 years) in females; an increase in life expectancy among T2DM patients: up to 72.4 years (+3.2 years) in males and up to 74.5 years (+2.9 years) in females; - a reduction in mortality of diabetes patients for 28.4%; Introduction of novel technologies for diagnostics, prevention and management of DM and its complications, that, in turn, allowed to: - decrease the incidence of diabetic nephropathy for 16.1% and extend the pre-dialysis period; - decrease the incidence of diabetic retinopathy for 11.4% and preserve vision in 86-91% patients who still develop this complication; - decrease the frequency of upper- and lower-level amputations for 24% and 28%, respectively, and to reduce the ulcer healing times 2.5 to 3-fold;Introduction of safe and highly efficient human recombinant insulin and its analogues in the regions of the Russian Federation, along with modern methods of insulin administration and outpatient blood glucose monitoring.Establishment of diabetes educational centres (more than 1100 in the regions of the Russian Federation) that resulted in substantial reduction of number and frequency of diabetes-associated hospital admissions.The sub-programme is estimated to have saved 6 727 596.9 thousand Russian rubles of the budgetary funds due to decrease in the incidence of microvascular complications. A total of 360 papers (including 35 in international medical journals), as well as 81 clinical guidance manuals and 23 monographs were published during the course of this sub-programme.
BACKGROUND: Type 2 diabetes mellitus (T2DM) is a disease with high medical and social impact due to its wide and ever-increasing prevalence, high hospitalization and mortality rates. Despite the whole range of therapeutic options, lifestyle intervention remains as important as ever since it underlies guidelines of T2DM management. AIMS: To evaluate the effect of intensive lifestyle management approach on body weight reduction and on long-term maintenance of the achieved results in patients with T2DM. MATERIALS AND METHODS: We performed multicenter, interventional, prospective, non-randomized 12 month study. One hundred thirty overweight (obese) patients with T2DM included in the study were divided into two groups: group of lifestyle intervention (n=100) and control group (n=30). Patients in the group of lifestyle intervention participated in the intensive lifestyle modification and body weight reduction programme, which consisted of components: change in diet plan, physical exercise programme, cognitive behavioral techniques, educational and medical support. Clinical assessment and completing of patient questionnaires were performed at baseline, after 3 and 12 months. Patients in the control group did not attend programme, their clinical data were recorded during clinical visits every 3 months. RESULTS: Ninety patients (90%) in the group of lifestyle intervention and twenty nine patients (96.7%) in the control group completed the study and fulfilled protocol. After 12 months, patients in the group of lifestyle intervention lost 5.8% of their baseline weight and patients in the control group showed 1.2% decrease of their baseline weight. Body weight loss 5% was observed in 49.2% of patients in the group of lifestyle intervention and in 12.9% of patients in the control group (OR=6.54 [2.01; 21.33]; р=0.002). By the end of observation, delta reduction of glycated hemoglobin (HbA1c) in the intervention group was higher than in the control group (0.79% and 0.11%, respectively), the odds ratio for lowering HbA1c 0.5% from baseline between two groups was 14.10 [3.24; 61.30] (p0.001) in favor of the group of lifestyle intervention. 58.3% patients in the group of lifestyle intervention with baseline level of HbA1c 7% (n=48) achieved a HbA1c target of 7%; in the control group two patients (13.3%) out of fifteen with baseline level of HbA1c 7% achieved the target after 3 months and returned to baseline by the end of observation. CONCLUSIONS: Our results suggest that the programme of intensive lifestyle modification in patients with T2DM is highly efficient and safe.

Изучение эпидемиологических характеристик синдрома диабетической стопы (СДС) представляет особую актуальность в связи с высоким риском ампутаций нижних конечностей у больных сахарным диабетом (СД).ЦЕЛЬ. Оценить эпидемиологические характеристики развития СДС и ампутаций нижних конечностей у взрослых па-циентов с СД 1 и 2 типа (СД1 и СД2) в РФ за период 2013-2016 гг.МЕТОДЫ. Объектом исследования является база данных Федерального регистра СД -81 региона РФ, включенных в систему онлайн-регистра. Оценивались показатели на 10 тыс. взрослых пациентов с СД (>18 лет).РЕЗУЛЬТАТЫ. В 2016 г. распространенность СДС в РФ составила: СД1 -4,7%, СД2 -1,9%, с выраженными межре-гиональными различиями -0,15-19,9%, 0,07-10,3% соответственно. Распространенность СДС в РФ в динамике 2013→2016 гг. имеет тенденцию к снижению: СД1 -506,3→473,6; СД2 -214,60→194,8/10 тыс. взрослых. Динамика но-вых случаев СДС/год стабильна при СД1 -20,8→20,4, отмечается рост при СД2 -13,2→14,2. Средний возраст разви-тия СДС увеличился на 2 года при обоих типах СД. Средняя длительность СД до манифестации СДС увеличилась: при СД1 -15,4→19,0 лет, СД2 -7,4→10,1 года. Соотношение различных форм СДС при СД1: нейропатическая с трофиче-ской язвой -41,6%, нейропатическая (стопа Шарко) -17,9%, нейроишемическая -28,3%, ишемическая -12,2%; СД2: 41,6%, 7,4%, 32,4%, 18,5% соответственно. Количество новых случаев ампутаций/год в динамике: СД1 -10,5→12,4, СД2 -9,6→10,9, с выраженной межрегиональной вариабельностью 0,13-2,9% при СД1, 0,04-6,0% при СД2. Средняя длительность СД до ампутации увеличилась: СД1 -18,4→21,3 года, СД2 -9,1→9,9 года. Средний возраст развития ам-путаций: СД1 -51,7 года, СД2 -66,2 года. Отмечается уменьшение доли высоких ампутаций: СД1 -43,6→37,0%, СД2 -52,2→45,5% за счет перераспределения в пользу ампутаций в пределах одного пальца стопы: при СД1 -4,0→10,0%, при СД2 -2,8→ 9,1%. ЗАКЛЮЧЕНИЕ.Динамика частоты новых случаев СДС у взрослых пациентов в РФ стабильна при СД1, при СД2 имеет тенденцию к повышению. Выраженные межрегиональные различия в частоте СДС и ампутаций между регионами могут быть обусловлены различиями в качестве оказания специализированной помощи, отсутствием или нехваткой кабинетов диабетической стопы, лечением пациентов с СДС в условиях общей хирургической практики в ряде реги-онов, что признано менее эффективной стратегией. Отмечается отчетливая положительная динамика уменьшения доли высоких ампутаций, развития СДС в более старшем возрасте и при большей длительности СД, что отражает повышение эффективности мер профилактики поражения нижних конечностей при СД.
