Neuroinflammation is a complex inflammatory process in the central nervous system, which is sought to play an important defensive role against various pathogens, toxins or factors that induce neurodegeneration. The onset of neurodegenerative diseases and various microbial infections are counted as stimuli that can challenge the host immune system and trigger the development of neuroinflammation. The homeostatic nature of neuroinflammation is essential to maintain the neuroplasticity. Neuroinflammation is regulated by the activity of neuronal, glial, and endothelial cells within the neurovascular unit, which serves as a “platform” for the coordinated action of pro- and anti-inflammatory mechanisms. Production of inflammatory mediators (cytokines, chemokines, reactive oxygen species) by brain resident cells or cells migrating from the peripheral blood, results in the impairment of blood-brain barrier integrity, thereby further affecting the course of local inflammation. In this review, we analyzed the most recent data on the central nervous system inflammation and focused on major mechanisms of neurovascular unit dysfunction caused by neuroinflammation and infections.

Streptococcus pneumoniae, a common human pathogen, colonizes the nasopharynx and causes diseases including acute otitis media (AOM). Herein, pneumococcal serotype distributions in children before and after PCV7 vaccination and in patients with pneumococcal disease in Siberian Russia (Krasnoyarsk) are reported. Analyses included antimicrobial susceptibility testing, sequence typing (ST), pulsed field gel electrophoresis, virulence‐related surface protein gene (VSG) typing with novel primers and structural analysis by scanning electron microscopy. In healthy children (HC) prior to administration of PCV7, drug‐susceptible serotype23F/ST1500 was a major pneumococcal genotype. In the PCV7 trial, multidrug‐resistant serotype19A/ST320 emerged in vaccinees after PCV7, exhibiting a PCV7‐induced serotype replacement. Multidrug‐resistant serotype19A/ST320 was evident in patients with AOM. Community‐acquired pneumonia (CAP) isolates showed genetic similarities to the AOM (ST320) genotype, constituting a common non‐invasive AOM–CAP group. In contrast, meningitis isolates were more divergent. Overall, 25 ST types were identified; five (20%) of which were Krasnoyarsk‐native. Regarding VSGs, PI‐1 (rlrA/rrgB), PI‐2 (pitA/B), psrP and cbpA were present at 54.3%, 38.6%, 48.6%, and 95.7%, respectively, with two major VSG content types, PI‐1−/PI‐2−/psrP +/cbpA + and PI‐1+/PI‐2+/psrP ‐/cbpA +, being found for HC and non‐invasive diseases, respectively. A major clone of serotype19A/ST320 (PI‐1+/PI‐2+) produced the longest pneumococcal wire (pilus) structures in colonies. ST1016 (PI‐1−/PI‐2−) in HC had HEp‐2 cell‐adherent pili. These results suggest that serotype19A/ST320 and related genotypes, with the VSG content type PI‐1+/PI‐2+/psrP −/cbpA +, emerged in vaccinees after PCV7 in Siberia, accompanying diseases in non‐vaccinated children, and that some genotypes (serotypes19A/ST320 and 18/ST1016) produced novel pneumococcal structures, predicting their roles in colony formation and adherence.
Резюме Ротавирусы-ведущий этиологический фактор кишечных инфекций (КИ). Эпидемиологические исследования подтверждают высокую частоту распространенности ротавирусных гастроэнтеритов у детей до 5 лет. ВОЗ с 2009 г. рекомендует включение рутинной вакцинации против ротавирусной инфекции (РВИ) в национальные программы иммунизации всех стран. В статье представлены результаты внедрения вакцинации против РВИ у детей г. Ачинска Красноярского края. Цель исследования: оценить эффективность внедрения региональной программы иммунизации детей, проживающих на территории г.
Мезиальный темпоральный склероз (МТС) является наиболее распространенной гистопатологической аномали-ей, обнаруженной у взрослых больных с фармакорезистент-ной формой височной эпилепсии [1,2]. У пациентов, пере-несших хирургическую резекцию гиппокампа различной этиологии, МТС выявлен в 33,6% случаев, а сочетанная па-тология (МТС и корковые мальформации, опухоли, сосуди-стые поражения, рубцовые изменения) -в 5,1% [3]. Тем не менее в настоящее время нет достоверной информации о распространенности мезиальной височной эпилепсии. На основании амбулаторного исследования взрослых пациен-тов с височной эпилепсией показано, что в 25% случаев при магнитно-резонансной томографии (МРТ) визуализирова-на атрофия гиппокампа [3,4]. Доля пациентов с признака-ми атрофии гиппокампа, направленных в эпилептические центры для предоперационной оценки фармакорезистент-ности височной эпилепсии, составляет примерно 70% [4]. Также описаны семейные случаи МТС, но без преобладания стороны поражения и полового различия. О нейродегенера-тивном повреждении клеток гиппокампа сообщалось при обследовании пожилых пациентов, не страдающих эпилеп-сией, но имеющих гипоксически-ишемическое поражение мезиальных отделов височных долей [5,6].Однако этиология МТС по-прежнему остается спор-ным вопросом, хотя большинство исследователей считают его приобретенной патологией многофакторного генеза.Современная гипотеза А. Мейера предполагает, что инициирующее событие, травма или повреждение, в частно-сти сложные и продолжительные фебрильные приступы на первом году жизни, способны в дальнейшем, спровоцировать развитие МТС [7]. Изолированный судорожный приступ, особенно продолжительный, или эпилептический статус, мо-гут повредить незрелые клетки гиппокампа [8]. Эксперимен-тально височная эпилепсия может быть индуцирована после длительного фебрильного приступа [9,10], а такое крупное исследование, как FEBSTAT, подтвердило развитие МТС (в соответствии с МРТ-критериями) после фебрильного эпи-лептического статуса [11,12]. Однако ни эпилептический ста-тус, ни продолжительные повторные фебрильные приступы, в большинстве случаев не могут привести к развитию МТС,
Modern methods of diagnosis and treatment of infectious diseases should be based on reliable data obtained through various methods of laboratory research. The main characteristics of the methods used are high sensitivity (the ability to analyze a small amount of sample, 10–6–10–4 g/ml), selectivity, reproducibility and others. Proteomic methods of research satisfy all the principles of evidence-based medicine. The advantages of using these methods to identify biomarkers (identifying proteins with altered expression levels), timely diagnosis and treatment of infectious diseases, described in the article, are obvious. Their introduction into practice is an element of personalized medicine.
The paper presents evidence of the relevance of pneumococcal infection (PI) in children of Krasnoyarsk. The most severe form of PI is pneumococcal meningitis that characterizes by lesion of not only meninges but brain substance, extreme severity, long-term nonsmooth flow with the development of complications that threaten a patient's life. The inclusion of pneumococcal conjugate vaccine in the planned childhood immunization scheme will significantly reduce the severity of these diseases.
