Изучить влияния милдроната (М) на эффективность лечения больных ишемичесой болезнью сердца (ИБС), получающих стандартную антиангинальную терапию (ААТ). Материалы и методы. Двойное слепое рандомизированное, плацебоконтролируемое, проводимое в параллельных группах исследование. Все пациенты продолжали принимать без изменений назначенную им ранее ААТ. По окончании контрольного периода (10-14 дней) проводилась рандомизация пациентов либо в группу лечения (прием М по 500 мг 2 раза/день) либо в контрольную группу (прием плацебо (Пл) 2 раза/день), период лечения-6 недель. Критерий эффективности терапии: изменение продолжительности пробы с физической нагрузкой на тредмиле (ТФН), частоты приступов стенокардии (ПС) и необходимости приема нитроглицерина (НТГ). ТФН проводился в начале (ТФН-1) и в конце контрольного периода (ТФН-2), а также в конце периода лечения (ТФН-3). Результаты. ТФН-1 и ТФН-2 имели хорошую воспроизводимость, их продолжительность достоверно не отличалась. В конце периода лечения на фоне дополнительной терапии М регистрировался достоверный прирост продолжительности ТФН-3 (р = 0,02), в то время как при приеме Пл прирост продолжительности ТФН не носил достоверного характера (р = 0,07). Во время периода лечения отмечалось достоверное снижение количества ПС как на фоне приема М (р = 0,002), так и на фоне приема Пл (р = 0,02). Однако на фоне приема М отмечалось достоверное снижение потребности в дополнительном приеме НТГ (р = 0,02), тогда как на фоне приема Пл потребность в НТГ в целом не изменилась (р=0,7). Количество побочных явлений, зарегистрированных на фоне приема М и Пл, достоверно не отличалось. Заключение. Применение милдроната предоставляет дополнительные возможности для терапии пациентов ИБС со стабильной стенокардией напряжения, у которых не достигается желаемый эффект на фоне подобранной ААТ.
